Úvod: Prevalence chronické rinosinusitidy s polypy (CRSwNP) je v populaci odhadována na 4 %. Nevede-li adekvátní konzervativní léčba ke zlepšení kvality života, jsou nemocní indikováni k chirurgické terapii, kterou představuje funkční endonazální chirurgie. Rekurence polypů po chirurgické léčbě je některými studiemi udávána až v 60 %. Z dosud dostupných dat zatím nevyplývá, jaký je optimální rozsah chirurgického výkonu, zejména v případě revizních operací. Cílem prezentované studie bylo získat informace, zda radikálnější revizní operace zahrnující frontální sinotomii Draf 3 (D3) může představovat přínos pro nemocné s těžkým průběhem onemocnění. Materiál a metody: Čtyřicet dva pacientů s recidivující polypózou bylo zahrnuto do retrospektivní studie případů a kontrol. U poloviny nemocných byl proveden radikálnější chirurgický výkon zahrnující D3, zatímco u kontrolní skupiny byla provedena standardní operace. U obou skupin byla hodnocena kvalita života pomocí české verze dotazníku SNOT-22, výskyt recidivy polypů a nutnost revize do dvou let od operace. Výsledky: Prezentovaná studie zaznamenala nižší pooperační hodnoty dotazníku SNOT-22 u nemocných po výkonu, jehož součástí byla frontální sinotomie D3, nicméně rozdíl nebyl statisticky významný ve srovnání se skupinou pacientů po tradiční FES. Byla však prokázána signifikantně nižší nutnost revizní operace u nemocných s radikálnějším výkonem. Závěr: Získaná data naznačují, že radikálnější chirurgická léčba u podskupiny nemocných s těžším průběhem recidivující polypózy, může být pro tyto pacienty přínosem.
Introduction: The prevalence of chronic rhinosinusitis with nasal polyps (CRSwNP) in the population is estimated at 4%. If adequate conservative treatment fails to improve the quality of life, patients are indicated for surgical therapy, which is represented by a functional endonasal surgery. The recurrence of polyps after surgery is reported by up to 60% in some studies. At the moment, available data does not indicate the optimal extent of surgery, especially in the case of revision procedures. The aim of the presented study was to find out whether a more radical revision surgery, including Draf 3-type (D3) frontal sinotomy, could be beneficial for patients with recurrent disease. Material and methods: Forty-two patients with recurrent polyposis were included in a retrospective case-control study. Half of the patients had undergone a more radical procedure including D3, while the control group had a standard operation. In both groups, the quality of life was assessed using the Czech version of the SNOT-22 questionnaire, the incidence of polyp recurrence and the need for revision within 2 years after the operation. Results: The presented study recorded lower postoperative values of postoperative SNOT-22 questionnaire in patients with D3 frontal sinotomy, but the difference was not statistically significant compared to the group of patients after traditional FES. A significantly lower need for revision surgery was proved in patients with more radical surgery. Conclusion: These data suggest that more radical surgical procedure in a subgroup of patients with severe recurrent polyps may be beneficial for these patients.

Adenoidně cystický karcinom (ACC) je jednou z nejčastějších malignit slinných žláz. Vzácně dochází v ACC k high-grade transformaci neboli dediferenciaci, která je spojená se špatnou prognózou oproti poměrně dlouhému přežití nemocných s konvenčním ACC. Konvenční ACC je charakterizován typickou histomorfologií s glandulární úpravou a ostře ohraničenými luminy, nádorové buňky mají sporou cytoplazmu a angulovaná hyperchromní jádra. High-grade transformovaný ACC pak tyto znaky ztrácí. V této práci popisujeme případ 46leté ženy, která přišla k operačnímu řešení lokálně pokročilého nádoru příušní slinné žlázy s krční lymfadenopatií. Jedna ze suspektních uzlin byla zaslána k peroperačnímu vyšetření ve zmraženém řezu, kdy byly mikroskopicky zastiženy velké nekohezivní buňky s vezikulárními jádry a prominentními jadérky při zachovalé architektonice uzliny. Tento nález vedl k suspekci na lymfom a ukončení operace v rozsahu superficiální parotidektomie a selektivní krční disekce skupin II-IV. Následné histopatologické vyšetření fixované uzliny prokázalo karcinomovou povahu atypických buněk s pozitivitou p63. V resekátu slinné žlázy pak byl identifikován ACC s high-grade komponentou, která vykazovala obdobný vzhled buněk jako popsaná uzlinová metastáza. 17 z 20 uzlin z krční disekce obsahovalo metastázy high-grade ACC. V samotném tumoru parotis byla pozoruhodná silná exprese CD117 v high-grade komponentě, přičemž konvenční ACC byl zcela negativní. High-grade ACC je v příušní žláze podle dostupných informací velmi vzácný, dosud bylo popsáno jen 10 případů.
