Cellular responses induced by surgical procedure or ischemia-reperfusion injury (IRI) may severely alter transcriptome profiles and complicate molecular diagnostics. To investigate this effect, we characterized such pre-analytical effects in 143 non-malignant liver samples obtained from 30 patients at different time points of ischemia during surgery from two individual cohorts treated either with the Pringle manoeuvre or total vascular exclusion. Transcriptomics profiles were analyzed by Affymetrix microarrays and expression of selected mRNAs was validated by RT-PCR. We found 179 mutually deregulated genes which point to elevated cytokine signaling with NFκB as a dominant pathway in ischemia responses. In contrast to ischemia, reperfusion induced pro-apoptotic and pro-inflammatory cascades involving TNF, NFκB and MAPK pathways. FOS and JUN were down-regulated in steatosis compared to their up-regulation in normal livers. Surprisingly, molecular signatures of underlying primary and secondary cancers were present in non-tumor tissue. The reported inter-patient variability might reflect differences in individual stress responses and impact of underlying disease conditions. Furthermore, we provide a set of 230 pre-analytically highly robust genes identified from histologically normal livers (<2% covariation across both cohorts) that might serve as reference genes and could be particularly suited for future diagnostic applications.

Objectives of the study were to investigate impact of ischemic preconditioning (Ipre) and sulforaphane (SFN) and combination of them on nuclear factor 2 erythroid related factor 2 (Nrf2) gene and its dependent genes, heme oxygenase-1 (HO1) and NADPH-quinone oxidoreductase1 (NQO-1) and inflammatory cytokines TNF-alpha, IL1beta, and intercellular adhesion molecule-1 (ICAM1) and caspase-3 in renal ischemia/reperfusion (I/R) injury. Ninety male Sprague Dawely rats were classified into 5 groups (each consists of 18 rats): sham, control, Ipre, sulforaphane and Sulfo+Ipre. Each group was subdivided into 3 subgroups each containing 6 rats according to time of harvesting kidney and taking blood samples; 24 h, 48 h, and 7 days subgroups. Renal functions including serum creatinine, BUN were measured at basal conditions and by the end of experiment. Expression of Nrf2, HO-1, NQO-1, TNF-alpha, IL-1beta, and ICAM-1 was measured by real time PCR in kidney tissues by the end of experiment. Also, immunohistochemical localization of caspase-3 and chemical assay of malondialdehyde (MDA), GSH and SOD activity were measured in kidney tissues. Both Ipre and SFN improved kidney functions, enhanced the expression of Nrf2, HO-1, and NQO-1, attenuated the expression of inflammatory (TNF-alpha, IL-1, and ICAM-1) and apoptotic (caspase-3) markers. However, the effect of sulforaphane was more powerful than Ipre. Also, a combination of them caused more improvement in antioxidant genes expression and more attenuation in inflammatory genes but not caspase-3 than each one did separately. Sulforaphane showed more powerful effect in renoprotection against I/R injury than Ipre as well as there might be a synergism between them at the molecular but not at the function level.

Časně po transplantaci jater jsou tzv. chirurgické/anastomotické komplikace různého typu častější než u transplantací jiných solidních orgánů. Je to dáno nejen tím, že jde o náročný chirurgický výkon, ale také tím, že faktorů, které ovlivňují výsledek transplantace, je velmi mnoho od kvality jaterního štěpu po stav příjemce. Za nejzávažnější komplikaci lze považovat trombózu arteria hepatica. Morfologické znaky v biopsiích jater při trombóze hepatické arterie jsou velmi variabilní od nepříznačné, téměř normální přes koagulační nekrózu, zduření hepatocytů centrolobulárně po biliární patologii s marginální duktulární proliferací anebo s cholangiolitidou. Proto je diferenciální diagnóza široká a zahrnuje skoro každý stav, který může vést k dysfunkci štěpu. Biliární strom je častým místem mírných i významných komplikací a za většinou z nich je ischemické poškození. Komplikace lokalizované v anastomóze žlučových cest nebo v její blízkosti jsou pro léčbu vhodnější než komplikace lokalizované v jaterní tkáni. Early after liver transplantation, surgical/anastomotic complications of various types are more common than in other solid organ transplants. It is not only because the complications can be related to the transplant procedure, but also due to the suboptimal quality of the graft, and/or recipient health issues. The most critical early post-transplant complication is thrombosis of the hepatic artery. Findings in peripheral core needle biopsies of allograft with a thrombosed artery can be quite variable from unremarkable features, to coagulative necrosis, marked centrilobular hepatocyte swelling and biliary features with marginal ductular proliferation and/or cholangiolitis. Therefore the differential diagnosis is broad and includes almost every syndrome associated with graft dysfunction. Biliary tract is frequently the site of minor and major complications and the majority of them are ultimately attributable to ischemic injury. Complications occurring at or near the biliary anastomosis are generally more amenable to treatment than those occurring in the liver parenchyma. V období do 3 měsíců po transplantaci jater představuje cholestáza různého typu a stupně 1 z nejčastějších morfologických nálezů v biopsiích jaterních štěpů. Morfologii cholestázy sdílejí nejen všechny varianty onemocnění s poškozením žlučovodů, ale i další onemocnění s těžším postižením hepatocytů včetně rejekce a rekurence virových hepatitid. Bioptická diagnostika je zlatým standardem při řešení příčin dysfunkce štěpu jater a ve velké většině případů umožňuje odlišit hlavní kategorie chorob vedoucí k cholestáze. V časném období po transplantaci jater se na poškození štěpu obvykle podílí kombinace různých příčin. Proto je interpretace omezených morfologických znaků, které většinou představují určitý typ tkáňové reakce a nikoli noxu, komplikovaná. Nezbytným předpokladem pro co nejlepší interpretaci morfologického závěru se stává úzká spolupráce hepatologa a patologa.
During the first three months after a liver transplant, cholestasis of various type and degree represents 1 of the most frequent morphological findings in liver graft biopsies. The morphology of cholestasis is typical for all conditions with bile duct impairment but also for other conditions with more severe impairment of hepatocytes, including rejection and recurrence of viral hepatitides. Histological diagnosis represents the gold standard in addressing liver graft dysfunction causes, and in the majority of cases it allows for distinguishing between the main categories of diseases resulting in cholestasis. Usually a combination of various changes can be identified as a cause of the liver graft dysfunction early after transplantation. Therefore, the interpretation of limited morphological characteristics, which usually represent a certain type of tissue reaction, not the cause, is complicated. The close cooperation between the hepatologist and pathologist has become a necessary prerequisite for the best possible interpretation of the morphological conclusion.

