Aim: Autonomy in nursing refers to nurses' ability to make independent decisions in providing nursing care without outside influence or control. Nurse autonomy is a key part of the provision of quality and safe care. Design: A narrative literature review was used for the study, performed according to the PRISMA ScR checklist. Methods: Relevant studies obtained from four databases were included in the narrative review. The data review was performed according to the PRISMA checklist. For data extraction and synthesis, the method of summative content analysis, in accordance with COSMIN criteria, was used. Results: The search identified four relevant studies published between 2003-2014. All instruments used in the studies were designed to measure nurse autonomy and were also used in combination with other instruments closely related to autonomy in nursing. The instruments demonstrated acceptable psychometric properties. Conclusion: Autonomy is an unexplored phenomenon in the context of the Slovak Republic and it deserves attention since nurse autonomy affects not only patients but also nurses themselves and their job satisfaction. It is important to enable nurses to participate in decision-making, planning and development of nursing as autonomous professionals.

Pôrodná asistencia ako vedný odbor a regulované povolanie je usmerňované viacerými právnymi predpismi SR na úrovni parlamentu a vlády SR (zákony, nariadenia vlády, odborné usmernenia, koncepcie odborov a pod), ako aj na úrovni Ministerstva zdravotníctva Slovenskej republiky a Ministerstva školstva Slovenskej republiky v oblasti vzdelávania sestier a pôrodných asistentiek. Právne predpisy poskytujú pôrodnej asistencii právne uznanie, podporujú ich profesionálnu autonómiu tým, že vymedzujú kompetencie, vytvárajú rámec intervencií v pôrodnej asistencii, ktoré sú v starostlivosti o pacienta legálne a zároveň diferencujú zodpovednosť pôrodnej asistentky od zodpovednosti iných zdravotníckych pracovníkov. Nezávislosť profesie vo významnej miere podporuje legislatíva, ako aj iné prostriedky dostupné v praxi.
Midwifery as a scientific discipline and regulated profession is governed by various legal regulations in the Slovak Republic at the level of the Parliament and Government of the Slovak Republic (laws, government regulations, professional guidelines, field concepts, etc.), as well as at the level of the Ministry of Health and the Ministry of Education of the Slovak Republic in the area of nurse and midwife education. These legal regulations provide midwifery with legal recognition and support its professional autonomy by defining competencies, establishing a framework for interventions in midwifery care that are legally applicable to patient care, and distinguishing the responsibilities of midwives from those of other healthcare professionals. The independence of the profession is significantly supported by legislation as well as other tools available in practice.

Autonómia je dôležitou súčasťou profesionálneho rozvoja ošetrovateľstva. V ošetrovateľstve sa vzťahuje na schopnosť sestry robiť nezávislé rozhodnutia pri poskytovaní ošetrovateľskej starostlivosti bez vonkajšieho vplyvu alebo kontroly. Sestry s autonómiou sú kľúčovou súčasťou pre poskytovanie kvalitnej a bezpečnej starostlivosti. Autonómia patrí v našom socio-kultúrnom kontexte k nepreskúmaným fenoménom. Je ovplyvnená environmentálnymi a kultúrnymi podmienkami. Ovplyvňuje nielen spokojnosť pacientov, ale aj samotné sestry a ich spokojnosť s prácou.
Autonomy is an important part of nursing professional development. In nursing, it refers to the nurse’s ability to make independent decisions in the delivery of nursing care without external influence or control. Nurses with autonomy are a key component to providing quality and safe care. Autonomy is one of the unexplored phenomena in our socio-cultural context. It is influenced by environmental and cultural conditions. It affects not only patient satisfaction but also the nurses themselves and their job satisfaction.

Pôrodná asistentka (PA) je v rámci svojho vzdelania pripravená poskytovať zdravotnú starostlivosť o ženu počas tehotenstva, pôrodu aj postnatálneho obdobia. Predpokladaný nedostatok pracovnej sily, slabá motivácia pre voľbu profesie pôrodnej asistentky na Slovensku môže mať vážne dopady na starostlivosť o ženu. Zdá sa, že táto skutočnosť je ovplyvnená viacerými faktormi. Aby pôrodné asistentky mohli plniť svoju profesionálnu úlohu, musia byť splnomocnené a nadobudnúť autonómiu pri práci. Cieľom štúdie bolo posúdiť vnímanie postavenia pôrodných asistentiek v spoločnosti na Slovensku. Štúdie sa zúčastnilo 354 pôrodných asistentiek (vo veku 18-49 rokov). Na zber údajov bol použitý dotazník (PEMS - Revised) pre účely skúmania Empowermentu (sila postavenia profesie), ktorý sme následne vyhodnotili pomocou deskriptívnej štatistiky, Mann- -Whitney U a Kruskal-Wallis testov (p<0,05). Cronbach alfa koeficient bol 0,83. V rámci porovnania jednotlivých subškál PEMS škály sa preukázalo, že pôrodné asistentky vnímajú ako najslabšiu oblasť Podporu od nadriadeného 3,25 (± 1,34) a Profesijné uznanie 3,27 (± 1,03). Tieto oblasti je na Slovensku potrebné výrazne zlepšiť zo strany ostatných zdravotníckych pracovníkov a dosiahnuť zlepšenie postavenia profesie pôrodnej asistentky v spoločnosti. Štúdia podporuje psychometrické vlastnosti PEMS-škály.
Midwives are educated to provide care for women during pregnancy, birth and the postnatal period. The expected midwifery workforce shortages and weak motivation for choosing the profession of midwife in Slovakia can have serious implications for the care of women. This fact seems to be influenced by several factors. In order to fulfil the professional role, midwives are to be empowered and gain autonomy at work. The aim of the study was to investigate the perception of the position of midwives in society in Slovakia. The research sample consisted of 354 midwives (aged 18-49). The PEMS – Revised questionnaire to examine empowerment in midwifery (the profession position strength) was used for data collection, which were subsequently evaluated using descriptive statistics, the Mann- -Whitney U and the Kruskal-Wallis tests (p <0.05). The Cronbach alpha coefficient was 0. 83. In the comparison of individual subscales of the PEMS scale, it was shown that midwives perceive Superior support 3.25 (± 1.34) and Professional recognition 3.27 (± 1.03) as the weakest areas. In Slovakia, it is necessary to significantly improve these areas as well as the position of the midwife profession in society. Our research study supports the psychometric properties of the PEMS-scale.

