The present study was conducted to investigate the virulence gene profile and antimicrobial resistance of non-typeable Streptococcus suis isolates circulating in pigs of North East India. Fifty-two non-typeable S. suis isolates from clinically healthy and diseased pigs were screened by using PCR for the presence of the muramidase-released protein (mrp), extracellular factor (epf), hemolysin suilysin (sly), arginine deiminase (arcA), and glutamate dehydrogenase (gdh) genes. Five different virulence gene profiles were observed and the most predominant virulence gene profile found in healthy pigs was mrp- + sly- + arcA- + gdh + + epf- whereas the most predominant virulence gene profile recorded in diseased pigs was mrp+ + sly- + arcA+ + gdh+ + epf-. Significantly lower carrier rate of mrp+ + sly- + arcA+ + gdh+ + epf- virulence gene profile was observed among the isolates from healthy pigs compared to those from diseased pigs (P < 0.05). Antimicrobial resistance patterns of the S. suis isolates revealed fourteen resistance groups (R1 to R14) where 88.46% isolates showed multi-drug resistance. The most predominant resistance pattern observed was CD-COT-E-TE. This is perhaps the first study reporting virulence gene profile and antimicrobial resistance of non-typeable S. suis isolates from pigs in North East India. The occurrence of relatively high levels of resistance of S. suis to some antimicrobials (e.g. macrolides, tetracyclines, and sulphonamides) as observed in the present study may represent a human health concern.

Streptococcus suis is an important pathogen of pigs but is also transmissible to humans, with potentially fatal consequences. Among 29 serotypes currently recognized, some are clinically and epidemiologically more important than others. This is particularly true for serotypes 2 and 14, which have a large impact on pig production and also on human health. Conventional PCR-based serotyping cannot distinguish between serotype 1/2 and serotype 2 or between serotype 1 and serotype 14. Although serotype 1/2 and serotype 2 have a very similar cps locus, they differ in a single-nucleotide substitution at nucleotide position 483 of the cpsK gene. Similarly, serotypes 1 and 14 have a very similar cps locus but also differ in the same nucleotide substitution of the cpsK gene. Fortunately, this cpsK 483G→C/T substitution can be detected by BstNI restriction endonuclease. A PCR-restriction fragment length polymorphism (RFLP) detection method amplifying a fragment of the cpsK gene digested by BstNI restriction endonuclease was developed and tested in reference strains of these serotypes and also in field isolates.

Práce se zabývá studiem grampozitivní bakterie Streptococcus suis. Jedná se o zoopatogena, který je schopný vy-volávat sporadické infekce lidí. Tyto infekce jsou zaznamenávány i v České republice . Touto prací se na ně snažíme upozornit a zkvalitnit jejich diagnostiku. Doposud byly využívány převážně metody klasické mikrobiologie a nyní se do možného spektra použití zavedly i metody molekulární.
The focus is on the study of the Gram-positive bacterium Streptococcus suis, a zoopathogen which causes spo-radic infection in humans. These infections also occur in the Czech Republic. This study draws attention to these infections in order to improve their diagnosis. To date, conventional microbiological methods have mostly been used for the diagnosis, and recently, molecular methods have also become available.
Streptococcus suis je významný patogen prasat s celosvětovým výskytem a možným přenosem infekce na člověka. Hlášeny byly sporadické výskyty onemocnění lidí po celém světě, včetně Evropy. U člověka může vést k rozvoji řady závažných stavů. Mezi nejčastější patří meningitida a sepse. Prezentovaná kazuistika popisuje rozvoj rozsáhlé klinické manifestace infekce od sepse, spondylodiscitidy, paravertebrálních abscesů až k osteomyelitidě a poškození sluchu. Onemocnění ve svém důsledku vedlo k několikaměsíční hospitalizaci a těžké invalidizaci pacienta.
Streptococcus suis is regarded to be a significant pathogen in swine with global prevalence and possible transmission of infection to humans. Sporadic cases of infection have been reported worldwide including Europe. Such infection in humans leads to development of various serious conditions, including, most frequently, meningitis and sepsis. The presented case report describes extensive clinical presentation of the infection, ranging from sepsis, spondylodiscitis and abscess formation in paravertebral muscles to osteomyelitis and hearing impairment. In our case, the disease led to several months of hospitalization and severe disability. Streptococcus suis is an important pathogen of pigs. In China, in addition to S. suis serotype 2, S. suis serotype 9 (SS9) is also a prevalent serotype. There is no vaccine available for SS9. An immunoproteome-based approach was developed to identify SS9 immunogenic proteins for vaccine development. Secreted proteins extracted from SS9 strain GZ0565 were screened by two-dimensional Western blotting using convalescent sera from pigs. Protein spots were excised from preparative gels and were identified by matrix-assisted laser desorption ionization time-of-flight mass spectrometry, which led to the identification of ten immunogenic proteins (sortases, ABC transporter substrate-binding protein-maltose/maltodextrin, ABC transporter periplasmic protein, CHAP domain containing protein, peptidoglycan-binding LysM, elongation factor Tu, elongation factor G, thymidine kinase, molecular chaperone DnaK, hypothetical protein SSU98_2184). These novel immunogenic proteins, which are encoded by genes that are reasonably conserved among SS9 strains, may be developed as antigens for further study of SS9 vaccine.

