Práce vychází z vyšetření 35 osob, z nichž 25 bylo při práci exponovano vibracím a 10 tvořilo kontrolní skupinu. Autoři porovnávali tyto diagnostické metody: prstovou pletyzmografii, termometrii a termografii. Při porovnávání považovali za základní kritérium poslední aktuální klinické vyšetření, zejména při něm provedený vodní chladový test. Též prováděli komplexní lékařské vyšetření včetně pracovní anamnézy a pracovnělékařského klinického vyšetření. Průměrný věk všech 35 vyšetřených byl 46,5 ± 12,1 roku. Ve skupině 25 exponovaných bylo 8 brusičů, 9 cídičů-apreterů, 3 cídiči-brusiči, 2 slévači, 1 kovář, 1 bílý zedník a 1 řidič nákladních vozů staršího typu. V kontrolní skupině bylo 10 pracovníků, z toho 6 lékařů, 1 zdravotní bratr, 2 laboranti a 1 technik. Celková průměrná doba expozice vibracím u souboru byla 18,12 ± 10,68 let. Denní průměrná expozice vibracím byla 3,64 ± 1,38 hodiny. Po zjištění nemoci byla většina exponovaných pracovníků přeřazena na jinou práci. Při porovnávání uvedených diagnostických metod v tomto malém souboru autoři konstatují, že termografie je názorná a užitečná metoda, která doplní vyšetřování onemocnění cév z vibrací a jiných onemocnění periferních cév . Není však závislá na tak dlouhém chlazení pacienta, jak ho vyžaduje Metodický pokyn MZ [48] pro ostatní diagnostické metody. Poměrně dobrá shoda (80%) je mezi aktuálním klinickým nálezem a provedenou termografií také při přípravě pacienta podle Chuckera. Velmi dobrou shodu zjistili autoři mezi pletyzmografií a termografií. Aktuální klinický nález vykazuje dobrou shodu jednak při porovnání s pletyzmografií, jednak s termografií. Neshodu mezi termometrií a aktuálním klinickým nálezem lze vysvětlit nejen tím, že před provedením termometrie byla použita metoda s ohřátím a ochlazením rukou pacienta podle Chuckera, ale také tím, že snímání termometrických hodnot na jednotlivých prstech bylo postupné, nikoli simultánní. Nevýhodou termometrie a termografie je časová náročnost obou metod. V další práci bude soubor vyšetřených pacientů rozšířen. Termografie se bude využívat také preventivně – při začleňování pracujících a učňů do práce v expozici vibracím.
The authors examined 35 individuals, including 25 men with professional exposure to vibrations and 10 men of a control group in our study. They compared these diagnostic methods: finger plethysmography, thermometry and thermography. The latest clinical examination, especially the water cooling test undertaken during this examination, was considered the basic criterion for comparison. The authors also carried on a complex medical examination including a professional history and an occupational clinical examination. The average age of all 35 examined individuals was 46.5 ± 12.1 years. There were eight grinders, nine polishers- -finishers, three polishers-grinders, two founders, one smith, one white bricklayer and one driver of lorries of the older type in the group of 25 exposed individuals. There were 10 workers in the control group, including six physicians, one nurse, two laboratory technicians and one technician. The total average time of the exposure to vibrations was 18.12 ± 10.68 years in our sample. The daily average exposure to vibrations was 3.64 ± 1.38 hours. The majority of exposed workers have been already shifted to the other work. Comparing presented diagnostic methods in this small sample; the authors can state that thermography is a practical and useful method, which completes the examination of vessel diseases caused by vibrations and other diseases of peripheral vessels. However, it does not require a long cooling of the patient, which is required in other diagnostic methods according to the Methodical Instruction of the MoH. There is a relatively good accordance (80 %) between a current clinical finding and undertaken thermography with the preparation of the patient according to Chucker. The authors found a very good accordance between plethysmography and thermography. The current clinical finding shows a good accordance both compared with plethysmography and thermography. The discordance between thermometry and the current clinical finding can be explained by the fact that we used the method with warming and cooling patient’s hands according to Chucker before undertaking thermometry and also by the fact that recording of thermometric values from particular fingers was consequent, but not simultaneous. The disadvantage is that thermometry and thermography are time consuming methods. The authors plan to extend the sample of examined patients in the next study. They will also use thermography for prevention in the process of placing workers and apprentices to working positions with exposure to vibrations.

