Maisonneuveova zlomenina (MZ) je definována jako luxační zlomenina hlezna, kde došlo vždy ke zlomenině proximální čtvrtiny fibuly a roztržení lig. tibiofibulare anterius a lig. tibiofibulare interosseum. Další poranění jsou variabilní. Recentní CT studie ukázaly, že MZ je mnohem komplexnější poranění, než se předpokládalo. Proto je třeba zásadním způsobem změnit pohled na tuto problematiku. MZ je spojena přibližně v 80 % případů se zlomeninou malleolus posterior a dále s malpozicí distální fibuly v incisura fibularis tibiae (IFT). Přesné zhodnocení těchto poranění vyžaduje úrazové CT, které by mělo být u MZ v dnešní době standardem. Hlavním cílem ošetření je anatomická repozice distální fibuly do IFT. To vyžaduje v případě odlomení většího fragmentu malleolus posterior provést nejdříve jeho repozici a fixaci z posterolaterálního přístupu. Tím dojde k obnovení integrity IFT. Zavřená repozice distální fibuly je spojena s malpozicí až v 50 % případů. Proto se dává přednost otevřené repozici z krátkého anterolaterálního přístupu. Přesnost repozice je nutné vždy ověřit pooperačním CT.
Maisonneuve fracture (MF) is defined as an ankle fracture-dislocation associated always with a fracture of the proximal quarter of the fibula and rupture of the anterior and interosseous tibiofibular ligaments. Other injuries are variable. Recent CT studies have demonstrated that MF is a far more complex injury than initially supposed. Therefore it is necessary to change substantially the current concepts related to this issue. MF is combined in about 80% of cases with a fracture of the posterior malleolus and also with malposition of the distal fibula in the fibular notch. An exact assessment of these injuries requires post-injury CT examination which should be used as a standard in MFs. The main goal of treatment is anatomical reduction of the distal fibula into the fibular notch. In case of avulsion of a larger fragment of the posterior malleolus, it is necessary to perform as the first step its reduction and fixation from the posterolateral approach and thus restore integrity of the notch. Closed reduction of the distal fibula is associated with malposition in up to 50% of cases and therefore open reduction from a short anterolateral approach is preferred. Accuracy of reduction should be always checked by postoperative CT scan.

PURPOSE: During study of anatomy of a fractured posterior malleolus of the ankle on CT scans, the authors noticed a prominent crest on the lateral malleolus, which they termed the lateral malleolar crest (LMC). As, in their view, LMC is a clinically important structure which was only briefly mentioned by a few authors without an official term, they focused on the anatomy of this structure. MATERIALS AND METHODS: A total of 352 dry fibulae were analyzed and the following parameters recorded: (F) length of the fibula, (LMC) total length of LMC, (A) length of the part of the examined crest from the superior border of the articular facet of the lateral malleolus (AFLM) to its most proximal intersection with the midline of the fibula, (B) height of the medial triangular rough surface, and (A/F) A/F ratio. RESULTS: The crest was observed in all specimens. (F) was 346.5 ± 26 mm (95% confidence interval [CI] 344-349), (LMC) was 85.4 ± 11.6 mm (95% CI 84.2-86.6), (A/F) was 25% ± 3% (95% CI 24.7-25.3) in the whole group. (A) was 25.9 ± 6.5 mm (95% CI 24.8-26.8) in the whole group, (B) was 34.9 ± 4.7 mm (95% CI 34.3-35.5) in the whole group, 36 ± 6.1 mm (95% CI 35.1-36.9). CONCLUSION: LMC is an important structure on the lateral malleolus. The knowledge of its anatomy is essential for placement of syndesmotic screws or/and the fibular plate.

PURPOSE: The aim of this study was to describe pathoanatomy and to raise awareness of a fracture of the lateral malleolus combined with a high subcapital fracture of the fibula caused by a dislocation mechanism. METHODS: The study comprised 11 patients, 5 men and 6 women, with the mean age of 57 years (range, 21-87), with a "Double Maisonneuve fracture". Individual lesions of ankle structures were described on the basis of radiographs, CT, and intraoperative findings. RESULTS: The distal fibular fracture was classified as Weber type B in 1 case and Weber type C in 10 cases. The proximal fibular fracture was described as a subcapital oblique spiral fracture with metadiaphyseal involvement in nine cases and a high short oblique fracture with fibular head involvement in two cases. Injury to the deltoid ligament was revealed in six cases; a bicollicular fracture of the medial malleolus was found in five patients. Posterior malleolar fractures were classified as type 1 in eight cases and type 2 in three cases. Avulsion of the Chaput tubercle was detected in four cases. Injury to the interosseous tibiofibular ligament was assessed in nine patients. CONCLUSION: Double Maisonneuve fracture is a rare but probably underreported injury that must be taken into consideration during examination, as it may be easily overlooked. The essential part of diagnosis is a careful clinical examination and radiological assessment of the lower leg with additional CT examination of the ankle.

