PURPOSE OF THE STUDY The purpose of this study is to remind of the specific features of diagnosis and therapy of dorsal dislocations of the glenohumeral joint to prevent frequent neglect or misdiagnosis during primary treatment. Also, we strived to present our experience and outcomes of posterior dislocation treatment by McLaughlin procedure and its modified version. MATERIAL AND METHODS We retrospectively evaluated 7 patients who had been treated at our department in the period 2015-2019 by McLaughlin procedure or its modification. The group consisted of 5 men and 2 women aged 33-79 years. In five patients, the dislocation occurred during an epileptic seizure, in two patients during an accident. The patients were treated by McLaughlin procedure or the Neer modification of McLaughlin procedure. The deltoideopectoral surgical approach was always used. Based on the preoperative planning, we decided, or intraoperatively changed our decision, as to whether mere transposition of the subscapularis is satisfactory or whether transposition of the lesser tuberosity to the defect, a bone graft and arthroplasty are necessary. Postoperatively, the operated limb was fixed for 6 weeks in neutral position, which was followed by rehabilitation. RESULTS We evaluated subjective and objective outcomes of the surgery, the radiological finding and in 2 patients also a control CT scan. The function of the shoulder was assessed using the Constant shoulder score and the OSIS scoring system. The resulting score: the mean OSIS was 55.7 points (92.8%) and the mean Constant score was 86.2 points (86.2%), which we regard as a very good result. All patients consider the surgery successful and are satisfied with the outcome. No patient reported recurrent dislocation. DISCUSSION When treating the posterior dislocation of the shoulder, vital is the shoulder reducibility and also the presence or the size of reverse Hill-Sachs defect. The transfer of the subscapularis tendon can be performed as an open procedure or arthroscopically. Inveterate irreducible posterior dislocation of the shoulder is quite a rare diagnosis, which is why even at our department the McLauglin procedure is performed in the order of units per year only. CONCLUSIONS The surgical solution of inveterate posterior dislocation of the shoulder is hardly ever uncomplicated. The reduction alone is often insufficient and some other way of shoulder stabilisation is necessary, often times addressing the humeral head defect. In our to date experience, the McLaughlin procedure or the Neer s modification is an elegant and safe method to treat posterior dislocation with a humeral head defect. The functional outcomes are very good and enable the patients to reengage in everyday activities. Thanks to this procedure shoulder joint arthroplasty can be avoided in younger patients. Nonetheless, in defects that are greater than 50% of the head the replacement is necessary. Due to frequently associated rotator cuff injuries, the most often procedure indicated by us is the reverse total shoulder replacement. Úvod: Abutment syndrom ulny, kdy distální část této kosti naráží na kosti karpu, je jednou z příčin bolestí na ulnární straně zápěstí. Příčina může být z vrozené plus varianty ulny či získaná například po zlomeninách distálního radia a nedostatečné korekci jeho délky. Materiál a metoda: Autoři hodnotili výsledky zkracovací osteotomie ulny u 22 pacientů průměrného věku 45,5 roku (16–84 let), kdy se jednalo o 12 žen a 10 mužů. Dominantní končetina byla poškozena u 13 pacientů a nedominantní u 9 pacientů. Pacienti byli zhodnoceni DASH dotazníkem před operací a s odstupem v průměru 82,4 měsíce (48–328 měsíců). Výsledky: Při hodnocení pacientů pomocí DASH skóre bylo předoperačně DASH skóre v průměru 44,6 bodu a po operaci 16,6 bodu. Nejhorší nález po operaci byl u pacienta po poškození předloktí typu Essex-Lopresti, kde po 8 letech od operace se stav výrazně zhoršil (o 27 bodů) oproti stavu před operací. Stav byl komplikován ve 3 případech nezhojením osteotomie a nutností reosteosyntézy a spongioplastiky. Jeden z těchto pakloubů byl komplikován infektem, kde bylo k sanaci lokálně použito nejprve vložení kostního cementu s antibiotikem a po zklidnění provedena spongioplastika. Diskuze: Zkracovací osteotomie ulny má dobré výsledky, pokud jsou dodrženy správné indikace. I když jde o technicky jednoduchý výkon, může být komplikován vznikem pakloubu, který je třeba aktivně řešit.
Introduction: Abutment syndrome of the ulna, where the distal part of the bone hits carpal bone, is one of the possible causes pain on the ulnar side of the wrist. The cause may be congenital plus-variants ulna, or developed after, for example, distal radius fractures and inadequate correction of its length. Material and methods: The authors evaluated the results of ulnar shortening osteotomy in 22 patients, average age 45.5 years (16-84 years); there were 12 women and 10 men. The dominant limb was damaged in 13 patients and non-dominant in 9 patients. Patients were evaluated using the DASH questionnaire preoperatively at an average interval of 82.4 months (48-328 months). Results: When evaluating patients using the DASH score, the preoperative averaged at 44.6 points preoperatively and 16.6 points postoperatively. The worst postoperative result was in the case of surgery to the patient's forearm, after damage of the Essex-Lopresti type, where 8 years after the operation, condition deteriorated significantly (by 27 points), compared to the situation before the operation. The condition was complicated in 3 cases by non-healing after osteotomy and the need for re-osteosynthesis and bone grafts. One of these non-unions was complicated by infections, which required local use of antibiotics delivered via bone cement insertion and a bone graft after soothing. Discussion: Butt ulnar osteotomy has good results if it follows the correct indication. Although it is a technically simple procedure, it can be complicated by non-unions, which need to be actively addressed.

Základem správné diagnostiky bolestí ulnární strany zápěstí je dobrá znalost anatomie zápěstí, kinetiky a všech patologických stavů, které mohou tyto bolesti způsobovat. Pro správnou diagnostiku si ve velké většině případů vystačíme s anamnézou, klinickým vyšetřením, rtg a artroskopií (AS) zápěstí. Přesto je rozpoznání problému obtížné, klinické obtíže jsou obdobné a často se překrývají. V tomto článku se snažíme poskytnout stručný přehled možných nejčastějších patologií s mechanizmem úrazu, základním diagnostickým postupem a terapií.
The correct diagnosis of the ulnar sided wrist pain requires profound knowledge of the wrist anatomy, kinetics of the wrist and all pathological conditions that may cause such pain. In most cases the case history, clinical examination, x-ray examination and wrist arthroscopy are sufficient to establish the correct diagnosis. Yet it is difficult to identify the problem as clinical symptoms are similar and often overlap. This article provides a brief overview of the most frequent pathologies and mechanisms of injury, basic diagnostic procedure and treatment.
