Hereditární angioedém je závažné a vzácné autozomální dominantně dědičné onemocnění, při kterém dochází k tvorbě nebolestivých, nesvědivých otoků podkoží a sliznic. Příčinou je genová mutace, která způsobuje absolutní či relativní nedostatek (afunkčního proteinu) C1 inhibitoru. V populaci se vyskytuje s frekvencí 1 : 50 000 až 1 : 10 000. V léčbě hereditárního angioedému se uplatňují 3 léčebné strategie, mezi které zahrnujeme dlouhodobou profylaxi, krátkodobou profylaxi a terapii akutního stavu. K dlouhodobé profylaxi jsou užívány v perorální formě inhibitory esteráz (kyselina tranexamová) či anabolické steroidy (atenuovaný androgen), u těžkých pacientů pravidelně intravenózně aplikovaný C1-inhibitor Cinryze. Krátkodobá profylaxe je nezbytná před traumatizujícími iatrogenními výkony substitucí purifikovaným koncentrátem C1-inhibitoru (Berinert P, Cinryze, Ruconest). Terapie akutního stavu je zásadní při rozvoji obstrukce laryngu, akutních gastrointestinálních potížích, deformujících kožních otoků. Používán je blokátor bradykininového receptoru RB2 (icatibant – Firazyr) či substituce C1-inhibitory buď plazmatickými (Berinert, Cinryze), nebo rekombinantním (conestat alfa – Ruconest). Příznivým a novým trendem je včasná samoléčba akutních atak samotným pacientem, ke které může být užit subkutánně aplikovaný icatibant (Firazyr). K domácí léčbě jsou taktéž povoleny intravenózně aplikované C1-inhibitory (Berinert, Cinryze, Ruconest). Rozhodnutí o využití domácí léčby by měl u každého pacienta individuálně učinit ošetřující lékař, který musí zajistit vhodnou instruktáž a v určitých intervalech kontrolovat techniku podávání.
Hereditary angioedema is a serious and rare autosomal dominant hereditary disease manifested by the formation of painless, nonpruritic subcutaneous and/or submucosal swelling. It is caused by a gene mutation resulting in an absolute or relative lack of C1 inhibitor (nonfunctional protein). It occurs with a frequency of 1 : 50 000 to 1 : 10 000 in the population. The treatment of hereditary angioedema involves the use of three therapeutic strategies comprising long-term prophylaxis, short-term prophylaxis, and acute state treatment. Long-term prophylaxis involves the use of oral esterase inhibitors (tranexamic acid) or anabolic steroids (attenuated androgen); in severe cases, regular intravenous administration of the C1-inhibitor Cinryze. Short-term prophylaxis with a purified C1-inhibitor concentrate (Berinert P, Cinryze, Ruconest) is necessary before traumatizing iatrogenic procedures. The treatment of an acute state is essential when laryngeal obstruction, acute gastrointestinal complaints, or deforming skin edema develop. A bradykinin B2 receptor antagonist (icatibant – Firazyr) or a replacement with either plasma (Berinert, Cinryze) or recombinant (conestat alfa – Ruconest) C1-inhibitors are used. A new and positive trend is early self-treatment of acute attacks by the patient themselves for which subcutaneous icatibant (Firazyr) can be used. Also available for home treatment are C1-inhibitors (Berinert, Cinryze, Ruconest) administered intravenously. The decision on utilizing home treatment should be made individually for each patient by the attending physician who must provide appropriate instruction and check the technique of administration in certain intervals.

Hereditární angioedém (HAE) je vzácné vrozené onemocnění, jehož hlavním klinickým projevem jsou masivní otoky podkoží a/nebo sliznic v důsledku nekontrolované aktivace komplementového a kininového systému. Podstatou onemocnění je mutace genu (SERPING1) pro C1 inhibitor serinové proteázy, způsobující buď jeho výrazně sníženou tvorbu, nebo afunkčnost. Jedná se o vrozený deficit C1 inhibitoru složky komplementu s autozomálně dominantním typem přenosu. Imunodeficit je klasifikovaný na HAE s deficiencí C1 inhibitoru (HAE-C1-INH, dříve HAE-I a HAE-II) a HAE s normální hladinou a funkcí C1 inhibitoru (HAE nC1-INH), označovaný též jako HAE-III typu s jinými mutacemi.Péče a léčba pacientů s HAE je celoživotní, včetně mezigenerační péče o celé rodiny. S rozšiřováním škály dostupných preparátů a zaváděním dlouhodobé profylaxe atak jsou pacienti, na rozdíl od minulého století, schopni kvalitního života bez omezení chorobou. Vznik HAE center v ČR před 10 lety postavil péči o tyto pacienty na úroveň evropských standardů.
Hereditary angioedema (HAE) is a rare congenital disease whose main clinical manifestation is massive swelling of the subcutaneous and / or mucous membranes due to uncontrolled activation of the complement and kinin systems. The cause of the disease is a mutation in the gene (SERPING1) for the C1 serine protease inhibitor, causing its either significantly reduced production or dysfunction. It is a congenital deficien-cy of the C1-inhibitor of the complement component with an autosomal dominant type of transmission. Immunodeficiency HAE is classified to a deficiency of C1-inhibitor (HAE-C1-INH, previously HAE-I and-II) and HAE with normal levels and functions C1 inhibitor (INH-nC1 HAE), also referred to as type HAE-III. with other mutations. Care and treatment of patients with HAE is a lifelong including intergenerational care for the whole families. Thanks to the expansion of the range of available drugs and the introduction of long-term prophylaxis of attacks, patients are, in contrast to the last century, able to live a quality life without being limited by the disease. The establishment of HAE centers in the Czech Republic 10 years ago raised the care of these patients to the level of European standards.
