Pneumocystis jirovecii is an airborne human-specific ascomycetous fungus responsible for Pneumocystis pneumonia (PCP) in immunocompromised patients, affecting >500,000 patients per year (www.gaffi.org). The understanding of its epidemiology is limited by the lack of standardised culture. Recent genotyping data suggests a limited genetic diversity of P. jirovecii. The objective of the study was to assess the diversity of P. jirovecii across European hospitals and analyse P. jirovecii diversity in respect to clinical data obtained from the patients. Genotyping was performed using six already validated short tandem repeat (STR) markers on 249 samples (median: 17 per centre interquartile range [11-20]) from PCP patients of 16 European centres. Mixtures of STR markers (i.e., ≥2 alleles for ≥1 locus) were detected in 67.6% (interquartile range [61.4; 76.5]) of the samples. Mixture was significantly associated with the underlying disease of the patient, with an increased proportion in HIV patients (78.3%) and a decreased proportion in renal transplant recipients (33.3%) (p<0.001). The distribution of the alleles was significantly different (p<0.001) according to the centres in three out of six markers. In analysable samples, 201 combinations were observed corresponding to 137 genotypes: 116 genotypes were country-specific; 12 in two; six in three; and two in four and one in five countries. Nine genotypes were recorded more than once in a given country. Genotype 123 (Gt123) was significantly associated with France (14/15, p<0.001) and Gt16 with Belgium (5/5, p<0.001). More specifically, Gt123 was observed mainly in France (14/15/16 patients) and in renal transplant patient (13/15). Our study showed the wide population diversity across Europe, with evidence of local clusters of patients harbouring a given genotype. These data suggest a specific association between genotype and underlying disease, with evidence of a different natural history of PCP in HIV patients and renal transplant recipients.

Objev Pneumocystis jako původce intersticiální pneumonie novorozenců je spojen se jmény českých profesorů Vaňka a Jírovce. V roce 1976 byl identifikován druh způsobující onemocnění člověka a na počest českého přírodovědce pojmenován Pneumocystis jirovecii. Původní zařazení mezi protozoa bylo revidováno a dnes je hlavně díky rozvoji molekulární biologie Pneumocystis řazena do říše hub. Jedná se pravděpodobně o fylogeneticky velice starý, druhově specifický mikroorganismus a stále jsou identifikovány další druhy rodu Pneumocystis. Diagnostika je od doby objevu založena na přímém průkazu agens z materiálů získaných z dýchacích cest, dříve pouze mikroskopickými metodami, dnes i s využitím PCR technik. Pneumocystis se nedaří kultivovat a výsledky sérologické diagnostiky je možné využít pouze k epidemiologickým účelům. Zobrazovací metody a ostatní laboratorní vyšetření slouží jako doplňková.
Pneumocystis jirovecii and its microbiological diagnostics The discovery of Pneumocystis as the causal agent behind intersticial pneumonia in newborns is associated with the names of two Czech professors – Vaněk and Jírovec. In the year 1976, a species was identified as the one causing the disease in humans and in honour of the Czech naturalist, it was named Pneumocystis jirovecii. Its initial classification into the protozoa group has been revised and especially thanks to new findings from the field of molecular biology, Pneumocystis is currently classified as a part of the Fungi Kingdom. The organism is most likely phylogenetically very old and species-specific; new species of the Pneumocystis genus are being identified to this day. Ever since the original discovery, diagnosing this condition has been based on direct proof of the presence of the agent in material obtained from the respiratory tract. In the part only using microscophy methods, nowadays using PCR techniques as well. Pneumocystis cannot be properly cultivated and serological analysis can only be used for epidemiological purposes. Imaging methods and other laboratory examinations are supplementary.

Klinická lekárska mykológia zaznamenala už via jako 200 rôznych druhov mikroskopických húb, izolovaných z mykóz človeka. K novým zaujímavým druhom patria Penicillium marneffei, Cryptococcus neoformans var. gatii, Candida dubliniensis a k mikroskopickým hubám sa priradil druh Pneumocystis carinii. Stúpa počet pacientov s nosokomiálnymi mykózami. Sú problémy s diagnostikou, s terapiou a novými ale aj staršími antimykotikami, s citiivosťou a rezistenciou mikromycét, štandardizáciou metodík apod. Environmentálna lekárska mykológia sa venovala aflatoxínu (kwashiorkor a hepatómy), ochratoxínu A (u dialýzovaných pacientov) a novým mykotoxínom – fumonizínom, 2-nitropropionovej kyseline. Objavila sa problematika mikromycét v bytoch. V mykológii sa rozvíjajú „nekultivačné“ metódy detekcie mikromycét a metódy molekulárnej biologie. V referáte sa dala prednost poznatkom, ktoré majú dosah pre praktickú aplikáciu.
Clinical medici mycology records more than 200 different species of microscopic fungi isolated from patiens presenting with some type of mycosis. Rather interesting new species are Penicillium mameffei, Cryptococcus neoformans van gatti, Candida dubliniensis; the species Pneumocystis carinii has been addend to microscopic fungi. The number of patiens presenting nosocomial mycoses is risik. Problems koncern diagnosis, treatment, both new and older antimycotic agents, sensitivity and resistence of micromycetes, standardization of methods ETA. Environmental medici mycology investigated aflatoxin (kwashiorkor and hepatomas), ochratoxin A (in dialysed patients) and the new mycotoxins – fumonisinome and 2-nitropropionic acid. A new problem that has recentiy cropped up are nicromycetes in priváte homes (households). Mycology is developing „non-culture“ methods for the detection of micromycetes, as well as nethods of molecular biology. The paper highlights findings of importace for practical applications.
V roce 1909 Carlos Chagas při studiu rodu Trypanosoma jako první identifikoval mikroorganizmus, který se později stal známý jako Pneumocystis carinii. O dva roky později popsal Chagas tento mikroorganizmus v plicích člověka, který zemřel na trypanozomiázu. V této době však nebylo jasno, že se jedná o samostatný druh mikroorganizmu. Pneumocystis carinii jako samostatný druh odlišný od :rypanozom byl identifikován až v roce 1912. Delanoé a Delanoé identifikovali Pneumocystis carinii v plicích krys, které trypanozomiázu leměly. Přesto až do roku 1951 nebylo vyjasněno, zda Pneumocystis carinii může být vyvolávající příčinou onemocnění u lidí (13).
