OBJECTIVES: The objective of this study was to compare evidence requirements for health technology assessment of pharmaceuticals by national agencies across Europe responsible for reimbursement decisions focusing specifically on relative effectiveness assessment. METHODS: Evidence requirements from thirty-three European countries were requested and twenty-nine national agencies provided documents to review. Data were extracted from national documents (manufacturer's submission templates and associated guidance) into a purpose-made framework with categories covering information about the health condition, the technology, clinical effectiveness and safety. RESULTS: The level of detail in the required evidence varies considerably across countries. Some countries include specific questions while others request information under general headings. Some countries include all information in a single document, which may or may not include guidance on how to complete the template. Others have specific guidance documents or methods and process manuals that help with the completion of the submission templates. Despite differences in quantity and detail, the content of the evidence requirements is broadly similar. All countries ask for information on the health technology, target disease, and clinical effectiveness and safety. However, one country only requests clinical effectiveness information as part of cost-effectiveness analyses. We found twenty-six evidence requirements for which generic answers may apply across borders and nineteen in which countries requested nationally specific information. CONCLUSIONS: This work suggests that it would be possible to put together a minimum set of evidence requirements for HTA to support reimbursement decisions across Europe which could facilitate collaboration between jurisdictions.

Úvod: V súčasnosti neexistuje v SR systém evidencie, resp. registrácie detí so zrakovým postihnutím (ZP) ani údaje o prevalencii ZP v tejto populácii. Cieľom našej práce bolo zistiť súčasný výskyt ZP, slabozrakosti a slepoty, u detí v SR a navrhnúť zdravotnú starostlivosť poskytovanú oftalmológmi, ktorej súčasťou by bola aj evidencia, resp. registrácia detí so ZP. Materiál a metodika: Na základe štúdia literatúry sa získali údaje výskytu ZP u detí v SR a vypracoval sa tzv. Evidenčný list dieťaťa so ZP. Tento list, za predpokladu jeho adekvátneho vypĺňania oftalmológmi, by sa stal podkladom systému evidencie detí so ZP. Na základe zisteného výskytu ZP v SR a osobných skúsenností autorov je súčasťou práce aj návrh systému zdravotnej starostlivosti o túto populáciu pacientov. Výsledky: Na základe prevalencie ZP u detí vo veku nižšom ako 16 rokov v industrializovaných krajinách (10–22/10 000) sme dospeli k predpokladu, že po prepočte na slovenskú populáciu je v SR 1 500 až 3 200 zrakovo postihnutých detí vo veku do 19 rokov. V šk. r. 2009–2010 v SR podľa Ústavu informácií a prognóz školstva SR je do vyučovacieho procesu zaradených 752 žiakov so ZP v tomto veku. Navrhovaný systém zdravotnej starostlivosti o deti so ZP predpokladá sústredenie starostlivosti, v záujme kvality a komplexnej starostlivosti, do 3 centier poskytujúcich kompletnú starostlivosť a zároveň aj zavedenie systému evidencie detských pacientov so ZP a to vypĺňaním Evidenčných listov dieťaťa so ZP oftalmológmi a ich následným centrálnym spracovaním. Záver: Počet evidovaných žiakov so ZP v SR je prekvapujúco nízky. Zistenie prevalencie ZP u detí v SR by umožnilo zavedenie systému evidencie detí so ZP oftalmológmi. Systém starostlivosti o pacientov so ZP predpokladá rozdelenie starostlivosti na primárnu, sekundárnu a terciárnu (vrátane rehabilitácie zraku optickými pomôckami). Adekvátne ohodnotenie výkonov a ich uhrádzanie zdravotnou poisťovňou je predpokladom motivácie oftalmológov poskytovať túto náročnú zdravotnú starostlivosť.
Introduction: There is currently no system of registration for visually impaired children in Slovakia and the current prevalence of visual impairment (VI), low vision and blindness is unknown for this population. We propose a template for a process of registration of visually impaired children in Slovakia as well as a system for the Low Vision Health Care for this population. Material and methods: Based on a literature search, we report our data of the estimated prevalence of VI in children in Slovakia and the number of registered students with VI. We have created a registration form and suggested a template of registration for VI children as well as the Health Care System for this population. Results: In industrialized countries, the prevalence of VI, including blindness is 10 - 22/10,000 in children aged less than 16 years. Extrapolating these figures to the Slovak population, we estimate that there will be between 1500 to 3200 VI children under the age of 19 years. Only 752 students with VI of this age were recorded in Slovakia in 2009/2010. We suggest that three Low Vision Centres for VI children should be adequate to cater for the VI population, each of which should provide all levels of care and that ophthalmologists should register patients with VI by filling the proposed registration form. Conclusion: The number of VI children in Slovakia appears to be very low. The only way of accurately assessing the prevalence is to introduce a VI registration system into the country, to be carried out by the ophthalmologists. We suggest that the Low vision service provided by the health authority needs to be divided into primary, secondary and tertiary care (including visual rehabilitation by low vision aids). Only if the Health Insurance will adequately remunerate the Ophthalmologists for the individual procedures will they be motivated enough to provide this level of health care to VI patients.

