LADA (Latent Autoimmune Diabetes in Adults) je typ autoimunitního diabetu vznikající v dospělém věku. V porovnání s diabetes mellitus 1. typu je LADA charakterizován nižší intenzitou autoimunitního procesu, pozvolnějším rozvojem a progresí a spíše mírnou hyperglykemií v době manifestace. Objevuje se v dospělém věku (> 30 let), pacienti jsou obvykle bez výrazněji vyjádřených známek metabolického syndromu, inzulinové rezistence či obezity. Přesto, zejména je‑ li diagnostikován ve vyšším věku, může být mylně klasifikován jako diabetes mellitus 2. typu, a to i proto, že ho lze většinou úspěšně léčit neinzulinovými antidiabetiky po dobu nejméně 6 měsíců od diagnostiky. V léčbě LADA je doporučena časná iniciace inzulinoterapie, u pacientů s výraznější inzulinovou rezistencí v kombinaci s metforminem. Klinické studie naznačují také efektivitu preparátů umožňujících prodloužení životnosti reziduálních beta‑buněk, např. inkretinů.
Latent Autoimmune Diabetes in Adults (LADA) is an autoimmune disease arising at adulthood. LADA is characterized by a less intensive autoimmune process, slower progression and a mild metabolic decompensation at onset compared with young-onset type 1 diabetes mellitus. The onset of LADA is usually in non-obese patients over 30, without prominent features of metabolic syndrome and insulin resistance. Nevertheless it may be falsely classified as type 2 diabetes, especially, when diagnosed in older age and for the possibility of non-insulin treatment for at least 6 months after diagnosis. LADA is treated early with insulin and combined with metformin in patients with a higher level of insulin resistance. Clinical studies suggested also effectivity of other oral antidiabetics enabling preservation of residual beta-cell function, such as particularly incretines.
Diabetes typu LADA (Latent Autoimmune Diabetes in Adults) je charakterizován nižší intenzitou autoimunitního procesu, pozvolnějším nástupem se spíše mírnou hyperglykemií v době manifestace v porovnání s diabetes mellitus 1. typu. Často, zejména objevíli se ve vyšším věku, může být chybně klasifikován jako diabetes mellitus 2. typu. LADA se typicky manifestuje ve vyšším věku (> 30 let), nebývají u něj vyjádřeny výraznější známky metabolického syndromu, inzulinové rezistence či obezita. V léčbě je užíván inzulin, je kombinován s metforminem u pacientů s výraznější inzulinovou rezistencí. Testovány jsou další preparáty jako gliptiny a glitazony s cílem prodloužit životnost beta-buněk.

The majority of patients with adult-onset autoimmune diabetes usually do not require insulin treatment for at least 6 months after diagnosis. Thus, they may be falsely classified as having type 2 diabetes. The onset of LADA is usually in non-obese patients older than 30 years, without prominent features of metabolic syndrome and sings of insulin resistance. LADA is treated with insulin and combined with metformin in patients with a higher level of insulin resistance. Other oral antidiabetics as gliptins, glinitazones are under investigation as they might preserve beta-cells.

BACKGROUND: A portion of adults with humoral immune changes have clinical diabetes that is initially not insulin-requiring (latent autoimmune diabetes of the adult, LADA). One of the genes strongly associated with autoimmune diabetes is PTPN22. We hypothesized that the manifestation and clinical features of LADA are linked to functional variants of PTPN22. METHODS: We genotyped allelic frequencies of 1 protective and 3 risk-associated PTPN22 variants in 156 Czech LADA patients, 194 type 2 diabetes mellitus patients with LADA-like progression to insulinotherapy and 324 type 1 diabetes mellitus patients, and subsequently examined the associations of PTPN22 variants with the expression of autoantibodies and other clinical features of LADA. RESULTS: We challenged the paradigm that stated that the PTPN22 c.1858T allele serves as a risk allele for LADA, although we confirmed its risk status in the geographically matched T1DM cohort. In contrast, the frequencies of other PTPN22 alleles (c.-1123C, c.788A and c.1970-852C) differed significantly from the healthy controls. We confirmed gender-related differences in the frequency of some PTPN22 polymorphisms (but not c.1858C>T) in LADA. The particular PTPN22 alleles and genotypes were associated with specific clinical features of the examined patients (autoantibodies, HbA1c and age at diagnosis of diabetes). CONCLUSIONS: The variability in PTPN22 haplotypes suggests that the genetic signature of LADA is independent and should not be considered a hybrid form of T1DM and T2DM. Further studies should elucidate the associations with clinical characteristics of the LADA patients and focus on the newly emerging types of diabetes with the disease onset in early to mid-adulthood.

Autoimunitný mechanizmus vzniku ochorení je v súčasnosti veľmi aktuálna problematika. V diabetológii sa autoimunitný mechanizmus spomína v patofyziológii diabetes mellitus 1. typu (DM1T) najmä u mladých. Avšak v súčasnosti narastá aj počet pacientov s typickým obrazom DM1T manifestujúcim sa dospelosti – stav známy ako Latent Autoimmune Diabetes in Adults (LADA). Autoimunitné mechanizmy sa spomínajú ale aj v procese vzniku inzulínovej rezistencie či dokonca gestačného diabetes mellitus. Teória autoimunity sa v diabetológii dostáva teda do popredia vo všetkých oblastiach a dokonca začína byť reflektovaná aj v posledných guidelines Americkej diabetologickej asociácie (ADA) 2017 pri novej klasifikácii DM.
Autoimmune mechanism of various diseases is currently a hot topic in medicine. In diabetology is the autoimmune mechanism typically mentioned in the pathophysiology of type 1 diabetes (T1D), particularly among the young. But there is an increasing number of patients with clinical picture of T1D manifesting in adulthood – a condition known as Latent Autoimmune Diabetes in Adults (LADA). Autoimmune mechanisms are in the last time mentioned also in the process of insulin resistance and even gestational diabetes. Therefor the theory of autoimmunity is now in all areas of diabetology, and even starting to be reflected in the last American Diabetes Association (ADA) guidelines 2017 in the reclassification of diabetes.
