Membrane transporters are important determinants of drug bioavailability. Their expression and activity affect the intracellular drug concentration in leukemic cells impacting response to therapy. Pharmacogenomics represents genetic markers that reflect allele arrangement of genes encoding drug transporters associated with treatment response. In previous work, we identified SNP rs460089 located in the promotor of SLC22A4 gene encoding imatinib transporter OCTN1 as influential on response of patients with chronic myeloid leukemia treated with imatinib. Patients with rs460089-GC pharmacogenotype had significantly superior response to first-line imatinib treatment compared to patients with rs460089-GG. This study investigated whether pharmacogenotypes of rs460089 are associated with sustainability of treatment-free remission (TFR) in patients from the EUROpean Stop Kinase Inhibitor (EURO-SKI) trial. In the learning sample, 176 patients showed a significantly higher 6-month probability of molecular relapse free survival (MRFS) in patients with GC genotype (73%, 95% CI: 60-82%) compared to patients with GG (51%, 95% CI: 41-61%). Also over time, patients with GC genotype had significantly higher MRFS probabilities compared with patients with GG (HR: 0.474, 95% CI: 0.280-0.802, p = 0.0054). Both results were validated with data on 93 patients from the Polish STOP imatinib study. In multiple regression models, in addition to the investigated genotype, duration of TKI therapy (EURO-SKI trial) and duration of deep molecular response (Polish study) were identified as independent prognostic factors. The SNP rs460089 was found as an independent predictor of TFR.

Přehledový článek, věnující se diferenciální diagnostice anémií, jako jednomu z nejčastějších klinických symptomů napříč všemi obory medicíny. Jedná se o stav definovaný nižší hodnotou koncentrace hemoglobinu v erytrocytech, než je stanovená fyziologická dolní hranice. Z mnoha kritérií, podle kterých lze anémie rozdělovat, je v tomto článku použité morfologické dělení na základě velikosti erytrocytů na makrocytární, mikrocytární a normocytární a speciální skupinu věnující se anémiím s retikulocytózou v periferní krvi. Vzhledem k primárnímu zaměření na diagnostiku, je u každé definované skupiny věnován prostor patofyziologii, diagnostickým postupům a klinickému stavu, vzhledem k rozsahu je terapie zmiňována pouze v zásadních bodech. Vzhledem k multioborovému přesahu anémie a nutnosti správného stanovení její příčiny pro nastavení terapie, se tato problematika týká valné většiny oborů současné medicíny.
This work presents an overwiev od the differential diagnosis of anaemia - one of the most frequent clinical symptom in all fields of medicine. Anaemia is defined as an hemoglobin concentration in erythrocytes lower than normal range. There are various criteria for categorising anaemia and we have selected the morfological classification based on erythorocyte size, i.e. macrocytic, microcytic, normocytic. We have also included a special group of anaemia associated with peripheral reticulocytosis. This article thus mainly focuses pathophysiology and diagnostic tools, while treatment is discussed only at key points. Given the multi - disciplinary implications ans the necessity of correct diagnosis for subsequent appropriate treatment, this issue touches most fields of modern medicine.

Tento článek popisuje dvouleté zkušenosti s používáním přípravku midostaurin na našem pracovišti - v Ústavu hematologie a krevní transfuze (ÚHKT) v Praze. Po teoretickém úvodu a literárně publikovaných klinických zkušenostech je dále prostor věnován managementu léčby midostaurinem, klinickým zkušenostem, které vzešly z našeho souboru pacientů, krátkému zhodnocení dostupných výsledků a závěrem jsou popsány kazuistické případy. Midostaurin je cytostatikum ze skupiny inhibitorů FLT3 (FMS-like tyrosine kinase 3), který se v podobě tablet používá v kombinaci se standardní chemoterapií k léčbě nově diagnostikovaných akutních myeloidních leukemií s FLT3/ITD/TKD mutací. V současnosti se stále jedná o jediný přípravek z této skupiny, který má v České republice schválenou úhradu pro použití v 1-2 cyklech indukční léčby a ve 3-4 cyklech léčby konsolidační. Celkem jsme za dva roky midostaurin podali 18 pacientům v 37 cyklech. Dosažení kompletní remise, míra celkového přežití a přežití bez choroby se blíží údajům vycházejícím ze studie RATIFY. Při dodržování doporučení k léčbě midostaurinem je tento přípravek pacienty dobře tolerován. V kombinaci se standardní indukční a konsolidační léčbou nedochází v rámci našeho souboru k výraznému zvýšení rizika infekčních komplikací či jiných projevů toxicity nad obvyklý rámec léčby pacientů bez podávání midostaurinu.
This article describes our 2-year experience with the administration of midostaurin at the Institute of Hematology and Blood Transfusion (IHBT) in Prague. A short theoretical section and review of published data is followed by the clinical experiences with midostaurin treatment management in our group of patients and a short evaluation of available results. The article ends with some case reports. Midostaurin is an FLT3 (FMS-like tyrosine kinase 3) inhibitor used in the form of tablets in combination with standard chemotherapy in the treatment of patients with newly diagnosed acute myeloid leukemia (AML) and the FLT3/ITD/TKD mutation. To-date it remains the only preparation from this group registered and reimbursed in the Czech Republic for 1-2 cycles of induction therapy and 3-4 cycles of consolidation therapy. Over two years, we administered midostaurin to a total of 18 patients in 37 cycles of chemotherapy. Complete remission rate, overall survival and disease-free survival concur with the data from the RATIFY study. By adhering to the relevant recommendations, treatment with midostaurin is well tolerated. We did not record in our sample of patients treated with midostaurin and standard chemotherapy any increase in the risk of infectious complications or unusual toxicities compared to patients not receiving midostaurin.

The increasing interest in exploring the human genome and identifying genetic risk factors contributing to the susceptibility to and outcome of diseases has supported the rapid development of genome-wide techniques. However, the large amount of obtained data requires extensive bioinformatics analysis. In this work, we established an approach combining amplified fragment length polymorphism (AFLP), AFLP in silico and next generation sequencing (NGS) methods to map the malignant genome of patients with chronic myeloid leukemia. We compared the unique DNA fingerprints of patients generated by the AFLP technique approach with those of healthy donors to identify AFLP markers associated with the disease and/or the response to treatment with imatinib, a tyrosine kinase inhibitor. Among the statistically significant AFLP markers selected for NGS analysis and virtual fingerprinting, we identified the sequences of three fragments in the region of DNA repeat element OldhAT1, LINE L1M7, LTR MER90, and satellite ALR/Alpha among repetitive elements, which may indicate a role of these non-coding repetitive sequences in hematological malignancy. SNPs leading to the presence/absence of these fragments were confirmed by Sanger sequencing. When evaluating the results of AFLP analysis for some fragments, we faced the frequently discussed size homoplasy, resulting in co-migration of non-identical AFLP fragments that may originate from an insertion/deletion, SNP, somatic mutation anywhere in the genome, or combination thereof. The AFLP-AFLP in silico-NGS procedure represents a smart alternative to microarrays and relatively expensive and bioinformatically challenging whole-genome sequencing to detect the association of variable regions of the human genome with diseases.