Úvod: Optické pomôcky (OP) sú najekonomickejšou a najjednoduchšou pomôckou kompenzácie zrakového postihnutia (ZP). Cieľom práce bolo zistiť možnosti kompenzácie ZP prostredníctvom OP u detí a mládeže. Materiál a metodika: Retrospektívne sme analyzovali údaje zo zdravotných záznamov 113 pacientov (pac.) so ZP (binokulárna zraková ostrosť (ZO) 6/18 a horšia alebo prítomné výrazné poruchy zorného poľa), ktorí boli vyšetrení na Ambulancii pre slabozrakých pri Očnej klinike NsP F. D. Roosevelta v Banskej Bystrici od januára 2000 po december 2005. Súbor tvorilo 61 chlapcov a 52 dievčat. Priemerný vek bol 9,4 r. (medián 9 r., v rozmedzí od < 1 r. do < 19 r.). Výsledky: ZO bolo možné vyšetriť formálne u 109 pac. zo 113. Slepota (ZO horšia ako 3/60) sa vyskytla u 31 pac. (28,4 %) a slabozrakosť (ZO horšia ako 6/18 alebo rovná 3/60) u 67 (61,5 %). Zo zvyšných 11 pacientov mali 4 výrazné zúženie zorného poľa, a tak sa slabozrakosť vyskytla u 71 (65 %). Ostatní 7 pacienti pre mentálne postihnutie alebo nízky vek neboli schopní formálneho vyšetrenia zorného poľa. Z hľadiska časového faktora pôsobiaceho činiteľa zapríčiňujúceho ZP, slepotu zapríčinili faktory prenatálne u 45 %, perinatálne u 52 % pac. a postnatálne iba u 3 %. Slabozrakosť bola zapríčinená až v 74,6 % prípadoch faktormi prenatálnymi. Príčiny slepoty podľa postihnutej anatomickej časti boli nasledovné: afekcie zrakového nervu 12x (39 %), postihnutie zrakového centra mozgu 8x (26 %), a choroby sietnice 6x (20 %). Slabozrakosť bola zapríčinená chorobami sietnice 23x (34,3 %), šošovky 11x (16,5 %) a postihnutím zrakových centier 10x (14,9 %). Celkovo u uvedeného súboru 113 pac. sa predpísalo u 74 pacientov (65 %) 171 optických pomôcok: 76 (44,4 %) lúp, 21 (12,3 %) hyperokulárov alebo hyperkorekcií, 65 (38 %) teleskopov a 9 (5,5 %) príložných lúp k teleskopom. V priemere bolo predpísaných 1,5 pomôcok na pacienta. U všetkých pacientov, ktorým sa predpísala OP, došlo k subjektívnemu i objektívnemu zlepšeniu zrakových funkcií, resp. funkčného zraku. Záver: Naše výsledky potvrdili, že slepota sa vyskytla takmer u tretiny našich pac., čo sa zhoduje so svetovo odhadovaným výskytom ZP. OP kompenzujú aspoň čiastočne následky zrakového postihnutia. Nenašli sme žiadnu súvislosť medzi ZO, diagnózou, vekom, a veľkosťou zväčšenia OP. Pri ich predpise sa musí prihliadať na individuálne potreby dieťaťa zohľadňujúc predovšetkým vek a náročnosť zrakových úloh, ktoré by mali a chceli vykonávať.
Introduction: Low visual aids (LVAs) are the simplest and the most economic method of reducing the consequences of visual impairment (VI).We evaluate prescribing of LVAs in order to improve the quality of life of visually impaired children. Material and methods:We retrospectively reviewed the notes of 113 patients (pts.) with VI (visual acuity (VA) 6/18 or worse or with visual field defects) who attended the Low Vision Clinic, F.D. Roosevelt Hospital in Banska Bystrica, between Jan 2000 – Dec 2005. There were 61 boys and 52 girls with ages ranging from < 1 year to < 19 years (mean age 9.4 years, median 9 years). Results: Of the 113 notes reviewed, only 109 of patients were cooperative in having their VA formally tested. Blindness (VA worse than 3/60) occurred in 31 pts. (28.4 %) and Low vision (VA worse than 6/18 or equal to 3/60) in 67 (61.5 %). Of the remaining 11 patients, 4 had gross field defects (and good VA), (indicating that the total number with Low vision was 71 (65 %). The other 7 patients had either developmental delay or were too young to perform a formal visual field test. Blindness was caused by perinatal factors in 52 % of cases, prenatal factors in 45 % and postnatal factors in 3 %. Low vision was caused in 74.6 % of cases by prenatal factors. The causes of blindness according to the affected anatomical site were: optic nerve disease in 12 (39 %), cerebral visual impairment (CVI) in 8 (26 %), and retinal disease in 6 (20 %). Low vision was caused by retinal disease in 23 (34.3 %), lens pathology in 11 (16.5 %) and CVI in 10 (14.9 %). 171 LVAs were prescribed to 74 patients (65 %): 76 (44.4 %) were magnifiers, 21 (12.3 %) were hyperoculars or high adds, 65 (38 %) were telescopes and 9 (5.5 %) telescope extensions. An average of 1.5 devices was prescribed for each patient. Every patient who was prescribed an LVA had subjective and objective improvement of their visual function and functional vision. Conclusion: Blindness occurred in almost one third of our patients which agrees with the worldwide accepted estimate.The LVA are improving the quality of life of visually impaired children.The prescribing of LVA depends on the individual: it is not possible to anticipate the optical power of the LVA that a patient will need simply from looking at their diagnosis and measuring their VA because there are so many different requirements depending on their age and visual demands.
