Východiska: Císařský řez patří mezi nejčastější porodnické operace, kterými je ukončován porod a těhotenství ve třetím trimestru. Jde o chirurgický výkon, a proto jako každá operace skýtá svá rizika jak pro matku, tak pro novorozence. Po provedeném císařském řezu je dle studií možné pozorovat souvislost mezi císařským řezem a vznikem gynekologických potíží, komplikací v následujících graviditách a dysfunkcí pánevního dna u matky. Cíl: Hlavním cílem přehledové práce byla prezentace a sumarizace publikovaných poznatků o problematice císařského řezu z hlediska rizik pro matku. Metody: K vyhledávání literárních zdrojů byly využity elektronické informační databáze PubMed, EBSCO, Medvik (BMČ) a databáze Google Scholar. Dokumenty byly vyhledávány ve stanoveném období 2007-2018 pomocí klíčových slov ověřených v třídníku tezauru Medical Subject Headings a kombinování pomocí Booelovských operátorů. Výsledky: Z hlediska krátkodobých rizik je plánovaný císařský řez srovnatelný s vaginálním porodem. Akutní císařský řez je však provázen vyšším výskytem krevních ztrát, viscerálních poranění, infekčních a tromboembolických komplikací. Mezi dlouhodobá rizika, na která může mít císařský řez vliv, lze zařadit gynekologické potíže jako je intermenstruační spotting, dysmenorea a pelvialgie. Významná rizika představuje císařský řez pro následující těhotenství. Závěry: Mnoho studií zabývající se riziky císařského řezu ukazuje rozpoluplné výsledky. V souvislosti se zvyšujícím se počtem císařských řezů jsou zapotřebí další a podrobnější výzkumy, které by pomohly osvětlit další souvislosti. Nelze také zapomínat na to, že císařský řez může představovat rizika nejen pro matku, ale i pro novorozence.

It is the surgical procedure and like any other operation, it can pose risks to both, the mother and the newborn child. According to current studies, after the caesarean section is perfermed it is possible to observe a link between the operation and occurence of gynecological problems, complications in subsequent pregnancies, and dysfunction of mother ́s pelvic floor. Aim: The main aim of this review was to present and summarize the published findings of the issue of caesarean section from the perspective of maternal risks. Methodology: Electronic information databases PubMed, EBSCO, Medvik (BMČ), and Google Scholar were used for searching literal sources. Documents from the period of 2007-2018 were searched using keywords verified in the Medical Subject Headings thesaurus and combined via using Booel ́s operators. Results: In the perspective of short-term risks, planned CS is comparable with vaginal birth. However, acute SC is associated with a higher incidence of blood loss, visceral injury, infectious and thromboembolic complications. The long-term risks of CS consist of gynecological problems such as intermenstrual spotting, dysmenorrhoea, and pelipathy. CS also poses significant risks for next pregnancies. Conclusion: Many studies dealings with the risks of caesarean section show inconsistent results. In the context of the increasing number of caesarean sections, more detailed research is needed to clarify this area of interest. It should also be noted that caesarean section may pose risks not only to the mother but also to the newborn child.