Adenoid cystic carcinoma (ACC) is one of the most common salivary gland malignancies. In rare cases, ACC undergoes high-grade transformation, which is associated with poor prognosis, in contrast to relatively long survival in the conventional ACC. Conventional ACC is characterized by typical histopathology showing glandular arrangement with sharply demarcated lumina, the tumor cells have sparse cytoplasm and angulated hyperchromatic nuclei. ACCs undergoing high-grade transformation lack these morphological features. In this paper we present a case of 46 years old female patient presenting with locally advanced tumor of the parotid gland and neck lymphadenopathy, coming for surgery. A suspect lymph node was sent to freeze section histology. Large non-cohesive cells with vesicular nuclei and prominent nucleoli along with well persevered lymph node architecture were seen in the frozen slide. This finding lead to suspicion of a lymphoma, the surgery finished in the extent of superficial parotidectomy and selective neck dissection of regions II-IV. Subsequent histopathological examination of formalin-fixed lymph node proofed epithelial nature of the atypical cells by p63 positivity. In the parotid gland resection specimen, an ACC with high-grade component was indentified. The high-grade ACC shared cell morphology with the lymph node metastasis. 17 from 20 lymph neck nodes contained metastases of high-grade ACC. Interestingly, there was strong CD117 expression in the high-grade ACC, whereas the conventional part was fully negative. To the best of our knowledge, the high-grade ACC of the parotid gland was reported only in 10 cases in the medical literature.

Úvod: Sinonazální invertovaný papilom je benigní a relativně vzácný tumor postihující nosní dutinu a paranazální dutiny. Charakteristické biologické vlastnosti tohoto nádoru jsou destruktivní růst, tendence k recidivám a synchronní či metachronní koincidence s dlaždicobuněčným karcinomem. V současnosti je základní léčebnou modalitou invertovaného papilomu chirurgie, zpravidla endoskopická. Tato probíhá v několika krocích, z nichž zcela zásadním je identifikace inzerce tumoru. Místo inzerce je možné predikovat již předoperačně pomocí zobrazovacích metod, přičemž nejdůležitějším je nález tzv. fokální hyperostózy. Cílem práce je prospektivně zhodnotit shodu předoperační a peroperační identifikace inzerce tumoru. Materiál a metody: U 27 nemocných s histologicky prokázaným invertovaným papilomem bylo místo inzerce tumoru předoperačně určováno identifikací fokální hyperostózy na CT snímcích. Předpokládaná lokalita byla poté korelována s peroperačním nálezem. Výsledky: Předoperační analýzou CT snímku se u 74 % nemocných podařilo identifikovat fokální hyperostózu jako předpokládanou inzerci IP. Peroperační nález prokázal shodu inzerce tumoru u 95 % nemocných. Závěr: Pokud se podaří předoperačně identifikovat na CT snímcích fokální hyprostózu, odpovídá tato lokalita s velkou pravděpodobností místu inzerce invertovaného papilomu. Předoperační identifikace inzerce tumoru umožňuje plánování rozsahu výkonu, dobrou radikalitu a minimalizaci morbidity operace.
