Backgroung and Aim. This work builds on our publication on the subject of creating a mathematical model for calculating the volume of proplaped soft tissue of the orbit in blow-out fractures, which aids us greatly in our choice of the most effective treatment immediately post-accident. PATIENTS AND METHODS: In this prospective study (2014 - 2016) we treated 29 patients with blow-out fractures. 18 (62%) were treated conservatively and in 11 (38%) we proceeded surgically. We decided whether surgical or non-surgical therapy was appropriate on the basis of clinical ENT, eye examination and the total volume of prolapsed orbital soft tissue. All procedures were perfomed by the same operating team with a uniform subciliary approach and using PMR splints adapted to the correct size and shape. RESULTS: On the basis of the mathematical model we reassessed findings in 2 patients: in one we decided against a surgical solution and in the other a surgical approach was indicated. All 18 patients treated conservatively, fully recovered and are free of diplopia. The 11 operated patients are also free of diplopia, only 1 patient (3%) displays clinically insignificant postoperative diplopia in extreme posisitons when looking upwards. CONCLUSION: With proper selection of the optimal treatment, the rate of complete disappearance of diplopia and fully preserved motility of the eyeball ranges from 91 to 97%. Surgical treatment of orbital floor fractures is important, mainly to minimise persistent post-traumatic diplopia which significantly reduces a patient's quality of life.

BACKGROUND AND AIM: The otolaryngologist often meets with fractures of the orbital floor. The most serious complication is diplopia, arising as a result of herniation of the orbital contents, with or without fixation of the inferior rectus muscle. The aim of our work was to create a mathematical model to calculate the volume of prolapsed soft tissue of the orbit in blow-out fractures, as a factor in deciding on the need for surgical treatment. PATIENTS AND METHODS: In a retrospective study (2007-2013), we evaluated 80 patients with blow-out fractures, divided into two equal groups: 40 conservatively treated and 40 surgically treated patients. We created the model by measuring the fracture lines and herniation of the orbital soft tissues in the coronal and sagittal sections from CT images, equivalent to half the volume of a rotating ellipsoid. RESULTS: According to the proposed model, posterior and anterior fractures with a prolapse volume above 500 mm3, and anteroposterior fractures with a volume over 1400 mm3, are indicated for surgery. CONCLUSION: The volume of prolapsed soft tissue relative to the location of the fracture is the main indicator for selecting the best treatment procedure immediately after injury.

One possible use of CT navigated surgery in Otorinolaryngology includes traumatology of the facial skeleton. CT navigation enables a surgeon to assess the range and quality of reposition and fixation of a fracture, in our case the fracture of the orbital floor, using a navigated tube. We proceed from pre-operative CT of the paranasal sinuses images to navigation with 1,5-2,0 mm incisions and subsequent registration. Formerly, we used mainly transantral approach at our department, while currently we prefer subciliar approach with the use of absorbable materials (PDS or PMR splint). We use a rigid endoscope for infraorbital nerve decompression and delicate removal of free bone fragments from the maxillary sinus. We applied this procedure in 2 patients. We verified the range of decompression, degree of reposition of the orbital blow-out fracture, and placement of PMR splint and titanium grid using video navigation.

Jedním z možných využití CT navigované chirurgie v ORL oblasti je traumatologie obličejového skeletu. CT navigace zde umožňuje operatérovi pomocí navigované sondy posoudit rozsah a kvalitu provedené repozice a fixace zlomeniny, v našem případě spodiny očnice. Vycházíme z předoperačních CT VDN snímků pro navigaci s řezy 1,5–2,0 mm a následné registrace. Na našem pracovišti jsme v období od r. 2007 – 2011 chirurgicky ošetřili 28 blow-out zlomenin spodiny očnice. 21krát jsme postupovali transantrálně (8krát s CT navigací) a 7krát ze subciliárního přístupu (vždy navigovaně). Při transantrálním přístupu jsme zlomeniny podepřeli vzpěrou složenou z hydronové ploténky a superakrylátové destičky s ocelovou spirálkou. U subciliárních přístupů jsme použili 6krát PMR dlahu a 1krát PDS dlahu. Poměr mužů a žen byl 19 : 9, se stranovým zastoupením 15krát levá a 13krát pravá očnice. Průměrný věk operovaných činil 41 roků (16-85 let). K repozici jsme přistoupili 3. až 57. poúrazový den (v průměru 12. den). V naší sestavě jsme se zaměřili na porovnání anatomických a funkčních výsledků chirurgicky řešených zlomenin spodiny očnice v závislosti na operační technice a na použití CT navigace. Hodnotili jsme pooperační postavení, polohu, motilitu bulbu a stav diplopie. Ve skupině primárních výkonů bez CT navigace z transantrálního přístupu u 77 % nemocných diplopie zcela vymizela a u 23 % operovaných s frakturou staršího data přetrvává diplopie pouze v krajních polohách na hranicích zorného pole, klinicky nevýznamná. U všech operovaných nemocných s CT navigací jsme dosáhli úplného vymizení diplopie a správného postavení očního bulbu bez ohledu na přístupovou cestu nebo použitý fixační materiál.
One of possible application s of CT navigated surgery in the ORL region is traumatology of facial skeleton. CT navigation in this area makes it possible to the surgeon to use the navigation probe for evaluation of the extent and quality of performed reposition and fixation of the fracture, here specifically the eye-socket base. The procedure is based on preoperative CT VDN images with sections 1.5 – 2.0mm and subsequent registration. At our workplace surgery was performed for 28 blow-out fractures of eye-socket base in the period of 2007 – 2011. In 21 cases we used the transantral approach (8 times with CT navigation) and 7 times from the subciliary approach (under navigation in all cases). In the transantral approach the fractures were supported by a prop composed from a hydro plate and super acrylate lamina with a steel spiral. In the subciliary approach we used PMR splint six times and a PDS splint once. The proportion of men and women was 19:9 and the representation of sides was 15 on the left and 13 in case of the right eye socket. The mean age at the time of surgery was 41 years (16-85). The reposition was performed on day 3 up to 57 after the injury (day 12 on the average). In the analysis of our cohort we paid attention to comparing anatomical and functional results of surgically treated eye-socket base fractures in relation to surgical technique and the use of CT navigation. We evaluated the postoperative placement, position, eyeball motility and the state of diplopia. In the group of primary interventions without CT navigation from transantral approach, diplopia disappeared in 77% of patients completely and in 23% operated on fracture of an older date the diplopia continues only in the outside positions of visual field and is considered as clinically insignificant. In all surgically treated patients with CT navigation we reached a complete disappearance of diplopia and the correct position of the eyeball irrespective of the way of approach or the used fixation material.