Caesarean section is currently the most frequently performed intervention after episiotomy in obstetrics and one of the most common abdominal operations overall. Rates of caesarean section have been rising globally. Given the increasing rate worldwide it is therefore necessary and important to understand how caesarean section affects child development. Attention-deficit/hyperactivity disorder (ADHD) is the most common neurobehavioural disorder in children. ADHD is characterized by a combination of symptoms including inattention, impulsivity and hyperactivity. Caesarean section may affect psychological development through changes in microbiota or stress response, and birth by caesarean section can be associated with a small increased risk of ADHD. In the current literature, there is no systematic review or protocol of the systematic review answering the question of whether the mode of delivery has influence on the risk of ADHD development.The objective of this review is to synthesize the best available evidence regarding the epidemiological association between the mode of delivery (caesarean section versus vaginal delivery) as exposure and ADHD as the outcome.A three-step strategy will be utilized in this review, aiming to find both published and unpublished studies. The initial search will be conducted using the MEDLINE, CINAHL and EMBASE. The second search will involve 21 databases and sources. Following the Preferred Reporting Items for Systematic Review and Meta-analysis statement analysis of title, abstracts and full texts, critical appraisal and data extraction will be carried out on selected studies using standardized instruments developed by Joanna Briggs Institute. All steps will be performed by two independent reviewers. If possible, statistical meta-analysis using Joanna Briggs Institute within the System for the Unified Management, Assessment and Review of Information will be pooled. Statistical heterogeneity will be assessed.The results will be disseminated by publishing in a peer-reviewed journal. Ethical assessment is not needed - we will search/evaluate the existing sources of literature.
Úvod: Endometrióza patří mezi nejčastější gynekologická onemocnění u žen v reprodukčním věku. Hlavními klinickými projevy jsou bolest a neplodnost, které mohou následně negativně ovlivnit fyzickou, psychickou a emoční stránku, a tedy i celkovou kvalitu života ženy. Cílem předkládané práce je přehledně shrnout současné základní poznatky týkající se problematiky endometriózy se zaměřením na epidemiologii, rizikové faktory, etiologii, diagnostiku, klasifikaci, léčbu a vliv na plodnost a kvalitu života ženy. Metodika: Při tvorbě nesystematického literárního přehledu bylo provedeno vyhledávání podle předem určených klíčových slov. Jednotlivá klíčová slova byla kombinována pomocí booleovských operátorů. Zahraniční a české zdroje, které byly v práci použity, byly vyhledávány v databázích PubMed, EBSCO, UpToDate a Medvik. Vyhledávání nebyl dán striktní časový a jazykový limit. Převážně jsme se soustředili na články od roku 2008 do prosince roku 2017 v anglickém a českém jazyce. Zahrnujícím kritériem práce byly ženy s onemocněním endometrióza. Výsledky: Pro tvorbu výsledků práce bylo použito celkem 55 zdrojů, z nichž dva jsou systematická review (10, 36), tři doporučené postupy (29, 32, 54), sedm prospektivních kohortových studií (2, 4, 37, 38, 42, 46, 53), šest studií případů a kontrol (3, 7, 11, 14, 34, 52), sedm průřezových studií (9, 13, 25, 39, 41, 47, 49), 18 literárních přehledů (1, 5, 8, 12, 15, 16, 17, 20, 22, 23, 24, 26, 33, 34, 40, 49, 51, 55), čtyři série případů (18, 19, 44, 48), jedna kvalitativní studie (zakotvená teorie) (27) a šest knižních publikací (6, 21, 30, 31, 43, 45). Závěr: Endometrióza patří mezi časté gynekologické onemocnění postihující ženy v reprodukčním věku. U žen se z velké části podílí na vzniku neplodnosti a zároveň tak ovlivňuje i jejich psychiku a kvalitu života v mnoha oblastech. V rámci léčby je třeba k pacientkám přistupovat individuálně podle převažujících symptomů. Je doporučováno pacientky odesílat do center, která se zabývají léčbou i pokročilejších forem endometriózy. Prozatím žádná z dnes nabízených možností terapie nezaručuje úplné vyléčení a rekurence onemocnění dosahuje stále vysokých čísel.
Introduction: Endometriosis is one of the most common gynaecological diseases amongst women in reproductive age. The main clinical manifestations include pain and infertility, which can subsequently adversely affect the physical, mental and emotional well being and the overall quality of life of the woman. The aim of the presented review is to summarize the current knowledge about endometriosis with focus on epidemiology, risk factors, aetiology, diagnostics, classification, treatment and the effect on fertility and quality of life of women. Methodology: For a non-systematic literature review, the search was performed based on predefined keywords. The individual keywords were combined using Boolean operators. Foreign and Czech sources in the PubMed, EBSCO, UpToDate and Medvik databases were used. The search was not given a strict time limit and language limit. However, we focused mainly on articles from 2008 to December 2017 in English and Czech. Women with endometriosis was the only inclusion criterion. Results: A total of 55 sources were used to generate results, 2 of which are systematic reviews (10, 36), 3 guidelines (29, 32, 54), 7 prospective cohort studies (2, 4, 37, 38, 46, 53), 6 case studies and controls (3, 7, 11, 14, 34, 52), 7 cross-sectional studies (9, 13, 25, 39, 41, 47, 49), 18 references (8, 12, 15, 16, 17, 20, 22, 23, 24, 26, 33, 34, 40, 49, 51, 55), 4 series of case studies (18, 19, 44, 48) (grounded theory) (27) and 6 books (6, 21, 30, 31, 43, 45). Conclusion: Endometriosis is a common gynaecological disorder affecting women in reproductive age. It largely involves infertility and at the same time it affects women’s psychological health and quality of life in many areas. Patients should be treated individually according to the prevailing symptoms. It is recommended that patients be sent to treatment centres, which offer more advanced care for endometriosis. At present, none of the therapy options at hand guarantees a full cure and recurrence of the disease is still high.

Fetální makrosomie je spojena se zvýšeným rizikem perinatální mortality a morbidity pro plod a zvýšeným rizikem poporodních poranění pro matku. Frekvence porodů těchto plodů se pohybuje mezi 1,3–1,5 %. Jako makrosomní bývají označené plody s hmotností vyšší než 4500 g. Diagnostika a predikce makrosomie respektive porodní váhy plodu je klíčová pro stanovení taktiky vedení porodu. Je založeno na kombinaci a posouzení anamnestických údajů, klinických parametrů a ultrazvukové biometrie. Za hranici pro elektivní císařský řez je považována odhadovaná hmotnost plodu u zdravých matek více než 5000 g. U matek s diabetem je vhodné zvážit indikaci k SC při hmotnosti plodu více než 4000 g.
Fetal macrosomia is associated with an increased risk of perinatal mortality and morbidity for both, mother and fetus. The frequency of these births is between 1.3 to 1.5%. Macrosomic fetuses are defined with a weight of 4500 g and more. Diagnosis and prediction of macrosomia or fetal birth weight is key to determine the tactics of delivery. It is based on the combination and assessment of medical history, clinical parameters and ultrasound biometry. The limit for elective caesarean section is considered the estimated fetal weight in healthy mothers of more than 5000 g. For mothers with diabetes an indication of the caesarean section should be considered for fetal weight of more than 4000 g.
