Hypervirulentní kmeny Klebsiella pneumoniae (hvKP) mohou způsobovat atypické multilokulární infekce u jinak zdravých pacientů. Diagnostika infekce vyvolané hvKP je založena především na klinickém nálezu a laboratorních výsledcích včetně detekce genů virulence. Typicky se projevuje jako jaterní absces s metastatickým šířením. Léčba je založena na chirurgickém řešení v kombinaci s cílenou antimikrobiální terapií. Výskyt infekce hvKP je relativně častý v Asii. V Evropě je sice stále vzácný, ale incidence onemocnění se zvyšuje. Cílem článku je poskytnout stručný přehled problematiky a upozornit na možný výskyt infekcí hvKP.
Hypervirulent strains of Klebsiella pneumoniae (hvKP) can cause atypical multilocular infections in otherwise healthy patients. Diagnosis of infection caused by hvKP is based mainly on clinical findings and laboratory results, including detection of virulence genes. It typically manifests as hepatic abscess with metastatic spread. Treatment is based on surgical intervention in combination with targeted antimicrobial therapy. The occurrence of hvKP infection is relatively common in Asia, and while still rare in Europe, incidence is increasing. The article aims to provide a short overview of the issue and increase awareness of the possible occurrence of hvKP infections.

Multinational surveillance programmes for methicillin-resistant Staphylococcus aureus (MRSA) are dependent on national structures for data collection. This study aimed to capture the diversity of national MRSA surveillance programmes and to propose a framework for harmonisation of MRSA surveillance. The International Society of Antimicrobial Chemotherapy (ISAC) MRSA Working Group conducted a structured survey on MRSA surveillance programmes and organised a webinar to discuss the programmes' strengths and challenges as well as guidelines for harmonisation. Completed surveys represented 24 MRSA surveillance programmes in 16 countries. Several countries reported separate epidemiological and microbiological surveillance. Informing clinicians and national policy-makers were the most common purposes of surveillance. Surveillance of bloodstream infections (BSIs) was present in all programmes. Other invasive infections were often included. Three countries reported active surveillance of MRSA carriage. Methodology and reporting of antimicrobial susceptibility, virulence factors, molecular genotyping and epidemiological metadata varied greatly. Current MRSA surveillance programmes rely upon heterogeneous data collection systems, which hampers international epidemiological monitoring and research. To harmonise MRSA surveillance, we suggest improving the integration of microbiological and epidemiological data, implementation of central biobanks for MRSA isolate collection, and inclusion of a representative sample of skin and soft-tissue infection cases in addition to all BSI cases.

Kožní absces patří mezi nejčastější infekce kůže a měkkých tkání. Standardní léčbou je incize a drenáž abscesu prováděná v místním znecitlivění. Na základě menších studií nebyl benefit antibiotik u pacientů s nekomplikovaným abscesem prokázán, přesto se v naší praxi setkáváme u pacientů s nekomplikovanými abscesy s jejich užitím. Naším cílem bylo zhodnotit současná data hodnotící vliv adjuvantního užití antibiotik na léčbu abscesu. Ačkoli poslední dvě multicentrické studie naznačují, že antibiotika mohou mít při léčbě pozitivní efekt, je důležité zvažovat rizika možnosti vzniku rezistence při jejich nadbytečném užívání a při léčbě přistupovat k pacientům individuálně.
Skin abscess is one of the most common infections of skin and soft tissues. Incision and drainage under local anesthesia is the mainstay of treatment. Although previous small-scale studies failed to show a benefit of antibiotics in patients with simple abscesses, the use of antibiotic therapy is still encountered in the treatment of uncomplicated skin abscesses in our practice. The purpose of our study was to evaluate existing data assessing the effect of adjuvant antibiotic therapy on abscess management. Although as indicated by two recent multicenter studies, antibiotics may provide a positive effect in the management, it is important to consider the risk of resistance associated with antibiotic overuse and to approach each case individually.
With the alarming rise in antimicrobial resistance, phage therapy represents a new paradigm for combating antibiotic-resistant infectious diseases that is worth exploring for its clinical success. With this scenario, the present study aimed at evaluating the in vivo potential of phage MR-5 (broad host range Staphylococcus aureus phage) against soft tissue infections induced by methicillin-resistant S. aureus (MRSA). Also, the usefulness of relatively simple murine air pouch as a dual-purpose model (to study both anti-bacterial and anti-inflammatory parameters) in the field of phage therapeutics has been put to test. Murine air pouch model was established with experimental skin infection induced by S. aureus ATCC 43,300 followed by subcutaneous administration of phage alone as well as along with linezolid. Phage MR-5 alone and in combination with linezolid (showing synergy) brought significant reduction in the bacterial load (both extracellular as well as intracellular) that led to faster resolution of pouch infection. The main conclusions surfaced from the present study include the following: (a) murine air pouch model represents a simple useful model (mimicking subcutaneous skin infection) for studying anti-bacterial potencies of drug candidates. Therefore, its use and further adaptations especially in field of phage therapeutics is highly advocated and (b) phage MR-5 proved to be a potential therapeutic candidate against treatment of MRSA-induced skin and soft tissue infections and use of combination therapy is strongly recommended.

