Z dostupných statistických dat o činnosti jednotlivých středisek LZS za rok 2017 vyplývají značné rozdíly ve způsobu využití této nákladné služby. Tyto rozdíly mohou být způsobeny objektivními lokálními specifiky, jistý je však i vliv mezikrajsky rozdílného přístupu k operačnímu řízení leteckých výjezdových skupin. Cestu k vyšší efektivitě vidíme v racionalizaci praxe jednotlivých řídí-cích operačních středisek, jejíž nutnou podmínku spatřujeme v precizním sledování a pravidelné analýze vhodných parametrů popisujících činnosti LZS.
Based on available statistical data of year 2017 significant differences in the way of HEMS deployment among individual HEMS bases and medical dispatching are evident. Causes of such a dissimilarity lie in objective locoregional specifics on one side and in different way of dispatching practice and management on the other side. Higher system efficiency is conditioned by rationalization of dispatching practice and precise quality management.
We provide a new assessment of the effect of hospital proximity in an emergency situation-road-traffic accidents-exploiting the exogenous variation in the proximity to cities that are legally allowed to have a hospital on the basis of their population size. Our instrumental variable results show that a one-standard-deviation increase in the distance to the nearest hospital (5 km) raises the fatality rate by 13.84% at the sample average. This figure is equal to 0.92 additional deaths per 100 accidents. We show that both ordinary least squares and difference-in-differences estimates, common approaches in the literature, provide a downward-biased measure of the true effect of hospital proximity because they do not fully solve spatial sorting problems. Proximity is more important when the level of road safety is low, when emergency services are less responsive, and when the nearest hospital has relatively low quality standards.

Cílem práce bylo porovnat využití LZZS na jednotlivých základnách v ČR ve vztahu k počtu obyvatel přirozeně spádového území. Výsledky porovnání nevylučují, že ve využívání LZZS existují v České republice markantní rozdíly mezi jednotlivými základnami. Příčina zjištěných rozdílů není známa, mohou být však signálem neracionálního využívání této finančně extrémně náročné služby. Z důvodu prevence pod- a nadužívání LZZS by bylo vhodné stanovit, sledovat a publikovat funkční parametry sledující efektivitu nasazení LZZS.
The aim of the study was to compare the use of air ambulance (HEMS) at various bases in the Czech Republic in relation to population of native catchment area. Comparing of the results suggest the existence of a huge difference between the use of individual helicopter bases. The cause of this difference is unclear, but may be a signal of irrational use (under-, as well as overuse) of this extremely expensive service. It would be appropriate to establish, monitor and publish a set of performance parameters in order to monitor appropriate use of the service.
