Cemiplimab, anti-PD-1 protilátka, byl začleněn do 1. linie léčby lokálně pokročilého a generalizovaného nemalobuněčného plicního karcinomu v monoterapii nebo v kombinaci s chemoterapií na základě výsledků klinických studií EMPOWER-Lung 1 a EMPOWER-Lung 3.
Cemiplimab, an anti-PD-1 antibody, was incorporated into the first-line therapy of locally advanced and metastatic non-small cell lung cancer as monotherapy or in combination with chemotherapy based on the results of the EMPOWER-Lung 1 and EMPOWER-Lung 3 clinical trials.
Přehledové sdělení předkládá informace o současných možnostech zlepšení výsledků radikální chirurgické léčby nemalobuněčného karcinomu plic, který neobsahuje řídící („targetabilní“) mutace. Původně se využívala perioperační, adjuvantní i neoadjuvantní chemoterapie, jejíž přínos byl s ohledem na nízkou účinnost na hranici 5% zlepšení pětiletého přežití. Imunoonkologická léčba inhibitory kontrolních bodů, podávaná jako monoterapie, kombinace dvou léků nebo v kombinaci s chemoterapií, výrazně zlepšuje dobu do progrese onemocnění a celkové přežití nemocných, jako účinnější i ekonomicky výhodnější se přitom ukazuje být léčba neoadjuvantní. K markerům její účinnosti patří dosažení parciální remise choroby, patologické kompletní remise před operací, prodloužení času do recidivy onemocnění a nízké hladiny cirkulující nádorové DNA nebo její vymizení v průběhu neoadjuvantní terapie. V současnosti je k dispozici především kombinace chemoterapie s nivolumabem podávaná předoperačně, z možností adjuvantní imunoterapie přichází v úvahu podávání atezolizumabu po radikální resekci nádorů stadia IB–IIIA s vysokou expresí PD-L1 (více než 50 %).
This review presents information on current possibilities to improve outcomes of radical surgical treatment of non-small cell lung cancer without targetable driver mutations. Initially, perioperative, adjuvant and neoadjuvant chemotherapy was used, with a marginal benefit of 5% improvement in 5-year survival given its low efficacy. Immuno-oncological treatment with checkpoint inhibitors, administered as monotherapy, as a combination of two drugs or in combination with chemotherapy, significantly improves the time to disease progression and overall survival of patients, while neoadjuvant treatment appears to be more effective and economically advantageous. Markers of its efficacy include achievement of partial remission of disease, pathological complete remission before surgery, prolongation of time to disease recurrence, and low levels of circulating tumour DNA or its disappearance during neoadjuvant therapy. Currently, combinations of chemotherapy with nivolumab preoperatively are mainly available, while adjuvant immunotherapy options include administration of atezolizumab after radical resection of stage IB-IIIA tumours with high PD-L1 expression (more than 50%).
Adjuvantní chemoterapie na bázi platiny je dlouhodobě standardní léčbou po operaci karcinomu plic ve stadiu IB až III s mírným přínosem pro celkové přežití. Vzhledem k účinnosti nových léčiv u pokročilého onemocnění se postupně a logicky tyto léky prosazují i v léčbě časnějších stadií a nedávné studie prokázaly u vybraných skupin pacientů přínos adjuvantní cílené léčby i imunoterapie. V současné době je patrný trend k zařazení těchto léků u operabilních pacientů co nejdříve v rámci neoadjuvantních a perioperačních léčebných postupů a začlenit též nové možnosti molekulární biologie, jako například monitoraci případného reziduálního onemocnění do běžné praxe.
Adjuvant platinum-based chemotherapy has long been the standard treatment after surgery for stage IB to III lung cancer with a modest benefit on overall survival. Given the efficacy of new drugs in advanced disease, these drugs are gradually and logically gaining ground in the treatment of earlier stages, and recent studies have demonstrated benefit of both adjuvant targeted therapy and immunotherapy in selected patient groups. There is now a clear trend to include these drugs in operable patients as early as possible in neoadjuvant and perioperative therapies, and to incorporate new molecular biology options such as monitoring for potential residual disease into routine practice.

Tato inovativní léčba, schválená Evropskou lékovou agenturou (EMA) v prosinci 2022, se zaměřuje na cílenou iradiaci buněk karcinomu prostaty s minimálním poškozením okolních zdravých tkání. Představuje významný pokrok ve zlepšení přežití a kvality života pacientů s metastatickým kastračně rezistentním karcinomem prostaty (mCRPC) po progresi na léčbě cílené na androgenový receptor a chemoterapii.
