Úvod. Nealkoholové jaterní tukové onemocnění (NAFLD) postihuje v současné době více než 25 % dospělé populace a zřejmě úzce souvisí s pandemií obezity a diabetu. Ačkoli mechanizmy vedoucí k akumulaci tuku v játrech jsou poměrně intenzivně studovány, dosud chybějí studie zabývající se efekty krátkodobé nutriční nebo jiné intervence na změny obsahu tuku v játrech, které lze hodnotit metodami magnetické rezonance. Je např. známo, že krátkodobé hladovění vede k akumulaci tuku v játrech u zdravých osob, není však známo jak na hladovění odpovídá jaterní tuk u obézních. Jaterní tuk je do cirkulace transportován lipoproteiny velmi nízké hustoty (VLDL) a není známo, jak hladovění ovlivní koncentraci VLDL u štíhlých/neobézních a obézních osob. Cíl. Charakterizovat odpověď obsahu tuku v játrech a koncentrace VLDL na krátkodobé hladovění u štíhlých/neobézních a obézních žen. Studovaný soubor a metodika: V rámci studie bylo vyšetřeno celkem 22 premenopauzálních žen – 11 neobézních (NOB): věk 36,6 ± 7,5 roku, BMI 21,4 ± 1,6 kg/m2, a 11 obézních žen (OB): věk 34,5 ± 7,9 roku, BMI 34,5 ± 4,8 kg/m2, u kterých byl stanoven obsah tuku v játrech (HFC) magnetickou rezonanční spektroskopií (1H-MRS) na začátku studie a po 60hodinovém hladovění. Současně byly z plazmy izolovány VLDL ultracentrifugací a stanovena jejich koncentrace. Výsledky: OB-ženy měly zvýšenou koncentraci TG (1,5 ± 0,6 vs 0,8 ± 0,3 mmol/l, p < 0,001) a zvýšený obsah tuku v játrech (7,9 ± 9,1 vs 0,9 ± 0,4 %, p < 0,001). U 5 OB-žen byla zjištěna steatóza (HFC > 5,5 %). Skupiny se nelišily v koncentraci cholesterolu (4,2 ± 0,6 vs 4,0 ± 1,0 mmol/l). U NOB-žen indukovalo 60hodinové hladovění vzestup HFC z 0,9 ± 0,4 na 1,8 ± 0,8 % (p = 0,001) a nárůst koncentrace VLDL-TG z 0,29 ± 0,14 na 0,52 ± 0,26 mmol/l (p = 0,005) a koncentrace VLDL-C z 0,10 ± 0,08 na 0,19 ± 0,11 mmol/l (p = 0,018). U OB-žen nebyl žádných z těchto parametrů krátkodobým hladověním ovlivněn (HFC: 7,9 ± 9,1 vs 7,9 ± 9,3 %, VLDL-TG: 0,83 ± 0,42 vs 0,90 ± 0,47 mmol/l, VLDL-C: 0,26 ± 0,12 vs 0,26 ± 0,13 mmol/l). Závěr. U OB-žen byla zjištěna zvýšená produkce VLDL, která v souladu s dřívějšími pozorováními koreluje s obsahem tuku v játrech. Rozdíly v odpovědi jaterního tuku a koncentrace VLDL mezi neobézními a obézními ženami by mohly souviset např. se zvýšenou inzulinovou rezistencí u OB-žen a jsou předmětem dalšího studia.

