BACKGROUND: Status epilepticus (SE) is a severe acute condition in neurocritical care with high mortality. Searching for risk factors affecting the prognosis in SE remains a significant issue. The primary study's aim was to test the predictive values of the Clinical Frailty Scale (CFS) and the Modified 11-item Frailty Index (mFI-11), the biomarkers and basic biochemical parameters collected at ICU on the Glasgow Outcome Scale (GOS) assessed at hospital discharge (hosp), and three months later (3 M), in comatose patients with SE. The secondary aim was to focus on the association between the patient's state at admission and the duration of mechanical ventilation, the ICU, and hospital stay. METHODS: In two years single-centre prospective pilot study enrolling 30 adult neurocritical care patients with SE classified as Convulsive SE, A.1 category according to the International League Against Epilepsy (ILAE) Task Force without an-/hypoxic encephalopathy, we evaluated predictive powers of CFS, mFI-11, admission Status Epilepticus Severity Score (STESS), serum protein S100, serum Troponin T and basic biochemical parameters on prognosticating GOS using univariate linear regression, logistic regression and Receiver Operating Characteristic (ROC) analysis. RESULTS: Our study included 60% males, with a mean age of 57 ± 16 years (44-68) and a mean BMI of 27 ± 5.6. We found CFS, mFI-11, STESS, and age statistically associated with GOS at hospital discharge and three months later. Among the biomarkers, serum troponin T level affected GOS hosp (p = 0.027). Serum C-reactive protein significance in prognosticating GOS was found by logistic regression (hosp p = 0.008; 3 M p = 0.004), and serum calcium by linear regression (hosp p = 0.028; 3 M p = 0.015). In relation to secondary outcomes, we found associations between the length of hospital stay and each of the following: age (p = 0.03), STESS (p = 0.009), and serum troponin T (p = 0.029) parameters. CONCLUSIONS: This pilot study found promising predictive powers of two frailty scores, namely CFS and mFI-11, which were comparable to age and STESS predictors regarding the GOS at hospital discharge and three months later in ICU patients with SE. Among biomarkers and biochemical parameters, only serum troponin T level affected GOS at hospital discharge.

Cíl studie: Epidemiologie mimonemocniční náhlé zástavy oběhu (OHCA) v České republice a v Evropě není přesně známá. Proto byla realizována mezinárodní epidemiologická studie EuReCa ONE (European Registry of Cardiac Arrest) s cílem zjistit incidenci OHCA, charakteristiku procesu kardiopulmonální resuscitace (KPR) a výsledky KPR v Evropě. Studie se zúčastnila i Česká republika a v této analýze prezentujeme národní data. Typ studie: Prospektivní observační multicentrická epidemiologická studie. Typ pracoviště: Zdravotnická záchranná služba. Materiál a metoda: Studie se v České republice zúčastnily zdravotnické záchranné služby Středočeského, Královéhradeckého, Plzeňského, Karlovarského, Libereckého a Ústeckého kraje a Zdravotnická záchranná služba hlavního města Prahy. Do studie byli zařazeni všichni nemocní, kteří byli v období od 1. 10. 2014 do 31. 10. 2014 v uvedených regionech ošetřeni zdravotnickou záchrannou službou a kdykoliv během přednemocniční péče u nich byla stanovena diagnóza OHCA. Sledována a analyzována byla data v rozsahu poslední verze Utsteinského protokolu pro sledování pacientů s OHCA. Výsledky: Studie byla v České republice realizována v geografické oblasti o souhrnné populaci 4 532 758 obyvatel. Během sledovaného období byla incidence případů potvrzené OHCA s jakýmkoliv resuscitačním pokusem 98,19 případů / 100 000 obyvatel / rok. Defibrilovatelný iniciální rytmus byl zaznamenán pouze u 22,5 %. Incidence ROSC byla 30,18 případů / 100 000 obyvatel / rok. Třicetidenního přežívání anebo propuštění z nemocnice bylo dosaženo u 16,69 % všech pacientů s OHCA, u kterých byla provedena KPR, a incidence byla 16,36 případů / 100 000 obyvatel / rok. Ve všech ukazatelích byla zaznamenána klinicky relevantní mezikrajová variabilita. Závěr: Výsledky potvrzují, že OHCA je v České republice závažný zdravotnický problém s relativně vysokou incidencí a s klinicky relevantními interregionálními rozdíly.
