The incidence and the burden of cardiovascular disease (CVD), coronary heart disease (CHD), type 2 diabetes mellitus (T2DM), and the metabolic syndrome are greatly increasing in our societies. Together, they account for 31% of all deaths worldwide. This chapter focuses on the role of two revolutionary discoveries that are changing the future of medicine, induced pluripotent stem cells (iPSCs) and CRISPR/Cas9 technology, in the study, and the cure of cardiovascular and metabolic diseases.We summarize the state-of-the-art knowledge about the possibility of editing iPSC genome for therapeutic applications without hampering their pluripotency and differentiation, using CRISPR/Cas technology, in the field of cardiovascular and metabolic diseases.
The metabolism of sulfur-containing amino acids (SAAs) requires an orchestrated interplay among several dozen enzymes and transporters, and an adequate dietary intake of methionine (Met), cysteine (Cys), and B vitamins. Known human genetic disorders are due to defects in Met demethylation, homocysteine (Hcy) remethylation, or cobalamin and folate metabolism, in Hcy transsulfuration, and Cys and hydrogen sulfide (H2S) catabolism. These disorders may manifest between the newborn period and late adulthood by a combination of neuropsychiatric abnormalities, thromboembolism, megaloblastic anemia, hepatopathy, myopathy, and bone and connective tissue abnormalities. Biochemical features include metabolite deficiencies (e.g. Met, S-adenosylmethionine (AdoMet), intermediates in 1-carbon metabolism, Cys, or glutathione) and/or their accumulation (e.g. S-adenosylhomocysteine, Hcy, H2S, or sulfite). Treatment should be started as early as possible and may include a low-protein/low-Met diet with Cys-enriched amino acid supplements, pharmacological doses of B vitamins, betaine to stimulate Hcy remethylation, the provision of N-acetylcysteine or AdoMet, or experimental approaches such as liver transplantation or enzyme replacement therapy. In several disorders, patients are exposed to long-term markedly elevated Met concentrations. Although these conditions may inform on Met toxicity, interpretation is difficult due to the presence of additional metabolic changes. Two disorders seem to exhibit Met-associated toxicity in the brain. An increased risk of demyelination in patients with Met adenosyltransferase I/III (MATI/III) deficiency due to biallelic mutations in the MATIA gene has been attributed to very high blood Met concentrations (typically >800 μmol/L) and possibly also to decreased liver AdoMet synthesis. An excessively high Met concentration in some patients with cystathionine β-synthase deficiency has been associated with encephalopathy and brain edema, and direct toxicity of Met has been postulated. In summary, studies in patients with various disorders of SAA metabolism showed complex metabolic changes with distant cellular consequences, most of which are not attributable to direct Met toxicity.

Tato skutečnost se odráží v závazných doporučeních chirurgických společností, ale také v terapeutických algoritmech jiných společností, např. diabetologických. Jsou definovány standardní výkony, které tvoří podstatnou část provedených operací celosvětově, a také výkony, u kterých zatím chybí jednoznačný průkaz jejich efektivity. Objevují se také nové typy výkonů, které se zaměřují více než na redukci hmotnosti na úpravu komorbidit. Je patrná snaha o snižování invazivity výkonů, což vytváří prostor pro endoskopické výkony s asistencí laparoskopie nebo čistě endoskopické výkony. Autoři v práci prezentují současné názory na postavení metabolické chirurgie, prezentují nejčastější výkony a představují také novější chirurgické i endoskopické postupy, kterým možná bude patřit budoucnost metabolické chirurgie.
The term metabolic surgery has become familiar to the professional and lay public with the beginning of the new millennium. The focus is not only on weight reduction, but also on the improvement of obesity-related medical conditions. This is reflected in guidelines of surgical societies as well as in the therapeutic algorithms of other, for instance diabetology, societies. Standard procedures which represent a significant portion of the operations performed worldwide, along with procedures still lacking adequate proof of effectiveness, have been defined. There are also new types of interventions focusing rather on comorbidity treatment than on weight reduction. Effort to reduce invasiveness of the procedures has been evident, paving the way for endoscopic procedures with laparoscopic assistance or purely endoscopic procedures. The authors present the current view on the status of metabolic surgery, reporting on the most frequently performed procedures and also introducing newer surgical and endoscopic interventions which may represent the future of metabolic surgery.

