Asthma, rhinitis, and atopic dermatitis (AD) are interrelated clinical phenotypes that partly overlap in the human interactome. The concept of "one-airway-one-disease," coined over 20 years ago, is a simplistic approach of the links between upper- and lower-airway allergic diseases. With new data, it is time to reassess the concept. This article reviews (i) the clinical observations that led to Allergic Rhinitis and its Impact on Asthma (ARIA), (ii) new insights into polysensitization and multimorbidity, (iii) advances in mHealth for novel phenotype definitions, (iv) confirmation in canonical epidemiologic studies, (v) genomic findings, (vi) treatment approaches, and (vii) novel concepts on the onset of rhinitis and multimorbidity. One recent concept, bringing together upper- and lower-airway allergic diseases with skin, gut, and neuropsychiatric multimorbidities, is the "Epithelial Barrier Hypothesis." This review determined that the "one-airway-one-disease" concept does not always hold true and that several phenotypes of disease can be defined. These phenotypes include an extreme "allergic" (asthma) phenotype combining asthma, rhinitis, and conjunctivitis. Rhinitis alone and rhinitis and asthma multimorbidity represent two distinct diseases with the following differences: (i) genomic and transcriptomic background (Toll-Like Receptors and IL-17 for rhinitis alone as a local disease; IL-33 and IL-5 for allergic and non-allergic multimorbidity as a systemic disease), (ii) allergen sensitization patterns (mono- or pauci-sensitization versus polysensitization), (iii) severity of symptoms, and (iv) treatment response. In conclusion, rhinitis alone (local disease) and rhinitis with asthma multimorbidity (systemic disease) should be considered as two distinct diseases, possibly modulated by the microbiome, and may be a model for understanding the epidemics of chronic and autoimmune diseases.

Přibližně jedna třetina světové populace trpí některou formou alergického onemocnění. Nejčastější je alergické postižení dýchacích cest vznikající na podkladě přecitlivělosti na inhalované alergeny ‒ pylová zrna, roztoče, plísně a alergeny domácích zvířat. Běžná farmakoterapie spolu s režimovými opatřeními tlumí alergické příznaky u většiny pacientů, ale jedinou léčbou, která je schopna ovlivnit dlouhodobě průběh nemoci, je specifická alergenová imunoterapie. Kvalitní terapeutické extrakty nejvýznamnějších kauzálních alergenů jsou nyní k dispozici ve formě subkutánních injekcí, sublinguálních tablet nebo roztoků. Přestože alergenovou imunoterapii u nás aplikuje výhradně alergolog, je velmi důležité, aby se s ní seznámili i lékaři v první linii kontaktu s pacientem a byli schopni vytipovat pacienty indikované k této léčbě.
About one third of the world population suffers from allergy. Most prevalent are the diseases of respiratory tract which develop on the base of allergy to pollen grains, hous dust mites, molds and pets. The conventional pharmacotherapy together with avoidance strategies help to control allergic symptoms in the majority of patiens, but allergen immunotherapy is the only approach that has the potential to alter the natural course of allergic disease. All the most important therapeutic extracts are now available in good quality ether in the form of subcutaneous injections or sublingual tablets or solutions. Though the allergen immunotherapy is introduced only by allergists, it is important that also primary care providers have the knowledge about it and are able to select the patiens convenient to this type of therapy.

Většina pacientů s alergickou rýmou nedosahuje dostatečné úlevy od potíží, přestože se s ní léčí. Přitom nabídka možností, jak alergickou rýmu zvládnout, se zdá být široká. Osobní zkušenost se skutečným pacientem v jeho přirozeném prostředí s možností posoudit konečné výsledky léčby je však nenahraditelná pro budoucí správnou volbu ve správné chvíli. Kazuistika dokáže takové informace zprostředkovat, aniž bychom se museli obávat o osud pacienta. Následná diskuse navíc přinese řadu důležitých faktů, které se díky spojení s příběhem lépe zapamatují. Základem farmakoterapie alergické rýmy jsou antihistaminika, topické nosní kortikosteroidy a alergenová imunoterapie. Pro úspěch naší léčby je zcela zásadní adekvátní znalost jejich farmakokinetiky a farmakodynamiky. Dokonalý přehled o aktuálně dostupných přípravcích nám pak umožní včas reagovat na dynamickou situaci na trhu s léčivy.
Most patients with allergic rhinitis do not report satisfactory relief from bothersome symptoms in spite of using therapy. Nevertheless, there are many effective options how to cope with this condition. Personal experiences with real patients in their natural settings with an opportunity to assess the outcome are the best way how to identify the right decision at the right time in future. A case report may hand such information over without having to worry about the patient’s health. Subsequent discussion brings lots of extra facts that are important and easier to remember when linked to a story. Antihistamines, intranasal corticosteroids and allergen immunotherapy are the mainstay of pharmacotherapy for allergic rhinitis. However, adequate knowledge of their pharmacokinetics and pharmacodynamics is quite essential to achieve success with our treatment plan. Besides that, keeping track of pharmaceuticals available at the moment enables us to respond soon enough to a quickly changing situation on the medicine market.

OBJECTIVE: The aim of the present study was to investigate the effect of allergic rhinitis on the success of the operation in chronic otitis surgery by using score for allergic rhinitis (SFAR). MATERIALS AND METHODS: In the present study; 121 patients, who underwent type 1 tympanoplasty were examined retrospectively. SFAR of all patients were recorded. The graft success rates of 26 patients with allergic rhinitis (AR) and 95 patients with no allergic rhinitis group (NAR) were compared. RESULTS: While the graft success rate in NAR group was 89.5%, this rate was 80.8% in the AR group. However, the difference between groups was not statistically significant (p = 0.311). CONCLUSION: These findings suggest that allergic rhinitis decreases the graft success rate of the pathologies occurring in eustachian tube, middle ear and mastoid although statistically significant difference wasn't found. Prospective studies with larger patient groups are required in order to evaluate this pathology.