This paper deals with a fatal stab wound suffered by a 29-year-old man and nonfatal injuries of 35-year-old and 67-year-old saw operators. Rip saw is a machine that is specially designed for making a rip cut, a cut made parallel to the direction of the wood grain. Rip-saw-related injuries mostly occur when a person is struck by the cutting material, which usually involves splinters of irregular shape and diameter. When the splinter strikes the body diagonally, the injuries may include abrasions, lacerations, and cut wounds; in situations where the victim is struck directly, the most common injuries are oval- or star-shaped stab wounds with a varying width of abrasions around the wounds. Therefore, such injuries may come across as injuries produced by a knife-like instrument, which is an added difficulty in the interpretation of such injuries.

OBJECTIVES: Posterior malleolar fractures (PM) have been linked to inferior outcome in malleolar fractures. This study aims to analyze the prevalence and pathoanatomy of PM fractures in Bosworth fracture-dislocations (BF). MATERIALS & METHODS: Radiographs and computed tomography (CT) scans of 13 patients treated at our institution and 97 cases published between 1947 and 2018, identified in a systematic literature search, were evaluated with respect to the pathoanatomy of BF. In all 13 cases from the present study and in 10 cases from the literature, axial CT scans were performed. RESULTS: All 13 patients (100%) with BF from the present series and 61 of 97 documented cases (63%) of BF from the literature were associated with a PM fracture. In patients with a complete CT analysis, dislocation of the fibula behind the posterior tibial rim was associated with extraincisural (Bartoníček / Rammelt type 1) PM fractures. Displacement of the fibula between the displaced PM fragment and the tibia was associated with Bartoníček / Rammelt types 2 and 3 PM fractures. CONCLUSIONS: Seventy prevent of all reported BF are associated with a PM fracture. The true prevalence may be even higher because of the historically infrequent use of CT imaging. The pathoanatomy of the PM fragment is highly variable as is the kind of fibular displacement in BF. Therefore, CT scanning should be performed routinely in BF. Displaced PM fractures in BF involving the incisura should be treated operatively via a direct posterolateral approach.

Otevřené zlomeniny bérce s rozsáhlou dilacerací měkkých tkání patří mezi jednu z mnoha výzev moderní traumatologie. Rozhodnutí pro záchovný výkon na základě komplexního vyšetření představuje v dalším průběhu dlouhodobou, multidisciplinární péči s nejistým výsledkem a vysokými nároky na erudici ošetřujícího týmu a psychickou odolnost pacienta. V naší kazuistice prezentujeme výsledek a jednotlivé diagnosticko-terapeutické kroky u pacienta s monotraumatem bérce poraněného padajícím stromem.
Open tibial fractures with an extensive laceration of soft tissues are one of many challenges of modern traumatology. Decision to perform a limb-salvage procedure, based upon a complex assessment of the patient's condition, requires further long-term, multidisciplinary care during the further course of treatment, with uncertain results and high demands for the erudition of the attending team of healthcare professionals, and also psychological endurance of the patient. In our case report, we present the results and individual diagnostic-therapeutic steps in a patient hurt with a falling tree, with tibial monotrauma.

ÚVOD: Procento infekčních komplikací po nitrodřeňovém hřebování je 1–4 %. Otevřené zlomeniny způsobené vysokoenergetickými úrazy mají procento infektu vyšší. KAZUISTIKA: Je prezentován průběh léčení 20letého muže s infikovaným pakloubem tibie po nitrodřeňovém hřebování otevřené zlomeniny. Léčení zahrnovalo postupné procedury extrakce hřebu a konverze na zevní fixátor. Po radikální resekci infikovaného pakloubu a měkkých tkání byl kostní defekt řešen bifokálním segmentálním transportem pomocí unilaterálního zevního fixátoru. Popisujeme komplikace segmentálního transportu – pin tract infekci a potřebu revize místa docking site s použitím spongioplastiky. Pacient se po náročném léčení zhojil a byl schopen plné zátěže po 30 měsících. DISKUZE: Léčení infikovaného pakloubu tibie zahrnuje radiální resekci infektem postižené kosti a měkkých tkání s následnými sekundárními rekonstrukčními výkony na skeletu a měkkých tkáních, jako jsou segmentální kostní transport a spongioplastika. ZÁVĚR: Segmentální transport je používán v léčbě kostních defektů. Terapie infikovaných pakloubů vyžaduje opakované operační výkony a individuální přístup k pacientovi.
INTRODUCTION: The rate of infection complications after intramedullary nailing of long bone is 1-4 %. After high energy open fractures is considerably higher. CASE REPORT: This is case of 20 – year man with infected nonunion of the tibia following intramedullary nailing of open tibia fracture. We present a series of sequential surgical procedures such as nail removal, debridement of fracture site, stabilisation by external fixation and fibular osteotomy. The final treatment of infected nonunion of the tibia was radical bone resection and reconstruction by bifocal segmental transport using monolateral external fixation. We describe complications of segmental transport such as pin-tract infection and necessity of revision and bone grafting at the docking site. The pacient was healed to full weight bearing after 30 months. DISCUSSION: The classic techniques for the treatment of a tibial nonunion associated with chronic osteomyelitis include the resection of the infected and nonviable, avascular bone and soft tissue following reconstructive procedures such as bone transport or grafting. CONCLUSION: Bone transport is indicated for segmental bone defects. The treatment of infected tibial nonunion need a series of sequential surgical procedures.
