Familiárna chylomikronémia (Familial chylomicronemia syndrome – FCS) je autosómovo recesívne dedičné metabolické ochorenie, ktoré spôsobujú mutácie v génoch, ktoré majú kľúčovú úlohu v lipolýze lipoproteínov bohatých na triglyceridy. Charakteristickým prejavom FCS je prítomnosť chylomikrónov v plazme aj po 12-hodinovom lačnení. Najčastejšou príčinou tohto ochorenia sú mutácie v géne lipoproteinovej lipázy (LPL), ktoré sa zisťujú u viac ako 80 % pacientov. Táto forma FCS sa tiež nazýva deficiencia lipoproteinovej lipázy (LPLD). Populačný výskyt FCS sa udáva 1–2 na 1 milión. Avšak pokrok v diagnostike DNA poukazuje na to, že výskyt môže byť vyšší. Pacientka, ktorej prípad prezentujeme, prekonala 9-krát recidívu akútnej pankreatitdy. Až 2. tehotenstvo skončilo úspešne bez perinatálnych komplikácií a pôrodom zdravého chlapca. Úspech je výsledkom intenzívnej multidisciplinárnej starostlivosti, dôslednému diétnemu plánovaniu a edukácií a správnemu rozhodnutiu začať liečbu plazmaferézou v 3. trimestri gravidity.
Familial chylomicronemia (Familial chylomicronemia syndrome – FCS) is an autosomal recessive inherited metabolic disorder which causes mutations in the genes which have a central role in lipolysis of triglyceride-rich lipoproteíns. A characteristic manifestation of FCS is the presence of chylomicrons in plasma also after 12- hour fasting. The most common cause of this disorder are mutations in the lipoprotein lipase (LPL) gene that are found in more than 80% patients. This form of FCS is also called lipoprotein lipase deficiency (LPLD). Incidence of FCS in the population is reported to be 1–2 per 1 million. Nonetheless the progress in DNA diagnostics suggests that the incidence may be higher. A patient whose case we present has overcome recurrent acute pancreatitis 9 times. It was only her 2nd pregnancy which was free from perinatal complications and ended by birth of a healthy boy. The success is the result of intensive multidisciplinary care, consistent diet planning and education and the right decision to commence the plasmapheresis treatment in the 3rd trimester of pregnancy.

Východisko: Hemolyticko-uremický syndróm (HUS) je typickým predstaviteľom skupiny trombotických mikroangiopatií. Spôsobuje akútne a tiež chronické poškodenie obličiek s dominantným poškodením glomerulov a artérií. Z dlhodobého hľadiska sa u pacientov popisuje rôzny stupeň poškodenia obličkových funkcií. Ciele práce a metodika: Retrospektívne sme hodnotili prognózu vývoja chronickej obličkovej choroby pacientov s atypickým HUS (aHUS) a Shiga toxin-producing Escherichia coli HUS (STEC HUS). V skupine aHUS sme zistené informácie porovnali s informáciami z Global aHUS Registry. V skupine STEC HUS boli parametre obličkových funkcií a systémový tlak krvi hodnotené po 6 mesiacoch, prvom a piatom roku od manifestácie ochorenia. Výsledky: Priebeh a vývoj ochorenia pacientov s aHUS bol rôzny. Získané údaje sú porovnatelné s údajmi z Global aHUS Registry. Skupinu STEC HUS tvorilo 15 detí. Po 6 mesiacoch od manifestácie ochorenia mali 6/15 (40 %) prítomnú proteinúriu, 3/6 (50 %) mali systémovú hypertenziu. Po prvom roku od manifestácie ochorenia mali 5/15 (33 %) prítomnú proteinúriu, 2/5 (40 %) systémovú hypertenziu a 2/5 (40 %) zníženú glomerulovú filtráciu. Po piatich rokoch od manifestácie ochorenia mali 3/11 (27 %) pacientov prítomnú proteinúriu, systémovú hypertenziu a pokles glomerulovej filtrácie.Záver: Naše výsledky v skupinách aHUS a STEC HUS sú porovnatelné s výsledkami publikovaných štúdií. Vzhľadom k výsledkom je potrebné dlhodobé sledovanie pacientov s HUS.
Introduction: Hemolytic-uremic syndrom (HUS) is the typical representative of the thrombotic microangiopathy. It causes acute and chronic kidney damage with dominant glomerular and arterial damage. In the long term renal outcome, patients have varying degrees of renal impairment.Aim and metode: In a retrospective single-center study renal prognosis was analyzed in patients with atypical HUS (aHUS) and Shigatoxin-producing Escherichia coli HUS (STEC HUS) pacients was analyzed. In the aHUS group obtained information was copared with information from Global aHUS Registry. In the STEC HUS group renal function and systemic blood pressure were analyzed after 6 months, after the first and fifth year after disease manifestation. Result: The course and development of disease in aHUS group varied. The obtained data is comparable to the Global aHUS Registry. The STEC HUS group consisted of 15 children. Six months after disease manifestation, 6/15 (40 %) patients had proteinuria, 3/6 (50 %) had systemic hypertension. After the first year of the disease manifestation, 5/15 (33 %) patients had proteinuria, 2/5 (40 %) had systemic hypertension and 2/ 5 (40 %) had reduced glomerular filtration. Five years after the disease manifestation, 3/11 (27 %) patients had proteinuria, systemic hypertension, and reduced glomerular filtration. Conclusion: Our results in the aHUS and STEC HUS groups are comparable to published studies. These findings underline the importance of a consequent long-term follow-up of HUS patients.

INTRODUCTION: Hormone changes during pregnancy lead to increased plasma lipid levels. When there is added disorder of lipid metabolism, this otherwise physiological change can cause extremely high triglyceride levels with potentionally life-threatening complications, such as non-biliary acute pancreatitis. MATERIALS AND METHODS: We present a case report of a 27-year-old pregnant woman with familial hyperchylomicronemia and a history of 7 hypertriglyceridemia-induced acute pancreatitis attacks. Three attacks occured during her first pregnancy with the last one leading to its termination at 33 weeks owing to the death of the fetus. During her second pregnancy, standard treatment was not able to lower the triglyceride levels sufficiently and she suffered another acute pancreatitis attack. Therapeutic plasma exchange was therefore chosen as the treatment method. RESULTS AND CONCLUSION: Plasma exchange was succesful in the secondary prevention of acute pancreatitis attack and she delivered a healthy baby at 36 weeks of gestation. Treatment was very well tolerated by the mother and the fetus and this supports the use of apheresis as a safe and efficient method in tackling gestational hypertriglyceridemia.
