Kazuistika popisuje 43letou ženu, která byla vyšetřována pro dolní dyspeptický syndrom s nechutenstvím a váhovým úbytkem. Při endoskopickém vyšetření byly odebrány vzorky sliznice na histologii s průkazem Whippleovy choroby. Pacientka přeléčena roční antibiotickou terapií, po které došlo ke zlepšení klinického stavu, úpravě laboratorních nálezů k normě, vysazení medikace kromě substituce hypothyreózy. V dalším sledování byla pacientka bez recidivy onemocnění.

Case study describes 43 years old women, who was examined for lower dyspeptic syndrome accompanied by lack of appetite and weight loss. Mucosa samples were collected during endoscopic examination and histology confirmed the diagnosis of Whipple disease. The patient was treated by one year long antibiotic therapy after which the clinical state improved, laboratory valves returned to normal and medication was discontinued except substitution for hypothyreosis during follow-up there was no relace of the disease.

Setkání s pacientem s eosinofilní esofagitidou (EoE) vyvolalo potřebu zjistit o tomto onemocnění co nejvíce. V první části sdělení najdete přehled recentních informací o eosinofilní esofagitidě, onemocnění s narůstající prevalencí, které je možná další manifestací atopie, prakticky jistě však dalším příkladem chronického neinfekčního zánětlivého procesu, a bude potřebné na ně myslet. Následuje začátek příběhu chlapce, u kterého bylo toto onemocnění diagnostikováno. Kolem EoE i dalšího vývoje onemocnění u dítěte zůstává mnoho nejasného.

Meeting the boy with eosinofilic esophagitis (EoE) invoked a need to find as much information about this disease as possible. In the first part of this article you can find reviews of the most recently published information about this disease with the increasing prevalence . EoE is maybe another manifestation of atopy, undoubtedly another type of chronic non-infectious inflammatory process and we need to keep thinking about this disease.
