Background: Review article points out the importance of nurses in the decision-making process, which is an important part of the cognitive activities of a nurse and is one of the main skills of characterizing the professional nursing. The Declaration of the conference in Vienna in 1988 states that the role of the nurse is that it has to act as a partner in decision-making and planning (WHO, 1997). Objective: The main aim is to definine the decision-making process and decision-making in nursing. On these bases, describe the factors influencing decision-making in nursing, barriers to decision-making for nurses and also the importance of participatory decision-making. Methods: The contained theoretical bases are the result of the analysis of monographs, periodicals and legislative documents. Via the special keywords have been searching monographic resources and articles in scientific databases and then these articles were then subjected to content analysis. Results: The decision and decision-making process in nursing were defined. The factors influencing decision-making process in nursing and also the barriers in nursing decision-making process have been described. Due to the nature of nurse´s work the importance of participatory decision-making process have been also described. Conclusion: In the context of the provided care, nurse has to make numbers of decisions. The decision-making processes in nursing are influenced by many factors. But ultimately, they always affect patients care.

People in their merits tend to associate, communicate and solve reasons of their behaviour. The same situation is in nursing which is demonstrated by many organizations and groups seeking to develop nursing. Professional nursing organisations (further nursing organisations) present in public and at the same time represent nurses while co-operating with other professional or state institutions. They endeavour to develop the branch and significantly supervise provision of professional quality and education of their members in particular countries. The paper – pursuant to content analysis of the found foreign papers from the scientific databases – presents factors influencing the interest of nurses to get involved in nursing organizations. The papers were sought pursuant to key words and the predetermined limits.
Působení porodních asistentek v terénu má ve zdravotnictví ČR dlouhou tradici. Jejich činnost je však v důsledku reorganizace zdravotnictví na počátku 90. let na většině míst ČR zrušena, jedině ve Zlínském kraji je terénní péče porodní asistentky opravdu součástí zdravotnického systému. V rámci výzkumu bylo zjišťováno, zda si potřebnost této péče uvědomují občané ČR. Sdělení objasňuje stanoviska laické i odborné veřejnosti k místům, ve kterých by měly komunitní porodní asistentky především působit a jaké činnosti by měly vykonávat. Výsledky byly získány na základě grantového projektu, který je zaměřený na uplatnění komunitní sestry v systému českého zdravotnictví. K realizaci byl proveden řízený rozhovor s 1007 lékaři, s 1005 sestrami a 2020 občany-laiky.

Článek seznamuje s dílčími výsledky grantového projektu, který je zaměřený na uplatnění komunitní sestry v systému českého zdravotnictví. Komunita je dnes pokládána za významné prostředí pro poskytování zdravotní péče, která je bezpečná, laskavá, úsporná a kvalitní jak pro matky, tak pro kojence a rodiny. Podle G. Leifera (2004) je současná porodnická praxe zaměřená na co nejlepší zkušenost celé rodiny a porodnictví uznává rodinu jako jedinečný systém. K realizaci projektu byl proveden řízený rozhovor s 1 007 lékaři, s 1 005 sestrami a 2 020 občany-laiky. Prvotním záměrem je analýza klíčových názorů na možnost uplatnění sestry a porodní asistentky v oblasti komunitní péče. Ke statistickému zpracování byl užit program SASD, bylo provedeno třídění prvního i druhého stupně. Uvedený příspěvek je zaměřen na oblast zajištění péče o ženu porodní asistentkou pro komunitní péči. Největší část dotázaných občanů neví, jaká je současná podoba práce porodních asistentek pro komunitní péči v České republice. Jejich stanovisko k této otázce je závislé na pohlaví, věku a vzdělání. Odborná i laická veřejnost se shoduje v tom, že porodní asistentka pro komunitní péči by měla poskytovat péči především v terénu. Lékaři jsou na rozdíl od sester a občanů častěji toho názoru, že služby komunitních porodních asistentek nejsou potřebné. Odborná a laická veřejnost jednoznačně preferuje takové činnosti, které jsou spojené s předporodní a poporodní péčí o ženy-rodičky a o novorozené. Podle světových i českých legislativních podkladů porodní asistentka poskytuje zdravotní péči a rady ženám během těhotenství, při porodu a v šestinedělí, pokud probíhají fyziologicky, podílejí se na vedení fyziologického porodu a poskytování péče o novorozence; součástí této zdravotní péče je také ošetřovatelská péče o ženu na úseku gynekologie. Dále se porodní asistentka ve spolupráci s lékařem podílí na preventivní, léčebné, diagnostické, rehabilitační, neodkladné nebo dispenzární péči.
The article acquaints the reader with partial results of a grant project focused on the role of community nurses in the system of Czech health services. The community is today considered as an important environment for mothers, suckling babies and families for granting the health care, which is safe, kind, economical and of good quality. According to G. Leifer, the contemporary obstetric practice is aimed at the best experience of the whole family and the obstetrics recognizes the family as a unique system. For the project implementation, a controlled interview was implemented with 1007 physicians and 1005 nurses – professionals, and with 2020 citizens – laymen. The primary intention is the analysis of crucial opinions concerning the possibility of the role of nurses and midwives in the field of the community care. The SADS programme was employed for the statistical processing, and classing of the first and second degrees was provided. The contribution presented here is focused on the field of providing the care for women by midwives within the community care. A majority of citizens inquired do not know what the contemporary nature of the work of midwives for the community care in the Czech Republic is. Their standpoint concerning this question is dependent on the gender, age and education. The professional as well as lay public agrees in the opinion that the midwife for the community care should particularly provide the care under field conditions. The physicians, in contrast to nurses and citizens, more frequently consider that services of community midwives are not necessary. The professional as well as lay public unambiguously prefers activities, which are associated with taking prenatal and postnatal care of delivering women and newborns. In accordance with world and well as Czech legislative basic data, the midwife provides the healthcare and advices to women in the course of pregnancy, during the delivery and in the puerperium (in the case of their physiological nature), conduction of the physiological delivery and taking care of newborns; this healthcare also includes taking the nursing care of women in the field of gynaecology. The midwife in cooperation with the physician furthermore participates in the preventive, therapeutic, diagnostic, rehabilitation, urgent or subsequent care.