177Lu-PSMA-617 represents a targeted radioligand therapy that offers new hope for patients with advanced prostate cancer. Approved by the European Medicines Agency (EMA) in December 2022, this innovative therapy targets targeted irradiation of prostate cancer cells with minimal damage to surrounding healthy tissues. It represents a significant advance in improving survival and quality of life for patients with metastatic castration- -resistant prostate cancer (mCRPC) after progression on androgen receptor-targeted therapy and chemotherapy.
Pokročilý karcinom prostaty (KP) má relativně často změny v genech, které se účastní opravy DNA. V tom případě můžeme opravu zlomů DNA zablokovat pomocí inhibitorů poly(adenosindifosfát-ribóza)polymerázy (PARP). Nejvíce dat máme o účinnosti inhibitoru PARP olaparibu, který je možné podat pacientům s metastatickým kastračně rezistentním KP (mCRPC) a pozitivitou mutace BRCA1/2 po předchozí léčbě abirateronem nebo enzalutamidem. Druhou indikaci představuje kombinace olaparibu s abirateronem v 1. linii léčby mCRPC (bez nutnosti genetického testování), pokud není klinicky indikována chemoterapie. Talazoparib můžeme podat v kombinaci s enzalutamidem pacientům s mCRPC, u kterých také není indikována chemoterapie. Testování nádorových mutací není vyžadováno. Obdobně také niraparib v kombinaci s abirateronem můžeme použít u pacientů s mCRPC a mutacemi genu BRCA1/2, u kterých nezvažujeme chemoterapii. Výsledky v léčbě mCRPC má ještě rukaparib, který v České republice zatím není možné mužům s KP podávat.
Advanced prostate cancer (PC) relatively often has alterations in genes involved in DNA repair. In that case, we can block the repair of DNA breaks using poly-adenosine diphosphate-ribose-polymerase (PARP) inhibitors. The most studied PARP inhibitor is olaparib, which can be given to patients with metastatic castration-resistant PC (mCRPC) and a positive BRCA1/2 mutation after previous treatment with abiraterone or enzalutamide. The next indication is the combination of olaparib with abiraterone in the firstline treatment of mCRPC (without the need for genetic testing), if chemotherapy is not clinically indicated. Talazoparib can be given in combination with enzalutamide to patients with mCRPC who are also not indicated for chemotherapy. Tumor mutation testing is not required. Similarly, we can also use niraparib in combination with abiraterone in patients with mCRPC and BRCA1/2 gene mutations, if chemotherapy is not considered. Also rucaparib has results in the treatment of mCRPC, but is not yet available for men with PC in the Czech Republic.
Metastatický hormonálně senzitivní karcinom prostaty (mHSPC) je různorodé onemocnění vyžadující individuální léčebný přístup. Androgen-deprivační terapie (ADT) je základem terapie diseminovaného onemocnění s velmi dobrou iniciální odpovědí na léčbu u většiny pacientů, ale s limitovaným trváním odpovědi. Léčba ADT v kombinaci s ARTA (androgen receptor targeted agents – abirateron, apalutamid, enzalutamid) významně prodlužuje přežití bez progrese (PFS) i celkové přežití (OS), a stala se tak standardem v léčbě pacientů s mHSPC. Obdobného účinku dosahuje i kombinovaná léčba ADT s docetaxelem, tedy tzv. dublet. U části pacientů s agresivním onemocněním ale tato terapie není dostatečně účinná. Jednou z možností, jak zvýšit účinnost léčby, je kombinace ADT s ARTA a docetaxelem, tedy tzv. triplet. V článku se zaměříme na základní studie týkající se této problematiky a pokusíme se definovat pacienty, pro které je tato kombinovaná léčba výhodná.
Metastatic hormone-sensitive prostate cancer is a diverse disease requiring an individual treatment approach. Androgen deprivation therapy (ADT) is the mainstay of treatment for disseminated disease with a very good initial response to treatment in most patients, but with a limited duration of response. Combined treatment with ADT and ARTA (androgen receptor targeted agents – abiraterone, apalutamide, enzalutamide) significantly prolongs progression-free survival (PFS) and overall survival (OS) and has thus become the standard of treatment for patients with mHSPC. A similar effect is achieved by combined treatment with ADT and docetaxel, the so-called doublet therapy. However, this treatment is not effective enough for some patients with aggressive disease. One possibility to increase the effectiveness of treatment is the combination of ADT with ARTA and docetaxel, the socalled triplet therapy. In this article, we will focus on basic studies regarding this issue and try to define patients for whom this combined treatment is beneficial.
