Specifická alergenová imunoterapie je unikátní kauzální personalizovaný přístup k léčbě alergií. Jednou se základních podmínek úspěšné terapie je adekvátní dávkování. Mimo studie v rámci požadavků evidence-based medicine (EBM) osvědčujících efektivitu léčivých produktů pro určitou konkrétní udržovací (denní) dávku, najdeme i studie srovnávající účinnost různých dávek („vícedávkové“ studie). Z nálezů obou typů studií se odvozují doporučené udržovací (denní) dávky pro léčbu jednotlivých druhů inhalačních alergií, respektive vzhledem k jedinečnému a nepřenositelnému charakteru extraktových terapeutik různých výrobců pro konkrétní léčivé produkty.
Specific allergen immunotherapy is a unique causal personalized approach to allergy treatment. One of the basic conditions of successful treatment is adequate dosage. In addition to studies within the framework of EBM requirements certifying the effectiveness of medical products for a specific maintenance (daily) dose, we also find studies comparing the effectiveness of different doses (dose-response studies). Recommended maintenance (daily) doses for the treatment of individual types of inhalation allergies are derived from the findings of both types of studies, respectively due to the unique and non-transferable nature of extract therapeutics from different manufacturers for specific therapeutic products

Značný počet pacientů s průduškovým astmatem není pod uspokojivou kontrolou ani při intenzifikaci léčby kombinací inhalačního kortikosteroidu (IKS) s další léčebnou modalitou (standardně LABA). Přidání dlouhodobě působících anticholinergik (LAMA) do trojkombinace má potenciál v těchto případech žádoucí kontrolu nad astmatem zajistit, a to při zachování plné tolerance léčby. Mimo specifický případ syndromu překryvu astmatu a CHOPN (ACOS) jsou pro léčbu asthma bronchiale v České republice aktuálně indikovány a hrazeny tři inhalační přípravky s obsahem LAMA. Jedním z nich je fixní trojkombinace IKS/LABA/LAMA v inhalátoru Breezhaler® pod názvem Enerzair®. Článek navazuje na farmakologické profily přípravku Enerzair® Breezhaler® zveřejněné v tomto časopise dříve a zamýšlí se nad využitím inhalačních anticholinergik a konkrétně léčivého přípravku Enerzair® v praxi astmatologů.
A considerable number of patients with bronchial asthma are not under satisfactory control even with treatment intensification combining inhalation corticosteroid (ICS) with another treatment modality such as long-acting β2 agonist (LABA). The addition of long-acting muscarinic antagonists (LAMA) to triple combination has the potential to ensure sufficient control in these cases, with the maintenance of complete treatment tolerance. Currently, there are three inhalation drugs containing LAMA indicated and reimbursed in the Czech Republic, except for asthma-COPD overlap syndrome. One of them is a fixed triple combination of ICS/LABA/LAMA in the Breezhaler® inhaler called Enerzair®. The article follows the pharmacological profiles of Enerzair® Breezhaler® published earlier in this magazine, and reflects the use of inhalation anticholinergics, specifically with Enerzair®, in the practice of asthmatologists.

I z hlediska celospolečenského je třeba podtrhnout objektivní význam této choroby, a to zejména rýmy chronické a alergické. Článek má za cíl předložit základní přehled problematiky alergické rýmy od její charakteristiky, přes klasifikaci, význam až k léčebné intervenci. Vysvětluje pozici alergologa v systému péče o pacienty postižené alergickou rýmou a současně podrobněji rozvádí možnosti režimových opatření a obecné léčby. Navazuje tak na předchozí přehledové publikace v tomto časopise.

Even in society-wide terms, it is necessary to underscore the objective significance of this condition, particularly that of chronic and allergic rhinitis. The aim of the article is to present a fundamental review of the issue of allergic rhinitis, ranging from its characteristics to classification and significance to therapeutic intervention. It also explains the role of an allergist in the system of care for patients affected by allergic rhinitis as well as deals in more detail with the options of lifestyle measures and general treatment. This article is a follow-up to previous review papers in this journal.