BACKGROUND/OBJECTIVES: Branched chain amino acids (BCAA) are among nutrients strongly linked with insulin sensitivity (IS) measures. We investigated the effects of a chronic increase of BCAA intake on IS in two groups of healthy subjects differing in their basal consumption of BCAA, that is, vegans and omnivores. SUBJECTS/METHODS: Eight vegans and eight matched omnivores (five men and three women in each group) received 15 g (women) or 20 g (men) of BCAA daily for 3 months. Anthropometry, blood analyses, glucose clamp, arginine test, subcutaneous abdominal adipose tissue (AT) and skeletal muscle (SM) biopsies (mRNA levels of selected metabolic markers, respiratory chain (RC) activity) were performed at baseline, after the intervention and after a 6 month wash-out period. RESULTS: Compared with omnivores, vegans had higher IS at baseline (GIR, glucose infusion rate: 9.6±2.4 vs 7.1±2.4 mg/kg/min, 95% CI for difference: 0.55 to 5.82) that declined after the intervention and returned to baseline values after the wash-out period (changes in GIR with 95% CI, 3-0 months: -1.64 [-2.5; -0.75] and 9-3 months: 1.65 [0.75; 2.54] mg/kg/min). No such change was observed in omnivores. In omnivores the intervention led to an increased expression of lipogenic genes (DGAT2, FASN, PPARγ, SCD1) in AT. SM RC activity increased in both groups. CONCLUSIONS: Negative impact of increased BCAA intake on IS was only detected in vegans, that is, subjects with low basal amino acids/BCAA intake, which appear to be unable to induce sufficient compensatory changes within AT and SM on a BCAA challenge.

The consumption of lipids and simple sugars induces an inflammatory response whose exact molecular trigger remains elusive. The aims of the present study were to investigate (1) whether inflammation induced by a single high-energy, high-fat meal (HFM) is associated with endoplasmic reticulum stress (ERS) in peripheral blood mononuclear cells (PBMC) and (2) whether these inflammatory and ERS responses could be prevented by the chemical chaperone ursodeoxycholic acid (UDCA). A total of ten healthy lean men were recruited to a randomised, blind, cross-over trial. Subjects were given two doses of placebo (lactose) or UDCA before the consumption of a HFM (6151 kJ; 47·4 % lipids). Blood was collected at baseline and 4 h after the HFM challenge. Cell populations and their activation were analysed using flow cytometry, and plasma levels of inflammatory cytokines were assessed by ELISA and Luminex technology. Gene expression levels of inflammatory and ERS markers were analysed in CD14⁺ and CD14⁻ PBMC using quantitative RT-PCR. The HFM induced an increase in the mRNA expression levels of pro-inflammatory cytokines (IL-1β, 2·1-fold; IL-8, 2·4-fold; TNF-α, 1·4-fold; monocyte chemoattractant protein 1, 2·1-fold) and a decrease in the expression levels of miR181 (0·8-fold) in CD14⁺ monocytes. The HFM challenge did not up-regulate the expression of ERS markers (XBP1, HSPA5, EDEM1, DNAJC3 and ATF4) in either CD14⁺ or CD14⁻ cell populations, except for ATF3 (2·3-fold). The administration of UDCA before the consumption of the HFM did not alter the HFM-induced change in the expression levels of ERS or inflammatory markers. In conclusion, HFM-induced inflammation detectable on the level of gene expression in PBMC was not associated with the concomitant increase in the expression levels of ERS markers and could not be prevented by UDCA.

The aim of this study was to investigate the time-course of the expression of key lipolysis-regulating genes in the subcutaneous adipose tissue (SCAT) during different phases of a 6-month dietary intervention. Fifteen obese women (BMI 34.7+/-1.0 kg.m(-2)) underwent a 6-month dietary intervention consisting of 1 month very low calorie diet (VLCD), followed by 2 months low calorie diet (LCD) and 3 months weight maintenance diet (WM). At each phase of the dietary intervention, a needle microbiopsy of the abdominal SCAT was obtained to evaluate mRNA expression of key lipolysis-regulating genes and a hyperinsulinemic euglycemic clamp (HEC) was performed. Dietary intervention induced a body weight reduction of 9.8 % and an improvement of insulin sensitivity as assessed by a HEC. Compared to pre-diet levels, mRNA levels of the adrenergic beta(2)-receptor in SCAT were higher at the end of VLCD and not different at the end of LCD and WM. In contrast, the expression of the adrenergic alpha(2)-receptor was lower at the end of VLCD and LCD compared to the pre-diet levels and did not differ at WM. Adipose triglyceride lipase and hormone-sensitive lipase levels were lower than the pre-diet levels at the end of LCD only, while phosphodiesterase-3B and the insulin receptor levels did not change throughout the dietary intervention. The results suggest that the regulation pattern of the genes that are involved in the control of lipolysis is different at the respective phases of the dietary intervention and depends on the duration of the diet and the status of energy balance.
