Novou možnosťou prevencie diabetickej obličkovej choroby (nefropatie) u pacientov s ochorením diabetes mellitus 2. typu sú inhibítory sodíkovo-glukózového kotransportéra 2 (SGLT2), resp. gliflozíny, ktoré okrem dobrej glykemickej kontroly a nízkeho rizika hypoglykémie vykazujú signifikantný kardiovaskulárny a nefroprotektívny benefit. Klinické štúdie zamerané na kardiovaskulárnu bezpečnosť potvrdili redukciu aj sekundárneho renálneho kompozitu o 44 % pri liečbe empagliflozínom (štúdia EMPA-REG OUTCOME), o 40 % pri liečbe kanagliflozínom (štúdia CANVAS) a o 24 % v prípade dapagliflozínu (štúdia DECLARE TIMI-58). Nová štúdia CREDENCE u diabetikov s chronickou chorobou obličiek v štádiu 2–3b potvrdila nefroprotektivitu kanagliflozínu v zmysle redukcie primárneho renálneho kompozitu o 30 %. U pacientov s diabetickou obličkovou chorobou liečba kanagliflozínom má aj kardiovaskulárny benefit s redukciou kardiovaskulárneho kompozitu o 20 % a hospitalizácii pre srdcové zlyhávanie o 39 %.V novom rozšírenom SPC je kanagliflozín indikovaný aj ako doplnok štandardnej liečby pacientov s diabetickým ochorením obličiek v dávke 100 mg/d.

Sodium-glucose co-transporter 2 (SGLT2) inhibitors, gliflozins are a new possibility of prevention of diabetic kidney disease (nephropathy) in patients with type 2 diabetes mellitus. Apart from glycemic control and low risk of hypoglycemia, gliflozins have been shown to provide significant cardiovascular and nephroprotective benefit. CVOT studies also confirmed the reduction of secondary renal composite by 44 % during the treatment by empagliflozin (study EMPA-REG OUTCOME), by 40 % in canagliflozin treatment (study CANVAS) and by 24 % in the case of dapagliflozin (study DECLARE TIMI-58). New study CREDENCE in diabetic patients with stage 2–3b of chronic kidney disease confirmed nephroprotective effect of canagliflozin in terms of reduction of primary renal composite about 30 %. In patients with diabetic kidney disease treatment with canagliflozin is associated with cardiovascular benefit with reduction of cardiovascular composite about 20 % and hospitalization for heart failure about 39 %. According to new extended SPC canagliflozin is indicated also as addition to standard treatment of patients with diabetic kidney disease in dose 100 mg/day.

EMPRISE (Empagliflozin Comparative Effectiveness and Safety) je prebiehajúca rozsiahla RWE (Real World Evidence) štúdia, ktorá začala v roku 2016 a ktorej hlavným cieľom je zistiť, či kardiovaskulárny benefit empagliflozínu, ktorý bol pozorovaný v štúdii EMPA-REG OUTCOME, sa potvrdí aj v podmienkach bežnej klinickej praxe. Štúdia vychádza z údajov pacientov zozbieraných zo zdravotníckych databáz v USA a porovnáva výsledky u pacientov, u ktorých sa liečba začala empagliflozínom verzus inhibítorom DPP4 sitagliptínom. Prvé výsledky plánovanej interim analýzy štúdie EMPRISE nielenže potvrdili zistenia RCT CVOT štúdie EMPA-REG OUTCOME v otázke redukcie hospitalizácií pre srdcové zlyhávanie (HSZ), ale tiež ukázali že benefit na HSZ sa dostavuje aj u pacientov bez predošlého kardiovaskulárneho ochorenia (KVO) či bez predošlého srdcového zlyhávania (SZ), a teda že liečba by mohla byť prínosná aj v primárnej prevencii SZ a mala by byť preferovaná pred DPP4i, obzvlášť u pacientov, u ktorých je cieľom je redukovať rozvoj SZ a jeho dôsledkov. Hoci výsledky RWE štúdií môžu byť limitované možnými reziduálnymi premennými, dôsledný dizajn štúdie a rozsiahly "propensity score matching“ štúdie EMPRISE minimalizoval možné zdroje "biasu“, čo oprávňuje dôveryhodnosť a spoľahlivosť výsledkov tejto štúdie.

