Aim: The study aimed to describe the various ways in which Italian midwives reconcile their profession's philosophical views about labor with the use of epidural analgesia in clinical practice. Design: Interpretive description. Methods: A purposeful sample of 41 midwives and 12 midwifery students participated in the study. Information about their perceptions and experiences of the use of epidural analgesia and how this practice aligns with their professional values were explored through in-depth interviews (n = 10 focus groups; n = 1 one-to-one interview). The constant comparative method was used to analyze the data. Results: With respect to aligning midwifery care with the administration of epidural analgesia, two midwifery positions were identified: the midwife that acted "in harmony" and the "disoriented midwife", defined by three dimensions: 1) the midwife's conception of her professional role; 2) the woman's attitude towards labor and; 3) the midwife's relationship and comfort with other professionals in the labor room. Conclusion: The results of this research highlight how difficult it can be for a midwife to reconcile a specific philosophical view of labor with the use of epidural analgesia. These findings can provide useful insights to help midwives in the challenging task of combining epidural analgesia with their philosophical view of labor to offer a better birth experience to women.

Úvod: Porodní asistentka může poskytovat laktační poradenství v ambulantním, nemocničním i komunitním prostředí, což spadá do jejích odborných kompetencí i kompetencí v oblasti edukace v porodní asistenci. Podpora kojení je nesmírně důležitá jak z hlediska vytváření pouta mezi matkou a novorozencem v rámci utváření nové role matky, tak i ze zdravotního hlediska pro matku i dítě. Cíl: Posouzení efektivity laktačního poradenství v komunitním prostředí. Metodika: Kvantitativní dotazníkové šetření. Nástroj byl sestaven pomocí kombinace standardizované (BSES-SF – The Breastfeeding Self-Efficacy Scale-Short Form) a nestandardizované části dotazníku. Kritérium pro zapojení respondentek do dotazníkového šetření bylo: primipary s využitím služby komunitní porodní asistentky v komparaci s respondentkami bez využití této služby (n = 120). Dotazníkové šetření bylo provedeno od října do prosince roku 2021. Data byla vyhodnocena deskriptivní statistickou analýzou. Výsledky: Výsledné sebehodnocení ve standardizovaném dotazníku BSES-SF ukázalo, že průměrné bodové hodnocení skupiny respondentek s využitím služby komunitní porodní asistentky bylo 54,3 bodů. V průměru sebehodnocení v této skupině žen odpovídá vyšší sebejistotě při kojení. U žen, které nevyužily službu komunitní porodní asistentky, bylo průměrné bodové hodnocení 52,9 bodů. Rozdíl průměrného počtu získaných bodů v obou skupinách potvrdil, že využití komunitní porodní asistentky je efektivnější o 1,4 bodu. Závěr: Výsledky práce poukázaly, že podpora komunitní porodní asistentky v oblasti kojení je nesmírně důležitá. Proto by bylo vhodné podpořit komunitní a domácí péči porodních asistentek jak v časném, tak v pozdním šestinedělí. Vhodné intervence mohou pomoci odstranit překážky v oblasti kojení a eliminovat nedostatky při uspokojování potřeb kojících žen.
Introduction: A midwife can provide lactation consultancy in outpatient, hospital and community environment, which is included in her professional competences as well as competences of education in midwifery. Breastfeeding support is extremely important, both from the point of view of creating a bond between mother and newborn as part of the formation of the mother's new role, and also from a health point of view for both mother and child. Aim: Assessment of the effectiveness of lactation consultancy in a community setting. Methodology: Quantitative questionnaire survey. The tool was compiled using a combination of standardized (BSES-SF – The Breastfeeding Self-Efficacy Scale-Short Form) and non-standardized parts of the questionnaire. The criterion for the inclusion of respondents in the questionnaire survey was: primipara women using the service of a community midwife in comparison with respondents without the use of this service (N = 120). The questionnaire survey was carried out from October to December 2021. The data were evaluated by descriptive statistical analysis. Results: The results of self-assessment in the standardized BSES-SF questionnaire showed that the average score of the group of female respondents using the community midwife service was 54.3 points. On average, self-assessment in this group of women corresponds to higher self-confidence in breastfeeding. For women who did not use the service of a community midwife, the average score was 52.9 points. The difference in the average number of points obtained in both groups confirmed that the use of a community midwife is more effective by 1.4 points. Conclusion: The results showed that the support of the community midwife in the area of breastfeeding is extremely important. Therefore, it is appropriate to support community and home care of midwives both in early and late postpartum. Appropriate interventions can help remove barriers to breastfeeding and eliminate gaps in meeting the needs of breastfeeding women.