INTRODUCTION: European guidelines on the care of haemophilia recommend ready access to a range of services provided by a multidisciplinary team of specialists including physiotherapy. However, the scope of physiotherapy provided is unknown. METHODS: The Physiotherapists Committee of the European Association for Haemophilia and Allied Disorders (EAHAD) conducted a web-based survey to quantify the role and scope of practice of physiotherapists involved in haemophilia care. The survey was sent to more than 200 physiotherapists registered on the EAHAD database. Questions concerned their work practices including assessment and treatment activities and level of autonomy. RESULTS: Eighty physiotherapists from twenty-four European countries responded. Considerable heterogeneity exists in roles, responsibilities, and clinical practice of physiotherapists, particularly in access to and type of physiotherapy treatment provided, as well as the skill set and autonomy of physiotherapists to make independent assessment and treatment decisions. DISCUSSION: This pan-European survey establishes a context to support physiotherapy role development and professional identity. Key recommendations include the following: (a) establishing a pan-European network to support collaboration and education for physiotherapists working in haemophilia, (b) developing a core skills and capability framework to ensure person-centred approaches are central as well as working in partnership with those with the condition to maximize early recovery, support self-management and enablement in remaining active and independent, (c) regular training, standardized validation and maintenance of competency for assessment tools, (d) well-designed randomized clinical studies with larger numbers of participants from multiple sites should be the focus of future research.

BACKGROUND: Managing medical professionals is challenging because professionals tend to adhere to a set of professional norms and enjoy autonomy from supervision. The aim of this paper is to study the interplay of physicians' professional identity, their organizational identity, and the role of professional autonomy in these processes of social identification. METHODS: We test hypotheses generated according to social identity theory using a survey of physicians working in public hospitals in Italy in 2013. RESULTS: Higher degrees of organizational and economic professional autonomy are correlated with higher organizational identification. Identification with the profession is positively correlated with identification with the organization. CONCLUSIONS: Although the generalizability of our results is limited, this study suggests that organizations should support the organizational and economic autonomy of their physicians to project an organizational identity that preserves the continuity of a doctor's self-concept and that is evaluated as positive by doctors. As a result, organizations will be able to foster organizational identification, which is potentially capable of inducing pro-social organizational behavior.

Ošetrovateľstvo ako povolanie má systematickú teóriu, autonómiu, prestíž, etický kódex, profesionálnu kultúru a má byť hlavným zdrojom príjmov pre tých, čo ho praktizujú. Pojem autonómia v povolaní sestra vnímame mnohodimenzionálne. Za najvýznamnejší obsah pojmu autonómia sestry považujeme jej spôsobilosť na samourčovanie slobodnej vôle, rozhodovanie o sebe samej. Nemenej dôležitý aspekt autonómnosti v ošetrovateľstve je schopnosť sestry rozhodovať o iných, predovšetkým v situáciách ohrozujúcich život jednotlivca, ako aj u klientov/pacientov s deficitom sebaopatery. Pripravované zmeny rezortu zdravotníctva v oblasti kompetencií sestry na Slovensku si budú vyžadovať autonómne sestry, sestry zodpovedné za vlastné ošetrovateľské rozhodnutia a výkony, zodpovedné za profesionálnu kvalitu ošetrovateľskej starostlivosti v etickej, aj právnej rovine.
Nursing as a profession has systematic theory, autonomy, prestige, ethical code, professional culture, and should be the main source of income for those who practice it. We perceive the notion of autonomy in the profession of a nurse as multidimensional. The most important content of the concept of nurse autonomy is her ability to self-pursue free will, self-determination. An important aspect of autonomy in nursing is the nurse's ability to make decisions about others, particularly in situations that endanger the life of the individual, as well as in clients / patients with self-care deficiency. Prepared changes to the health sector in the area of nursing competence in Slovakia will require autonomous nurses, nurses responsible for nursing decisions and caretakers, responsible for the professional quality of nursing care in both ethical and legal terms.