Objective: At present, there is still a knowledge gap regarding the epidemiology of out-of-hospital cardiac arrest (OHCA) in Europe and in the Czech Republic. Therefore, the EuReCa ONE (European Registry of Cardiac Arrest) international epidemiological study was performed to determine the incidence of OHCA, the cardiopulmonary resuscitation process and OHCA outcome throughout Europe. Here we present national data from the Czech Republic. Design: Prospective, observational, multicentre, epidemiological study. Setting: Emergency medical services. Materials and methods: Emergency medical services of the Central Bohemia, Hradec Kralove, Pilsen, Karlovy Vary, Liberec and Usti nad Labem regions and the City of Prague participated in the study. All the patients who suffered an OHCA during the month of October 2014 attended by the Emergency Medical Service were eligible for inclusion in the study. The data were collected and analysed according to the latest version of the Utstein style. Results: Data covering a population of 4,532,758 inhabitants were analysed. Incidence of confirmed OHCA events with any kind of cardiopulmonary resuscitation attempt was 98.19 cases/100,000 inhabitants/year. Initial shockable rhythm was recorded in 22.5 % patients. Incidence of return of spontaneous circulation was 30.18 cases/100,000 inhabitants/year. Total 16.69 % patients survived for at least 30 days or to hospital discharge, equivalent to 16.36 cases/100,000/ inhabitants/year. Clinically relevant interregional variability was observed in all parameters. Conclusion: The results of EuReCa ONE confirm that OHCA is a major public health problem with considerable incidence and clinically relevant interregional differences.

Cíl studie: Kvantifikovat výskyt poresuscitačních traumat u pacientů po neúspěšně resuscitaci. Typ studie: Retrospektivní observační. Pracoviště: Krajská zdravotnická záchranná služba. Materiál a metoda: Do studie bylo zahrnuto 107 pitvaných pacientů po kardiopulmonální resuscitaci po netraumatické náhlé zástavě oběhu za rok 2015. U této skupiny jsme vyhledali pacienty, kteří měli v pitevním protokolu popsané závažné poresuscitační trauma. U těchto pacientů jsme provedli analýzu výjezdové dokumentace v systému ZZS. Tuto skupinu jsme dále porovnávali s celkovým počtem 394 kardiopulmonálních resuscitací v témže období. Výsledky: U 61 pitvaných pacientů po resuscitaci (tj. 57 %, respektive vztaženo k celkovému počtu resuscitací 15,5 %) byly při pitvě nalezeny traumatické změny, které nelze vykládat jinak, než jako následek kompresí hrudníku. Zároveň není možné považovat za jedinou příčinu nešetrnou laickou resuscitaci, 23 (37,7 %) resuscitací s traumatem bylo totiž provedeno pouze složkami integrovaného záchranného systému, ani biologická křehkost pacienta, protože 37 (60,7 %) těchto resuscitací proběhlo u pacientů mladších 70 let. Závěr: Je nutné další vzdělávání jak laiků, tak i zaměstnanců integrovaného záchranného systému s požadavkem přizpůsobení resuscitačního úsilí tělesné konstituci pacienta, zvláště u posádek záchranné služby. Další výzkum resuscitačních traumat je nezbytný.
Objective: Quantification of the incidence of CPR-related injuries in unsuccessfully resuscitated out-of-hospital cardiac arrest patients. Design: Retrospective observational study. Setting: Regional emergency medical services. Materials and methods: The study included the autopsy protocols of all 107 patients unsuccessfully resuscitated following non-traumatic out-of-hospital cardiac arrest in the year 2015. We identified all the patients with CPR-related trauma. These patients’ medical reports from the emergency information system were then analysed. They were compared to all the 394 resuscitations performed in the region in 2015. Results: CPR-associated major trauma of the chest and upper abdomen was found in 61 autopsy protocols (57% incidence in the deceased group; theoretically at least 15.5% incidence in all the resuscitations). This is most likely to be caused by chest compressions. Twenty three (37 %) of the resuscitations with post-CPR trauma were conducted by the rescue services only and 37 (60.7 %) of the resuscitated patients with post-CPR trauma were younger than 70 years, therefore not all the post-CPR trauma can be ascribed to CPR by lay-persons or the frailty of the patients. Conclusion: Further education of the general public in BLS and the adjustment of compressions to the body constitution of the particular patient need to be emphasized in the education of healthcare specialists, especially paramedics and EMS physicians. Further research of post-CPR trauma is necessary.