EMPRISE (Empagliflozin Comparative Effectiveness and Safety) is an ongoing, extensive RWE (“real world evidence”) study launched in 2016, whose main goal is to establish whether the CV benefit of empagliflozin as observed in the study EMPA-REG OUTCOME will also be confirmed under the conditions of normal clinical practice. The study is based on patient data collected from US medical databases and compares the results relating to patients who started treatment with empagliflozin versus a DPP-4 inhibitor, sitagliptin. The first results of the planned interim analysis of the EMPRISE study not only confirmed the findings of the RCT CVOT of the study EMPA-REG OUTCOME on the reduction of hospitalizations for heart failure (HHF), but also showed that HHF-related benefit is also present in patients without previous cardiovascular disease (CVD) or without previous heart failure (HF), and thus treatment could also be beneficial in primary prevention of HF and should be preferred over DPP4i, especially in patients where the goal is to reduce the development of HF and its consequences. Although the results of RWE studies may be limited by possible residual variables, the consistent study design and extensive “propensity score matching” of EMPRISE minimized possible sources of “bias”, which supports credibility and reliability of the results of this study.

EMPA-REG OUTCOME is recognised by international guidelines as a landmark study that showed a significant cardioprotective benefit with empagliflozin in patients with type 2 diabetes (T2D) and cardiovascular disease. To assess the impact of empagliflozin in routine clinical practice, the ongoing EMPRISE study is collecting real-world evidence to compare effectiveness, safety and health economic outcomes between empagliflozin and DPP-4 inhibitors. A planned interim analysis of EMPRISE was recently published, confirming a substantial reduction in hospitalisation for heart failure with empagliflozin across a diverse patient population. In this commentary article, we discuss the new data in the context of current evidence and clinical guidelines, as clinicians experienced in managing cardiovascular risk in patients with T2D. We also look forward to what future insights EMPRISE may offer, as evidence is accumulated over the next years to complement the important findings of EMPA-REG OUTCOME.

Gliflozíny, resp. inhibítory sodíko-glukózového kotransportéra 2 (SGLT2i) sú najnovšou skupinou antidiabetických liekov indikovaných na liečbu diabetes mellitus 2. typu. Okrem dobrej glykemickej kontroly a nízkeho rizika hypoglykémie vykazujú signifikantný kardiovaskulárny (KV) benefit. Významne znižujú riziko srdcového zlyhávania, ako ukázali štúdie EMPA-REG OUTCOME s empagliflozínom, CANVAS s kanagliflozínom a DECLARE-TIMI-58 s dapagliflozínom v podskupine pacientov s vysokým KV-rizikom. V štúdii EMPRISE sa v porovnaní s agonistami receptorov pre glukagón podobný peptid 1 potvrdil benefit empagliflozínu nielen na zníženie hospitalizácie pre srdcové zlyhávanie, ale porovnateľný vplyv aj na zníženú incidenciu aterosklerotických KV-príhod. V sekundárnej analýze štúdie EMPA-REG OUTCOME pri hodnotení všetkých opakovaných príhod sa liečba empagliflozínom spájala s redukciou incidencie infarktu myokardu o 21 %, hlavných koronárnych príhod (infarkt myokardu, alebo koronárna revaskularizácia) o 20 %, KV-mortality o 38 % a celkovej mortality z akejkoľvek príčiny o 31 %. Klinické a experimentálne štúdie poukazujú na skutočnosť, že liečba gliflozínmi sa okrem redukcie srdcového zlyhávania spája aj s dlhodobým priaznivým pôsobením na aterosklerotické kardiovaskulárne ochorenia.

Gliflozins or sodium-glucose co-transporter 2 (SGLT2) inhibitors, are the newest class of diabetic medications indicated for the treatment of type 2 diabetes mellitus. Apart from glycaemic control and low risk of hypoglycaemia, gliflozins have been shown to provide significant cardiovascular benefit. They significantly reduce risk of heart failure, as it was shown in studies EMPA-REG OUTCOME with empagliflozin, CANVAS with canagliflozin and DECLARE-TIMI-58 with dapagliflozin in subgroup of patients with high cardiovascular risk. In the study EMPRISE there were confirmed the benefit of empagliflozin not only in reduced hospitalization for heart failure but also comparable effect on reduced incidence of atherosclerotic cardiovascular events. In secondary analysis of EMPA-REG OUTCOME study with evaluation of first plus every recurrent event there were confirmed that treatment by empagliflozin is associated with reduction of incidence of myocardial infarction by 21 %, main coronary events (myocardial infarction or coronary revascularization) by 20 %, cardiovascular mortality by 38% and total mortality from every reasons by 31 %. Clinical and experimental studies suggest, that gliflozin treatment is associated with not only reduction of heart failure, but also with long term benefit an atherosclerotic cardiovascular diseases.