Východiska: Příspěvek se zabývá pohledem sester, které získaly pracovní zkušenost v cizině, na ošetřovatelskou péči v zahraničí a následně v České republice. Příspěvek popisuje základní pohledy sester, jak vnímají poskytování a způsoby ošetřovatelské péče v cizině a v České republice.Cíl: Hlavní cíl je popsat pohled sester, které pracovaly v cizině. Mezi dílčí cíle výzkumného šetření patří dosažené vzdělání a důvod, proč sestry odjely do ciziny. Náplň práce sestry a její hodnocení, největší rozdíly v práci sestry u nás a v dané zemi. A co bylo nejtěžší po návratu do České republiky a nynější pohled na zdravotnictví v cizině a v tuzemsku. Metody: Metodika využívá interpretační, narativní metody. V rámci výzkumného šetření jsou použity polostrukturované rozhovory a je vytvořen scénář kvalitativního výzkumu. Výsledky: Výsledky byly vyhodnoceny kvalitativním kódováním, analýzou dat a interpretací výsledků.Diskuze: Ve výzkumném šetření se setkáváme s osobnostmi sester, které mají zkušenosti z odlišného kulturního prostředí. Z výzkumného šetření vyplývá, že sestry jsou ovlivněny zkušeností v multikulturním prostředí, vnímají přínosy této pracovní zkušenosti a také problémy související s pracovní zátěží. Nejdůležitější se jeví seznámení se s novými přístupy, které vedly k otevřenosti a možnosti pracovat v multikulturním prostředí, s ohledem na vytvoření si kulturních kompetencí a přínos sester s kulturními kompetencemi v praxi. Závěry: Sestry pracující v kulturně rozmanitém prostředí mají cenné znalosti a zkušenosti v oblasti mezilidské kulturní komunikace. Tyto zkušenosti a znalosti je důležité předávat v rámci ošetřovatelské praxe.
Background: The paper deals with the view of nurses who have gained work experience abroad, nursing care abroad and subsequently in the Czech Republic. The paper describes the basic views of nurses, how they perceive the provision and methods of nursing care abroad and in the Czech Republic.Aim: The main aim is to describe the view of nurses who worked abroad. One of the partial goals of the research survey is the achieved education and the reason why the nurses went abroad. The content of the nurse's work and its evaluation, the biggest differences in the nurse's work in our country and in the given country. And what was the hardest thing after returning to the Czech Republic and the current view of health care in homeland and abroad.Methods: The methodology uses interpretive, narrative methods. Semi-structured interviews are used in the research survey and a qualitative research scenario is created. Results: The results were evaluated by qualitative coding, data analysis and interpretation of the results.Discussion: In the research survey we meet personalities of nurses who have experience from different cultural backgrounds. The research shows that nurses are affected by experience in a multicultural environment, they perceive the benefits of this work experience as well as problems related to workload. The most important thing seems to be to get acquainted with new approaches that have led to openness and the opportunity to work in a multicultural environment, with regard to the creation of cultural competencies and the contribution of nurses with cultural competencies in practice. Conclusions: Nurses working in a culturally diverse environment have valuable knowledge and experience in the field of interpersonal cultural communication. This experience and knowledge are important to pass on in nursing practice

Východisko: Príspevok sa zaoberá analýzou, ako ro- dičkám pomohla pôrodná asistentka informáciami a odporúčaniami v rozhodovaní sa o odchode do pô- rodnice. Výsledky štúdií ukazujú, že ženy prijaté v la- tentnej fáze pôrodu, oproti ženám prijatým v aktívnej fáze pôrodu, podstúpili cisársky rez častejšie. Súbor a metódy: Sledovaný súbor tvorilo 305 respon- dentiek, ktoré rodili v rokoch 2015 – 2020 v Slovenskej republike v pôrodnici. Anonymným dotazníkom sme zisťovali ich skúsenosti v súvislosti s latentnou fázou pôrodu. Vyselektovali sme skupinu 155 respondentiek. Výsledky: Zistili sme že, pôrodné asistentky poskytli rodičkám spolu 430 odporúčaní, t.j priemerne 2,77 na jednu ženu. Zväčša išlo o odporúčanie: využitie teplej vody v horúcej sprche (97), pohyb a pohyb na fit lop- te (96), klyzma (78), odpočinok (74), sladké nápoje a drobné jedlo na získanie energie (20). 49,51 % res- pondentiek uviedlo, že pôrodná asistentka v ambu- lancii gynekológa primárnej zdravotnej starostlivosti sa nezaujímala. 63 z týchto žien boli prvorodičky. Záver: Pôrodná asistentka pracujúca v ambulancii primárnej zdravotnej starostlivosti by si mala vytvoriť priestor na edukáciu tehotnej ženy a poskytnúť jej správne informácie o začiatku a priebehu pôrodu, ale- bo jej odporučiť absolvovať psychofyzickú prípravu na pôrod.