Cieľ: Cieľom práce bolo overiť hypotézu, či konduktívny prenos tepla pečeňou na začiatku hypotermickej prezervácie má vplyv na jej viabilitu. Metódy: Pečene potkana Wistar (330 ± 10 g) boli prepláchnuté in situ (4 °C) studeným fyziologickým roztokom (FS). Po excízii boli uložené do FS v prezervačno-perfúznej komore (PPK) s počiatočnou teplotou 3,8 ± 0,3 °C (skupina A; n = 7) a 9,5 ± 0,5 °C (skupina B; n = 7). Po 5hodinovej prezervácii vo FS boli pečene reperfundované 75 minút Krebsovým-Henseleitovým roztokom. Výsledky: Ochladzovanie pečení bolo v skupine a rýchlejšie v porovnaní so skupinou B (0,45 ± 0,2 vs 0,18 ± 0,1 °C · min–1; p < 0,01 ). Na druhej strane, počas normotermickej reperfúzie mali pečene v skupine B v porovnaní s pečeňami v skupine a signifikantne vyšší portálny prietok (3,85 ± 0,1 vs 3,21 ± 0,3 ml · min–1 · g–1 pečene; p < 0,05), takmer 2násobne vyššiu tvorbu žlče (0,67 ± 0,05 vs 0,36 ± 0,14 µl · min-1 · g-1 pečene; p < 0,01) a 3–6násobne nižšie uvoľňovanie cytoplazmatických enzýmov: LDH (0,65 ± 0,23 vs 4,26 ± 1,58 µkat · l–1; p < 0,01), ALT (0,25 ± 0,05 vs 0,96 ± 0,10 µkat · l–1; p < 0,001) a AST (0,30 ± 0,02 vs 0,89 ± 0,1 µkat · l–1; p < 0,001). Závery: Výsledky potvrdili, že pomalé konduktívne ochladzovanie na začiatku hypotermickej prezervácie znižuje ischemicko-reperfúzne poškodenie pečene. Background: In liver transplantation, has been reported that hepatic grafts may be impaired by convective heat transfer. We investigated whether conductive heat transfer during the initial period of cold preservation may influence liver viability during the reperfusion period. Methods: Rat livers were flushed out with 4.0 °C of physiologic saline solution. After hepatectomy livers were placed into the preservation-perfusion chamber filled with saline solution at 3.8 ± 0.3 °C (Group A, n = 7) or at 9.5 ± 0.5 °C (Group B, n = 7) and temperature of the preservation solution was monitored. Livers were preserved for 5 hours (equivalent to 16 hours in UW solution) and reperfused using a blood-free perfusion model (Krebs-Henseleit buffer). Results: The rate of cooling of the preservation solution was greater in livers of Group a compared to Group B (0.45 ± 0.2 vs 0.18 ± 0.1 °C · min–1; p < 0.01). On the other hand, livers in Group B compared to Group a exhibited: improved portal flow (3.85 ± 0.1 vs 3.2 ± 0.3 ml · min–1 · g–1 liver ; p < 0.05), appr. 2 times increased bile production (0.67 ± 0.05 vs 0.36 ± 0.14 µl · min–1 · g–1 liver; p < 0.01), and nearly 3–6 fold decreased cytoplasmatic enzymes LDH, ALT and AST release (0.65 ± 0.23 vs 4.26 ± 1.58 µkat · l–1; p < 0.01), (0.25 ± 0.05 vs 0.96 ± 0.10 µkat · l–1; p < 0.001), (0.30 ± 0.02 vs 0.89 ± 0.1 µkat · l–1; p < 0.001) respectively. Conclusions: These results suggest that conductive heat transfer during the initial period of liver preservation has effect on liver viability during reperfusion and that slow coolling of the preservation solution may improve initial hepatic graft function.