Skin and soft tissue infections (SSTIs) are a large group of diseases with a wide range of clinically different conditions, some of which can be immediately life-threatening. A number of bacteria play an important role in the etiology of SSTIs, especially gram-positive cocci Staphylococcus aureus and Streptococcus pyogenes. In this case report, a young woman with skin defects after a fasciotomy was treated using a dermal substitute application. Multiple infectious complications in the skin and soft tissues with a high risk of development of phlebitis and with significant intolerance to a variety of antimicrobials were observed. The dramatic SSTI was treated using intravenous administration of dalbavancin, a very potent bactericidal antibio tic representing a rational option in the treatment of SSTIs caused by gram-positive bacteria.
Infekce kůže a měkkých tkání jsou rozsáhlou skupinou onemocnění s širokým spektrem klinických projevů. Některé z těchto infekcí mohou bezprostředně ohrožovat život. V rámci etiologie se uplatňuje celá řada bakterií, mezi epidemiologicky privilegované patogeny patří bezesporu gram-pozitivní koky Staphylococcus aureus a Streptococcus pyogenes. V kazuistice prezentujeme komplikovaný případ mladé ženy s kožními defekty po fasciotomii řešenými aplikací dermální náhrady. U pacientky s výraznou nesnášenlivostí k celé řadě antimikrobiálních preparátů došlo k rozvoji mnohočetných infekčních komplikací v oblasti kůže a měkkých tkání s vysokým rizikem rozvoje flebitid. Dramatický obraz se podařilo zvládnout za pomoci intravenózního podání dalbavancinu, potentního baktericidního antibiotika reprezentujícího racionální možnost v léčbě SSTIs způsobených gram-pozitivními bakteriemi.

Zánětlivá onemocnění kůže a měkkých tkání představují významnou skupinu infekcí člověka. Jejich nejčastějšími původci jsou bakterie Staphylococcus aureus a Streptococcus pyogenes. S ohledem na pokračující růst antibiotické rezistence těchto patogenů se aktuální výzkum zaměřuje na hledání nových látek účinných na infekce, u nichž selhává konvenční antimikrobní terapie. Slibnou v tomto smyslu je léčba založená na antibakteriálním účinku enzymatických preparátů (tzv. enzybiotik), které degradují bakteriální buněčnou stěnu. Tyto látky působí specificky na daný patogen, nikoliv však na kožní mikrobiom, což příznivě ovlivňuje léčebný proces. Enzymy mohou být špatně rozpustné, nestabilní nebo se rychle eliminují z organismu, a proto je snaha o vytvoření tzv. "biobetters", enzymů s vylepšenými vlastnostmi. Důraz je také kladen na vývoj nových nosičů pro enzybiotika či krytů s hojivým efektem, do nichž jsou enzymy inkorporovány.
Inflammatory diseases of the skin and soft tissues are an important group of human infections. The most common causes are the bacteria Staphylococcus aureus and Streptococcus pyogenes. Given the growing resistance of these pathogens to antimicrobials, the current research focuses on the search for novel therapeutic options that would be effective against infections refractory to conventional antimicrobials. A promising alternative is the use of enzyme-based antimicrobials (enzybiotics) that degrade the bacterial cell wall. They target the specific pathogen but do not affect the skin microbiome, thus helping the healing process. As enzymes can be poorly soluble, unstable, or subject to rapid elimination from the body, efforts are made to create biobetters, i.e., enzymes with improved characteristics. Emphasis is also put on the development of novel enzybiotic carriers or wound healing dressings with integrated enzymes.

BACKGROUND: There is no universally accepted opinion on the use of granulocyte transfusions collected using apheresis (GTA) in neutropenic patients and severe infection. PATIENTS AND METHODS: The efficacy and safety of GTAs transfused at a single center over 10 years were analyzed retrospectively. GTAs were harvested from voluntary unrelated donors after priming with methylprednisolone using continuous apheresis and hydroxyethylstarch as sedimentation agent. RESULTS: 41 patients with neutropenia and hematologic malignancy (15 females and 26 males aged 22-69 (median 45.5)) were given a median 3.5 GTAs per patient (range: 1-17) containing a median 1.39×1010 granulocyte/GTA (range: 0.65-2.81). The indications for GTA use were soft tissue inflammation, sepsis, and pneumonia in 30, 22, and 14 cases, respectively. After GTA complete (30 patients: 73.2%) or partial (6 patients: 14.6%) healing of the infection was achieved. The success rate was 91.7% in soft tissue infections, 66.7% in invasive fungal infections, and 68% in sepsis. Septic shock (documented in 12 cases) was associated with a poor response (P<0.03; Chi-square test). Clinical worsening was observed in six cases (14.6%); four patients died. No significant short-term side effects of GTA treatment were recorded. CONCLUSIONS: In our study GTAs collected after steroid priming and used for the treatment of infection during severe neutropenia have shown comparable efficacy with several previously reported trials. However retrospective fashion of our study and inhomogeneous group of patients do not allow any firm conclusions. Prospective studies (including patients' registries) are needed for the better clarification of the role and the dose of GTAs necessary for the successful infection management during neutropenia.
