The article deals with and discusses two main approaches in building semantic structures for electrophysiological metadata. It is the use of conventional data structures, repositories, and programming languages on one hand and the use of formal representations of ontologies, known from knowledge representation, such as description logics or semantic web languages on the other hand. Although knowledge engineering offers languages supporting richer semantic means of expression and technological advanced approaches, conventional data structures and repositories are still popular among developers, administrators and users because of their simplicity, overall intelligibility, and lower demands on technical equipment. The choice of conventional data resources and repositories, however, raises the question of how and where to add semantics that cannot be naturally expressed using them. As one of the possible solutions, this semantics can be added into the structures of the programming language that accesses and processes the underlying data. To support this idea we introduced a software prototype that enables its users to add semantically richer expressions into a Java object-oriented code. This approach does not burden users with additional demands on programming environment since reflective Java annotations were used as an entry for these expressions. Moreover, additional semantics need not to be written by the programmer directly to the code, but it can be collected from non-programmers using a graphic user interface. The mapping that allows the transformation of the semantically enriched Java code into the Semantic Web language OWL was proposed and implemented in a library named the Semantic Framework. This approach was validated by the integration of the Semantic Framework in the EEG/ERP Portal and by the subsequent registration of the EEG/ERP Portal in the Neuroscience Information Framework.

Cíl: Cílem průzkumu bylo zjistit míru informovanosti rodičů či případného doprovodu dětského pacienta ohledně práv, která plynou z Charty práv dětí v nemocnici na území České republiky. Metodika: Pro realizaci průzkumného šetření byl použit anonymní dotazník vlastní konstrukce, který byl rozdělen na dvě části. První část byla pro společná pro všechny rodiče, druhá část byla pouze pro rodiče, kteří již měli zkušenost s hospitalizací dítěte v nemocnici. Soubor tvořilo 403 rodičů či zákonných zástupců, z nichž 205 již mělo osobní zkušenost s hospitalizací dítěte v nemocnici. Výsledky: Průzkum ukázal, že 66 % respondentů již o existenci Charty práv dítěte v nemocnici slyšelo. Dále bylo zjištěno, že pro rodiče je aktivní spoluúčast při hospitalizaci dítěte velmi důležitá a že právo dítěte na hospitalizaci spolu s rodičem znají. Respondenti poukázali i na to, že tato Charta, hlavně její etická část, často nebyla dodržována ze strany zdravotnického personálu. Respondenti ze strany zdravotnických pracovníků volnou otázku využili k poukázání na možné problémy ze strany rodičů při provádění léčebných a diagnostických výkonů a nedodržováním léčebného režimu ze strany rodičů. Závěr: Uvedenou práci je možno využít k lepší orientaci v problematice dětských práv a ke zkvalitnění péče o hospitalizované děti a jejich doprovod.
Objectives: The point of the research was to find out, how well are the parents or the legal guardians of child patient informed about the rights arising from The Charter for Children in Czech hospital. Metodology: To realization of investigation was used an self made anonymous questionnaire. It was divided into two parts, the first part was common to all the parents, the second part was only for parents who have had experience with hospitalization of the child in the hospital. The sample group contained 403 parents or legal guardians. From the total number, 205 had already personal experience with their children hospitalization. Results: The research results suggested that most parents had knowledge about the Hospitalized Children Rights Charter. Also it was found that the active participation of parents in the child‘s hospitalization is very important for them and the right to be present with child in the hospital all the time is known. The respondents pointed out that the Chart (mainly the ethical part) was not adhered by medical staff. On the other hand, the hospital staff pointed out the potential problems with parents being present during the treatment or diagnostic procedure and not adhering to treatment plans. Resume: The thesis can be used for better orientation in the field of children‘s rights and to improve the care of hospitalized children and their legal guardians.

BACKGROUND: Since 2010, the Czech Republic has been experiencing a spontaneous phenomenon in which General Practitioner (GPs) practices have been transferred to a legal limited liability company as defined by the Commercial Code. OBJECTIVES: Our research attempted to ascertain the reasons that induced GPs to change their legal form, thereby opting for an entrepreneurial orientation (EO). A second objective was to measure the impact of this change on the healthcare performance of GP practices. METHODOLOGY/APPROACH: We used exploratory research to examine this phenomenon in healthcare service provision. Data from 131 questionnaires was evaluated using an exploratory survey with descriptive statistics. To assess the healthcare performance of GP practices, we used secondary data from the Institute of Health Information and Statistics of the Czech Republic. FINDINGS: The GPs interviewed considered their practices to be businesses. Their decision to change to a limited company form was influenced by the possibility of selling or transferring the business, employment sustainability and job security, and advantages related to cost structure and taxation. Our study shows that, in a generational exchange, the change in legal form enhances the sustainability and operation of the practice within the context of the current demands for high-quality healthcare.

Background: Recently, the Austrian Patient Summary (APS) and Austrian Microbiology Report (AMR) document definitions were added to the Austrian national electronic health record repository. How to employ these standardized, structured patient data in clinical decision support (CDS) systems remains an interesting research subject. Objectives: To create a proof of concept for the emergency use of APS and AMR documents in a variety of CDS platforms and standards in an intensive care setting, in order to generate new insights by linking individual documents to different CDS approaches. Methods: APS and AMR definitions were stored using MongoDB, a document-oriented database system. For implementation of the clinical use case CDS, we selected the Drools business rule management system, as well as the ArdenSuite software for implementing the HL7 Arden Syntax for Medical Logic Systems standard. Results: Due to its manifold features, implementation of the CDS use case in Drools could be done quite efficiently without the need for additional coding of technical or algorithmic code, which results in smaller rules. In contrast, the use case implementations in Arden Syntax using the ArdenSuite required additional technical coding, distracting from the medical knowledge implementation. The Arden Syntax as a knowledge definition standard is, however, better understandable due to its resemblance to natural language compared to the Java-like definition language used in Drools. Conclusion: With the nation-wide availability of structured documents, the foundation for the implementation of clinical decision support systems has been laid. Commonly used open-source platforms offer extensive possibilities for the implementation of CDS systems. This evaluation of modern business rule management systems will advance the implementation of powerful clinical decision support solutions valuable to all stakeholders.
