We evaluated the prevalence and epidemiology of extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli isolates in pigs during production cycle on a Czech farm with the history of previous use of ceftiofur. ESBL-producing E. coli isolates were obtained from rectal swabs from pigs of different age groups (suckling piglets, weaned piglets, growers and sows). Collected samples were directly cultivated on MacConkey agar with cefotaxime (2 mg l-1 ), whereas intestinal swabs of slaughtered pigs and surface swabs from pig carcasses were also pre-enriched in buffered peptone water without antimicrobials before the cultivation. Clonal relationship of selected isolates was determined by XbaI pulse-field gel electrophoresis and multi-locus sequence typing. The transferability of plasmids carrying blaCTX-M genes was tested by conjugation experiments. From all examined samples, 141 (43·7%, n = 323) were positive for ESBL-producing E. coli. All ESBL-producing isolates showed resistance to multiple antimicrobials and were positive for blaCTX-M genes. The blaCTX-M-1 was carried by conjugative IncN/ST1 plasmids (c. 40-45 kb) while the blaCTX-M-15 was located on conjugative F plasmids with F:18:A5:B1 formula (c. 165 kb). This study demonstrated the persistence of CTX-M-positive E. coli isolates 2 months after banner of ceftiofur usage and indicated possible risk of transmission of these isolates to humans via the food chain.

In this article the authors discuss the problem faced by physicians when trying to use moisture-retentive dressing in pressure sores (decubitus ulcers). First, they report the results of an in vitro study using a new model of experimental wound (radio-isotopic investigation) that assesses the release of Ringer's solution from interactive dressings continually during fourteen hours. Second, they perform an animal experiment that assesses the incidence of wound infection in defects treated conventionally or using interactive dressings. The defects treated with interactive pads had lower incidence of wound infection, and the process of wound healing was rapid. Finally, the authors discuss their experience in four paraplegic patients with decubitus ulcers where they used moisture-retentive dressing on ulcers and on the surrounding intact skin before surgical procedure to detect the possibility of maceration of healthy skin. Histological evaluation was performed in order to find microscopically changes after moisture healing. The changes of healthy skin were not significant after treatment of moisture-retentive dressings.

Práce se zabývá hojením ran v mokrém prostředí a posuzuje dva odlišné způsoby ošetřování kožních defektů. Standardní postup, při kterém je rána kryta mastným tylem a vlhkou gázou, srovnává s novou metodou léčení za použití obvazu TenderWet. V první etapě výzkumu byly provedeny testy in vitro. Poté následoval experiment na zvířeti včetně mikrobiologické monitorace a planimetrického měření velikosti defektů. Práce rovněž popisuje dva klinické případy hlubokého dermálního popálení. Dosažené výsledky jednoznačně podporují systém mokrého hojení ran ve srovnání s běžným krytím.
We compared two types of moisture-retentive dressings (dressings that are capable of maintaining a moist environment) in wound healing. The conventional method of using an impregnated gauze in combination with a moist wound dressing was compared with TenderWet. First, we report results from an in vitro study and an animal experiment that included assesment of reepithelization and incidence of wound infection. Secondly, we discuss our experience using TenderWet in a clinical setting. Two cases of deep dermal burns are described and documented in detail. The results of our work suggest that a moist wound environment is more effective in facilitating wound healing than conventional methods.
