Přítomnost kardiostimulátoru (KS) nebo implantabilního kardioverteru-defibrilátoru (ICD) v těle pacienta byla doposud absolutní kontraindikací pro vyšetření magnetickou rezonancí (magnetic resonance imaging – MRI). V posledních několika letech jsou na trh uváděny podmíněně MR kompatibilní implantabilní přístroje umožňující pacientům MR vyšetření za specifických podmínek podstoupit. Rekapitulujeme současný stav poznání v této oblasti a přinášíme přehled dostupných MR kompatibilních KS a ICD. Vyšetření magnetickou rezonancí by u těchto pacientů mělo být prováděno v případech, v nichž požadovanou informaci nelze získat jinou zobrazovací metodou.
Implanted cardiac pacemaker (PM) or implantable cardioverter defibrillator (ICD) has been so far considered a contra-indication to magnetic resonance imaging (MRI). In the last few years MRI conditional cardiac implantable electronic devices have been marketed enabling patients undergo MRI under specific conditions. We present current state of the art and provide overview of available MRI conditional devices. Magnetic resonance imaging in these patients should be performed only in cases where the requested information can not be obtained using alternative imaging technique.

Cíl. Prezentovat prvé zkušenosti s užitím MR kompatibilního kardiostimulátoru EnRhythm MRI SureScan s MR kompatibilními elektrodami CapSureFix MRI Materiál, metodika a výsledky. MR kompatibilní kardiostimulátor EnRhythm MRI SureScan s MR kompatibilními elektrodami CapSureFix MRI byl v Nemocnici Na Homolce v rámci klinického zkoušení implantován 52 pacientům, z nichž na základě randomizace 25 podstoupilo MR vyšetření. Tento typ kardiostimulátoru je MR kompatibilní jen za přesně definovaných podmínek na uzavřeném MR systému 1,5 Tesla nejméně 6 týdnů po implantaci. Před uložením pacienta do MR přístroje je nezbytné nastavení stimulátoru do MR kompatibilního modu kardiologem nebo technikem. Po MR vyšetření se provádí zpětné nastavení a kontrola stimulátoru. Izocentrum vyšetření nesmí být uloženo mezi C1 a Th 12. Průměrná specifická míra absorpce (SAR) pro celé tělo smí být maximálně 2,0 W/kg, v oblasti hlavy maximálně 3,2 W/kg, slew rate MR přístroje maximálně 200 T/m/s. Závěr. Doporučujeme ověření správnosti stimulátoru i elektrod pomocí RTG snímku hrudníku. Pacient musí být v průběhu vyšetření monitorován stran vitálních funkcí (EKG nebo pulzní oxymetr nebo monitorace krevního tlaku), nesmí se vyšetřovat v poloze na boku.
Aim. To present first results and experience with the use of a MR-compatible cardiac pacemaker EnRhythm MRI SureScan with MR compatible electrodes CapSureFix MRI. Material, methods and results. A MRcompatible cardiac pacemaker EnRhythm MRI SureScan with MR compatible electrodes CapSureFix MRI was implanted in 52 patients in a clinical study at Na Homolce Hospital. Twenty-five of these were randomized for MR examination. This cardiac pacemaker is MR compatible only when the defined criteria are met on closed-bore 1,5 Tesla systems: The pacemaker has to be implanted for at least six weeks before scheduling MRI scanning. Before the patient is placed into a MR system, the pacemaker has to be switched into a MR compatible mode by a cardiologist or technician. After the scan the pacemaker is switched back and checked. Specific absorption rate (SAR) for the whole body must not exceed 2.0 W/kg, 3.2 W/kg for the cranial region (of the head 3.2 W/kg), with a slew rate that should not exceed 200 T/m/s. Conclusion. We recommend checking the type of the pacemaker and electrodes with a chest x-ray before scanning. The scanning isocenter cannot be focused between C1 and T12. All patients have to be monitored during MRI; i.e., ECG or pulse oxymetry or blood-pressure monitoring. The lateral decubitus position is not allowed.

Obstructive sleep apnea syndrome (OSA) is associated with different types of cardiac arrhythmias. The original studies, concentrated mostly on nocturnal brady- and tachyarrhythmias. More recent studies documented high prevalence of atrial fibrillati on (AF) and its association with obesity and other risk factors for AF. In addition, continuous positive airway pressure (CPAP) prevents recurrence of AF after cardioversion. In, OSA the highest risk for sudden death is at night in comparison to general population most of who die suddenly between six and noon. This observati on suggests that hypoxia or other nocturnal abnormality, trigger sudden death. An important recent finding is the beneficial effect of CPAP on sudden death. The role of pacing in OSA remains controversial. In general, pacemaker therapy is not indicated in patients with nocturnal bradyarrhythmias. However, some authors recommend pacing in those with severe nocturnal bradyarrhythmias not tolerating or not responding to CPAP. According to a recent study, 59% of patients with permanent pacemaker have OSA.

Cardiostimulators are regulated by spontaneous heart signals. Receiving these signals is important for good functioning of the stimulator. There are some signals present in the surroundings of the stimulator that have similar character. They can in certain circumstances interfere with the function of the stimulator.