Introduction: Sinonasal inverted papilloma is a benign and relatively rare tumor of the sinonasal tract. The tumor is characterized by destructive growth, frequent recurrences and coincident with squamous cell carcinoma. Currently, the endoscopic surgery is considered to be basic treatment modality for inverted papillomas. The identification of the tumor insertion represents the most important step of the surgical procedure. The insertion can be predicted preoperatively using imaging methods, especially by searching for so-called focal hyperostosis. The aim of the study is prospective evaluation possible correlation between pre and perioperative identification of the tumor insertion. Material and methods: In 27 patients with histologically confirmed inverted papilloma, the insertion was preoperatively determined by identification of focal hyperostosis. The expected location was then correlated with perioperative finding. Results: The preoperative analysis of CT scans revealed hyperostosis in 74 % of patients. The pre- and perioperative correlation of the tumor insertion was 95 %. Conclusion: Once the focal hyperostosis is identified, the location likely corresponds with the tumor insertion. The preoperative identification of the tumor insertion enables planning of the extent of the procedure with good radicality and minimal morbidity.

Head and neck paragangliomas Paragangliomas and pheochromocytomas are rare, mostly benign neuroendocrine tumors, which are embryologically derived from neural crest cells of the autonomic nervous system. Paragangliomas are essentially the extra-adrenal counterparts of pheochromocytomas. As such this family of tumors can be subdivided into head and neck paragangliomas, pheochromocytomas and thoracic and abdominal extra-adrenal paragangliomas. Ten out of fifteen genes that contribute to the development of paragangliomas are more susceptible to the development of head and neck paragangliomas when mutated. Gene expression profiling revealed that pheochromocytomas and paragangliomas can be classified into two main clusters (C1 and C2) based on transcriptomes. These groups were defined according to their mutational status and as such strongly associated with specific tumorigenic pathways. The influence of the main genetic drivers on the somatic molecular phenotype was shown by DNA methylation and miRNA profiling. Certain subunits of succinate dehydrogenase (SDHx), von Hippel-Lindau (VHL) and transmembrane protein 127 (TMEM127) still have the highest impact on development of head and neck paragangliomas. The link between RAS proteins and the formation of pheochromocytoma and paragangliomas is clear due to the effect of receptor tyrosine-protein kinase (RET) and neurofibromatosis type 1 (NF1) in RAS signaling and recent discovery of the role of HRAS. The functions of MYC-associated factor X (MAX) and prolyl hydroxylase 2 (PHD2) mutations in the contribution to the pathogenesis of paragangliomas still remain unclear. Ongoing studies give us insight into the incidence of germline and somatic mutations, thus offering guidelines to early detection. Furthermore, these also show the risk of mistakenly assuming sporadic cases in the absence of definitive family history in head and neck paragangliomas.

Osteoarthrosis (OA) of temporomandibular joint (TMJ) is a progressive degenerative disease, gradually affecting cartilage, synovial membrane and bone structures. OA of TMJ clinically manifests with joint noises, pain and restricted mouth opening. In late stages, it results in severe damage of TMJ structures and development of ankylosis. Osteoarthrosis is a multifactorial disease; the occurrence is associated with TMJ overloading. The cohort included 619 patients [538 women (87%) and 81 men (13%), with average age 40.6 years (age range 8-89 years)] with TMJ disorder, who were examined in the year 2014 in Department of Dental Medicine, First Faculty of Medicine, Charles University and General University Hospital in Prague, Czech Republic. The aim of this retrospective study was to analyse, if the lack of posterior teeth (supporting teeth zones) is the main etiologic factor of osteoarthrosis of TMJ. Diagnosis of OA was established on the clinical signs and the panoramic X-ray signs. Simultaneously other etiologic factors of OA were assessed. The presence of OA changes on X-ray had 171 patients (i.e. 27.6% of the total number of 619). 17.5% from these patients with OA had defect in posterior dentition. Other aetiological factors (stress, skeletal or vertebrogenous disorders) showed higher incidence of OA changes on X-ray. Defect of posterior dentition is not negligible, but it is not the main aetiological factor for osteoarthrosis of TMJ.