U souboru 107 pracujících (95 exponovaných vibracím a 12 členů kontrolní skupiny) jsme sledovali stav periferních cév pomocí kožní prstové termometrie (KPT), prstové pletyzmografie (PP), manévru dynamické komprese celých prstů (MDK) a vodního chladového testu (VCHT). Celkem 95 pracujících exponovaných vibracím jsme rozdělili na skupinu 11 nemocných s onemocněním cév z vibrací 2. a 3. stadia a 84 pracujících bez klinických projevů nemoci. Kožní teplota byla měřena na bříšcích posledních článků všech prstů obou rukou před VCHT a po VCHT. Měření kožní teploty jsme prováděli bezkontaktním přístrojem ALMEMO 2230-8 bezprostředně před provedením VCHT a po provedení VCHT v časových intervalech 2 - 15 minut po dobu 90 minut. Zjistili jsme tyto závažné skutečnosti: - u zdravých osob a u onemocnění cév z vibrací 1. stadia se vrací kožní teplota k původní hodnotě před chlazením v době od 15 do 45 minut, u zdravých je vzestup křivky rychlejší; - u onemocnění cév z vibrací 2. stadia se kožní teplota začíná vracet až po 45 minutách, výjimečně dříve, u těžších stadií onemocnění se kožní teplota začíná vracet až za hodinu, popř. po delší době. U nemocných kožní teplota dlouho setrvává v rozmezí 22 - 25 °C, což je ovlivněno teplotou místnosti; - statisticky významné rozdíly v dobách návratu krve mezi jednotlivými skupinami šetřeného souboru byly nalezeny mezi kontrolní skupinou a skupinou nemocných onemocněním cév z vibrací 2. a 3. stadia; - u akrohypotermií je vzestup termometrické křivky rovněž pomalejší než u zdravých osob, u nemocných akrocyanózou nebo Raynaudovým syndromem pozorujeme zpravidla studené ruce a kožní teplotu pod 27 °C již před chlazením a značné kolísání a nepravidelnosti z průběhu termometrické křivky po chlazení; - u každého jedince mají křivky kožních teplot na všech prstech obou rukou včetně palců obdobný charakter průběhu, samozřejmě ve vztahu k míře expozice vibracím; - metoda prstové pletyzmografie velmi dobře dokladuje spazmus periferních cév. Také metoda kožní termometrie uspokojivě prokazuje spazmus periferních cév a současně též dobře vypovídá o individuální charakteristice kožních teplotních reakcí; - doporučujeme při prevenci u osob exponovaných vibracím provádět prstovou pletyzmografii jako metodu povinnou; - u diagnosticky obtížných a posudkově složitých případů doporučujeme doplnit vyšetření o kožní prstovou termometrii. Optimální je kombinace všech čtyř námi sledovaných metodik; Naše závěry ve vztahu k vyšetřovacím metodám u osob exponovaných vibracím též mohou přispět ke zkvalitnění diagnostiky onemocnění souvisejících s pracovním procesem v oblasti tak zásadní, jakou je závodní preventivní péče, a to jak při zvažování vhodného prvotního pracovního zařazení, tak i pro průběžné sledování zdravotního stavu. Smysl a význam preventivního přístupu zdůrazňuje nejenom legislativa stávající - zákon č. 20/1966 Sb. v platném znění (zejména §§6, 35 a 40), ale i nově přijatý zákon č. 258/2000 Sb., o ochraně veřejného zdraví.
In a group of 107 workers (98 exposed to vibrations and 12 controls) the state of peripheral vessels was investigated by means of digital skin thermometry, digital plethysmography, the manoeuvre of dynamic compression of whole fingers and the cold water test. 95 subject exposed to vibrations were divided into a group of 11 patients with vascular disease caused by vibrations, stage 2 and 3 and 84 subjects without clinical manifestations of the disease. The skin temperature was assessed on the bellies of the last phalanges of all fingers of both hands before and after the cold test. Temperature was assessed by means of a contact-free apparatus ALMEMO 2230-8 closely before the cold test and after the cold test at time intervals of 2-15 minutes for 90 minutes. The following important facts were revealed: • In healthy subjects and in vascular disease due to vibrations stage 1 the skin temperature returns to the normal level before cooling within 15-45 minutes, in healthy subjects the rise of the curve is more rapid. • In vascular disease caused by vibrations stage 2 the skin temperature starts to return after 45 minutes, in exceptional instances sooner, in severe stages of the disease the skin temperature is restored after one hour or later. In sick subjects the skin temperature remains for a long time between 22-25 degrees C, depending on the room temperature. • Statistically significant differences in the times of return of blood in different groups were found between the control group and the group with vascular disease caused by vibrations stage 2 and 3. • In acrohypothermia the rise of the thermometric curve is also slower than in healthy subjects. In patients with acrocyanosis or Raynaud’s syndrome cold hands and a temperature below 27 degrees C is found as a rule already before cooling and there are considerable fluctuation and irregularities in the course of the thermometric curve after cooling. • In every subject the skin temperature curves on all finger incl. thumbs have a similar character in response to the exposure to vibrations. • The method of digital plethysmography provides evidence of spasms of peripheral vessels. The method of skin thermometry provides also reliable evidence of spasms of peripheral vessels and at the same time it provides information on individual characteristics of skin temperature reactions. • The authors recommend to use during prevention in subjects exposed to vibrations digital plethysmography as a mandatory method. • In diagnostically difficult and complex cases they recommend to supplement the examination by dermal digital thermometry. A combination of all four methods described by the authors is optimal. • The conclusions of examination methods in subjects exposed to vibrations may contribute to improvement of the diagnosis of diseases associated with the working process in such fundamental fields as occupational preventive care. This may influence enlistment at work and monitoring of the health status. The impotance of preventive approach is emphasized not only by contemporary legislation - no. 20/1966 (in particular §§ 6, 35 and 40) but also the new law no. 258/2000 on public health protection.
V této kazuistice popisujeme otravu olovem u dvou majitelů střelnice, kteří neodborně zpracovávali staré náboje. Podmínky pro oba nemocné byly přibližně stejné, příznaky otravy se lišily. Subjektivní potíže odezněly po aplikacích dvou injekcí Chelintoxu, laboratorní známky otravy však přetrvávají dosud.
In the submitted case-history the authors describe lead poisoning in two owners of shooting galleries who processed old cartridges unprofessionally. The conditions of both patients were similarl, the symptoms of intoxication differed. The subjective complaints receded after two injections of Chelintox, tfie laboratory signs of intoxication, however, still persist.