Background: The article deals with the decision of a woman-mother and her departure to the maternity hospital after the beginning of childbirth. The results of the studies show that women admitted in the latent phase, compared to women admitted in the active pha- se, underwent cesarean section more often. File and methods: The sample consisted of 305 res- pondents who gave birth in the maternity hospital in the Slovak Republic in 2015-2020. We used an ano- nymous questionnaire to find out their experience with the help of midwives in connection with the latent phase of childbirth. We selected a group of 155 res- pondents. Results: We found that midwives recommended a total of 430 recommendations to mothers, i.e., an average of 2.77 per woman. These were mostly recommenda- tions: use of hot water in a hot shower (97), move- ment and movement on a fit ball (96), enema (78), rest (74), sweet drinks and small food to gain energy (20). 49.51% of respondents stated that the midwife in the outpatient clinic of a gynecologist of primary health care was not interested. 63 of these women were first-borns. Conclusion: A midwife working in a primary care clinic should have a place to educate a pregnant wo- man and provide her with the right information abo- ut the beginning and course of childbirth, or recom- mend that she undergo psychophysical preparation for childbirth.
Článok sa zaoberá problematikou imidžu a profesionálnej prestíže pôrodných asistentiek na vy- braných pracoviskách na Slovensku. Súbor a metódy: Prieskum sme realizovali pomocou dotazníka vlastnej konštrukcie. Súbor tvorili pôrodné asistentky (n = 83) zo Slovenska. Výsledky: Zistili sme, že väčšina pôrodných asistentiek hodnotí imidž svojho povolania ako priemerný, 55,4 % respondentiek hodnotí prestíž povolania ako nízku, uvádzajú aj nízku mieru uznania zo strany zdravotníc- kych profesionálov aj verejnosti. K silným stránkam imidžu povolania patria zaujímavá práca, pregraduál- ne a ďalšie vzdelávanie, využívanie vedomostí a zruč- ností zo štúdia v praxi a dobrá atmosféra na praco- visku. Záver: Medzi slabé stránky, ktoré imidž profesie zni- žujú, priradili pôrodné asistentky pracovné priestory, nedostatočné personálne obsadenie a z toho vyplýva- júce preťaženie v práci a nízke finančné ohodnotenie. Introduction: The article deals with the issue of ima- ge and professional prestige of midwives at selected workplaces in Slovakia. File and methods: The survey was conducted by qu- estionnaire of our own design. Our set of respondents consisted of midwives (n=83) from Slovakia. Results: We found that most midwives rate the image of their profession as average, 55.4% of respondents rate the prestige of the profession as low, they also report a low level of recognition by health professio- nals and the public. The strengths of the profession image include interesting work, undergraduate and further education, the use of knowledge and skills from study in practice and a good atmosphere at the workplace. Conclusion: Among the weaknesses that reduce the image of the profession, midwives identified workspa- ces, insufficient staffing and the resulting work over- load and low financial evaluation.