This study was aimed to examine the genetic characteristics of 44 Streptococcus suis type 2 (SS2) isolates and the virulence attributes of 23 representative isolates. Multilocus sequence typing revealed five sequence types (ST1, ST7, ST28, ST86, and ST162) with 19 isolates assigned to ST7 (43.2%), 14 to ST1 (31.8%), and 9 to ST28 (20.5%). PCR typing of the pilus gene clusters classified the isolates into three types: A (72.7%), B (22.7%), and N (4.5%). All isolates of pilus types A and N were assigned to the ST1 complex containing ST1, ST7, and ST86, while those of type B belonged to the ST27 complex comprising ST28 and ST162. Only two strains had the putative pathogenicity island 89-kb cluster (89K) and were of type N. The type B strains had a significantly lower adhesion, were more readily killed by macrophage, and had lower virulence to mice than those of types A and N. We conclude that SS2 strains of both ST1 and ST27 complexes, parallel to pilus types A and B, were prevalent in the pig populations in Zhejiang Province, and ST7 and ST1 strains were the predominant genotypes in the diseased pigs with pneumonia.

Hlavním hostitelem Streptococcus suis je prase domácí a v současné době se streptokokové infekce vyskytují v chovech prasat po celém světě. Infekce se projevuje v podobě pneumonie, purulentní meningoencefalitidy, sepse atd. Přenos na člověka je velmi snadný a průběh onemocnění velmi podobný. Až donedávna byly tyto infekce u lidí považovány za relativně vzácné zoonózy. K průniku do organizmu dochází nejčastěji poraněnou kůží při manipulaci s masem infikovaných prasat, a v největším riziku jsou tudíž zaměstnanci jatek, kafilérií a chovných stanic. Prezentované kazuistiky poukazují na případy dvou pacientů hospitalizovaných na Klinice infekčních chorob Fakultní nemocnice Brno s purulentní meningoencefalitidou. U obou pacientů byl jako původce infekce prokázán Streptococcus suis. Diagnóza byla podpořena i pozitivní epidemiologickou anamnézou.
A main host of Streptococcus suis is a pig and streptococcal infections occur in nearly all countries with pig industry. The infection manifests usually as pneumonia, purulent meningoencephalitis, septicaemia etc. A transmission to humans is easy and course of illness is similar. Until recently these diseases have been known as relatively rare zoonosis. A transmission of disease is mostly by injured skin during contaminated meat handling. So in a great risk there are people working on abattoir and pig farms. This article presents cases of two people hospitalized in Department of Infectious Diseases Faculty Hospital Brno because of purulent meningoencephalitis. Both patients suffered from infection caused by Streptococcus suis. Positive epidemiologic anamnesis supported the diagnosis.

