Cíl práce: Analýza výskytu sérových autoprotilátek reprodukčního souboru u žen s tzv. idiopatickou sterilitou a dysfertilitou. Materiál a metodika:Bylo vyšetřeno 150 žen s diagnózami sterility, prvotrimestrálních a druhotrimestrálních potratů a habituálního potrácení na přítomnost sérových protilátek proti fosfolipidům, annexinu V, ovariím, zona pellucida, spermiím, tkáňové transglutamináze a jaderným antigenům. Výsledky: Ve skupině pacientek s primární nebo sekundární sterilitou dominoval nález pozitivity protilátek proti spermiím (12 %) a v menší míře protilátek proti ovariím a zona pellucida (oboje cca 5 %). U žen s prvotrimestrálními potraty patřily k nejvýznamněji zastoupeným protilátky proti fosfolipidům v izotypu IgG (12,5 %), tkáňové transglutamináze IgA a/nebo IgG (přes 12 %) a především proti annexinu V (téměř 70 %), druhotrimestrální potraty byly asociovány zejména s pozitivitou antifosfolipidových protilátek (> 26 %). Výskyt antinukleárních protilátek v sérech neplodných žen byl signifikantně vyšší ve srovnání s literárními údaji o výskytu těchto protilátek v populaci fertilních žen stejného věku. Závěr: Imunologická laboratorní diagnostika je nedílnou součástí vyšetření neplodných párů a má své místo jak při screeningových, tak i vysoce specializovaných vyšetřeních.
Objective: Analysis of the serum autoantibodies incidence in women with so-called. idiopathic sterility and/or dysfertility. Materials and Methods: We have examined 150 women diagnosed with infertility, first- and second-trimester abortions and habitual miscarriages on the p resence of serum antibodies against phospholipids, annexin V, ovary, zona pellucida, sperm, tissue transglutaminase and nuclear antigens. Results: In the group of patients with primary or secondary sterility anti-sperm antibodies positivity was dominant (12%), and to a lesser exte nt anti-ovarian and anti-zona pellucida autoreactivity (both about 5%). In women with first-trimester miscarriages, IgG antiphospholipid antibo dies were found in 12.5%, anti-tissue transglutaminase antibodies IgA and/or IgG in over 12% cases, and especially antibodies against annexin V in nearly 70% individuals. Second-trimester abortions were associated mainly with antiphospholipid antibodies (> 26%). The incidence of antin uclear antibodies in sera of infertile women was significantly higher compared to the literature data on the prevalence of these antibodies in th e population of fertile women of the same age. Conclusions: Immunological laboratory testing is an integral part of the examinations in infertile couples and has its place in both the screening and the highly specialized investigations.

OBJECTIVE: The aim of the study was to evaluate the importance of screening for thrombophilic mutations after the first early pregnancy loss. SETTING: Thrombophilic mutations were examined in a sample of 100 women with at least one miscarriage. DNA was isolated from venous blood sample. We used methods of microarray, fragmentation analysis, High Resolution Melting and PCR-ARMS with following gel electrophoresis and visualisation. Chi-square test and in cases of low expected frequencies Yates correction were used to compare relative frequencies of individual mutations. The comparison of averages was performed by t-test. RESULTS: We detected prevalence of factor V and II mutation of 9% and 3%, respectively. Single MTHFR mutation was found in 59% and double heterozygous MTHFR mutation in 23% of cases. No mutation was present in only 6% of the study group. Heterozygous mutations of factor V occurred 1.8 times more frequently in our study group compared to the general Czech women population. Also, the frequency of factor II mutation was 1.5-3 times higher. No carrier of these mutations had overt coagulation disorder, history of thromboembolic disease or that of habitual abortions. CONCLUSIONS: The frequency of thrombophilic mutations in the group of women with early pregnancy loss is 1.5-3 times higher than in the general population.

PROBLEM: The aim of this study was to investigate frequencies of eight antiphospholipid antibodies (aPLs) in serum, four genetic thrombophilic factors and their mutual relation in 206 patients with repeated pregnancy loss (RPL). METHOD OF STUDY: Enzyme-linked immunosorbent assay was used for detection of aPLs against ph-serine, ph-ethanolamine, ph-inositol, DL-glycerol, phosphatidic acid, anti-annexin V, cardiolipin, and beta2-GPI. FV 1691G>A (Leiden mutation), FII 20210G>A mutation, MTHFR 677C>T and MTHFR 1298A>C variant genotypes were determined using a melting curve analysis of the PCR amplification product detected by the fluorescence resonance energy transfer. Genotypic distribution and allelic frequencies were calculated. Correlation between aPLs and thrombophilic factors was tested by chi-square and Fisher exact test. RESULTS: Our results show significantly increased prevalence of aPLs against ph-inositol (17-19.6% dependent on number of spontaneous miscarriages) and against ph-serine (18-25%). aPLs in IgG prevail. In 96% of the studied group, at least one risk factor was found (either aPLs positivity or thrombophilic factor). Both aPLs and thrombophilic factors were present in 43%. In the group of women with three or more RPLs, strong positive correlation of aPLs positivity and thrombophilic risk factors was observed. CONCLUSION: Antiphospholipide antibodies and genetic thrombophilic factors are important risk factors in the pathogenesis of RPL. Both autoantibodies against various kinds of phospholipides and genetic thrombophilic factors must be studied together in diagnosis of RPL for appropriate treatment

OBJECTIVE: There is a growing view that inherited or acquired thrombophilia may predispose a woman towards an adverse pregnancy outcome. The aim of this study was to investigate whether risk factors for placental abruption because of such thrombophilias (such as carriership of factor V Leiden (FVL), prothrombin G20210A gene mutation and homozygous MTHFR C677T) might be used as a predictor for placental abruption. METHODS: A retrospective case-control study conducted at the University Hospital, Palacky University, Olomouc, Czech Republic. One hundred and eighty women with placental abruption out of 20,175 deliveries (0.79%) were compared to 196 unselected gravidae. A detailed medical history was taken with special reference to factors related to hypercoagulation and blood was drawn for polymerase chain reaction analysis. The prevalence of FVL, prothrombin G20210A and MTHFR C677T was related to placental abruption. RESULTS: The heterozygous form of FVL was present in 20of 142 cases (14.1%) in the placental abruption group, compared to ten of 196 (5.1%) in the control group (odds ratio 3.0, 95% confidence interval 1.4-6.7). CONCLUSIONS: We found that factor V Leiden is a significant risk factor for placental abruption.