Česká republika se díky dlouhodobé tradici úzké spolupráce obezitologů a chirurgů řadí na přední evropské příčky v poskytování vysoce kvalitní komplexní mezioborové péče nemocným obezitou a s ní souvisejících metabolických onemocnění. Největší vliv na rozvoj bariatricko-metabolické chirurgie v českých zemích měla bezesporu pražská 1. lékařská fakulta UK a Všeobecná fakultní nemocnice v Praze. Mezi velké úspěchy české bariatrie patří některá mezinárodní prvenství a také postavení oboru v mezinárodním kontextu. Článek tyto priority a uznání zmiňuje podrobněji. Ve svých historických začátcích měla chirurgická léčba obezity prakticky jediný terapeutický cíl, a to zajistit výrazné váhové úbytky u vysoce obézních pacientů, u kterých nedošlo ke snížení hmotnosti konzervativní léčbou. Na začátku nového tisíciletí ovšem získávají mnohem důležitější místo indikace k chirurgickému zákroku s primárním cílem zlepšení či úplné remise některých s obezitou spojených nebo obezitou způsobených metabolických onemocnění, zejména diabetu 2. typu. Rodí se tak metabolická chirurgie, u níž je kritériem léčebného úspěchu nikoli váhová redukce, ale efektivní ovlivnění metabolického onemocnění, a to prakticky u všech stupňů obezity. Kromě zásadního posunu bariatrické chirurgie směrem k metabolické získává v posledním desetiletí na zásadní důležitosti i hledisko šetrnosti vykonů (od miniinvazivních a laparoskopických k ještě méně invazivním výkonům), ať již z hlediska perioperační zátěže nebo důrazu na provádění anatomicky co nejšetrnějších a co nejméně nevratných zákroků. Projevuje se to zkoušením a zaváděním stále nových operačních postupů.
Czech Republic may be counted among the leading European Countries in regards of the level of delivered high quality multidisciplinary care in treatment of obesity and obesity related metabolic diseases. The 1st Faculty of Medicine (Charles University) and the Faculty General Hospital in Prague played the most important role in the development of bariatric and metabolic surgery in the Czech Republic. pCzech bariatric surgery achieves great successes both on national and international levels. Just to mention some of them: M. Fried and M. Pešková were among the very first worldwide to implant the non-adjustable gastric banding laparoscopically in 1993, the Czech Republic was among the seven Countries to establish the International Federation for the Surgery of Obesity /IFSO/ (Fried in 1995), to organize the first IFSO World Congress in Prague (1996), to co-establish the IFSO-European Chapter in Prague (2004), to implant the first SAGB VC worldwide (Fried, Doležalová, 2007), to organize the first European Workshop on Gastric Plication (Fried, Doležalová, 2010), to co-lead development of the European Interdisciplinary Guidelines on Metabolic and Bariatric Surgery (Fried et al.,2013), and many others. In the beginning of bariatric surgery, the almost only indication criterion for operation was the criterion of weight loss. On the turn of the Century metabolic surgery gradually gained importance. The most important indication criterion for metabolic operations started to be improvement and/or resolution of obesity related co-morbidities, such as type 2 diabetes mellitus. Thus, the criterion of successful treatment shifted from weight loss towards improvement and resolution of metabolic diseases regardless the body mass index. In conjunction with importance of metabolic surgery, more emphasis is given to lowering the invasiveness of so far available minimally invasive/laparoscopic approaches form the perioperative perspective as well as from the anatomically sparing/reversible surgeries.

Akutní renální selhání je závažný, život ohrožující stav spojený s poruchou metabolizmu a často s rozvojem uremického syndromu. Příčiny mohou být prerenální (porucha cirkulace), renální (patologie ledvinné tkáně) nebo postrenální (obstrukce močových cest). Stav často vyžaduje akutní dialýzu. V případě přítomnosti obstrukce močových cest je nutno urgentně zajistit derivaci moče. Při nefron-šetřících výkonech na ledvinách je nutné minimalizovat dobu teplé ischémie jako prevenci akutní tubulární nekrózy.
Acute renal failure is life-threatening situation associated with severe metabolic pathology rarely presented with uremia. Causes could be pre-renal (insufficient blood supply), renal (renal parenchyma pathology) or post-renal (urinary tract obstruction). Acute situation usually needs acute haemodialysis. It is mandatory to set urinary diversion in case of urinary tract obstruction presence immediately. It is necessary to minimize warm ischemia time during nefron-sparing procedures on renal parenchyma as the acute tubular necrosis prevention.