Bakterie Streptococcus suis je běžným patogenem u prasat po celém světě. Je odpovědná za řadu nemocí prasat a působí značné každoroční celosvětové ekonomické ztráty v živočišné výrobě. K nákaze dochází u osob, které byly v kontaktu s nemocnými nebo infikovanými prasaty, tedy především u chovatelů a ošetřovatelů vepřů, veterinárních lékařů a techniků a u zpracovatelů vepřového masa. Ve většině případů se tedy jedná o profesionální onemocnění. K přenosu nákazy dochází cestou kožních poranění včetně oděrek, cestou slizničních lézí dutiny ústní a nosní, méně často inhalační cestou i nepoškozenou orofaryngeální sliznicí. Onemocnění se klinicky projevuje nejčastěji jako meningitida, eventuálně septikemie, pneumonie, endokarditida. Vývoj syndromu toxického šoku je spojen s vysokým stupněm mortality (celosvětově průměrně 17 %). U 75 % nemocných s meningitidou vzniká trvalá porucha sluchu a rovnováhy. Nutným předpokladem pro úspěšnou léčbu je přesná diagnóza etiologického činitele. Nejčastější sérotyp S. suis – typ 2 je zpravidla citlivý na penicilin a ampicilin, cefalosporiny a cotrimoxazol. V kazuistice je popsán ojedinělý případ onemocnění purulentní meningitidou u mladé ženy, ošetřovatelky prasat. Po odeznění akutní fáze se objevila audio-vestibulární porucha, která vedla k trvalé invaliditě pacientky. Na nákazu S. suis je třeba pomýšlet v případě septikemie komplikované meningitidou u dospělého s anamnézou předchozího kontaktu s prasaty nebo tepelně neupraveným vepřovým masem. V prevenci vzniku této nákazy je zásadní důraz kladen na důslednou a zodpovědnou osobní ochranu každého exponovaného jednotlivce.
The bacterium Streptococcus suis is a common pathogen in pigs worldwide. It is responsible for a range of diseases in pigs and causes large economic losses in livestock production annually. Individuals that come into contact with sick or infected animals are at risk: primarily pig breeders, veterinarians, vet technicians, butchers and other workers engaged in raw pork processing. Most of these cases are therefore classified as occupational diseases. The bacterium enters the body through a cut/wounds or lesions of the skin or mucosa of the mouth and nasal cavity; infection through intact oropharyngeal mucosa is less common. The disease usually manifests itself clinically as meningitis (septicaemia, pneumonia or endocarditis are rare). The development of toxic shock syndrome is connected with a high mortality rate (overall average of 17 % throughout the world). 75 % of patients with meningitis suffer from permanent hearing loss and vestibular dysfunction. Precise diagnosis of the aetiologic agent is necessary for successful treatment. The most commonly isolated S. suis serotype, type 2, is generally sensitive to penicillin, ampicillin, 2nd and 3rd generation cephalosporins and cotrimoxazol. The article describes an isolated case of purulent meningitis in the Czech Republic in a young woman working as a pig keeper. The development of audio-vestibular disorder after the acute phase passed led to permanent disability in this patient. Infections caused by S. suis must be taken into account in cases of septicaemia with meningitis in adults with a history of previous contact with pigs or uncooked/undercooked pork. The most important prevention measure against the occurrence of this infection is on-going and responsible personal protection of individuals in risk environments.

Zoonózy stále hrají významnou roli v humánní i veterinární medicíně. Jejich eradikace je obtížná a někteří autoři předpokládají, že jejich význam bude nadále narůstat. Mezi původce infekčních onemocnění přenosných ze zvířat na člověka patří také řada grampozitivních bakterií. Autoři se zaměřili na tři původce, kteří představují rizika ve třech rozdílných oblastech – alimentární infekce (Listeria monocytogenes), profesionální infekce (Streptococcus suis) a infekce chovatelů tzv. „pets“zvířat (Staphylococcus aureus a Staphylococcus intermedius). Práce analyzuje rizika infekce člověka z animálních zdrojů a stručně informuje o aktuálním výskytu původců u zvířat, případně živočišných surovin v regionu spádové oblasti Státního veterinárního ústavu Olomouc (Morava).
Zoonoses continue to play a signicant role in both human and veterinary medicine. Their eradication is difficult and some authors are convinced of their growing importance. Among the pathogens of infectious diseases transmissible from animals to man are several gram-positive bacteria. The authors focus on three agents that present a risk in three different areas - alimentary infections (Listeria monocytogenes), professional infections (Streptococcus suis) and infections of breeders of so-called „pet" animals (Staphylococcus aureus and Staphylococcus intermedius). The paper analyses the risk of human infections from animal sources and briefly informs on the present incidence of pathogens in animals and/or raw materials of animal origin in the catchment area of the State Veterinary Institute in Olomouc, Moravia.
