Efekt řízené pohybové aktivity v terapii roztroušené sklerózy (RS) byl opakovaně prokázán řadou studií, které také ukazují, že cvičení neovlivňuje závažnost ani četnost atak. Článek shrnuje současný stav poznatků o mechanizmech a možnostech pohybových aktivit ovlivňujících hlavní neurologické symptomy RS (únava, spastická paréza, ataxie, posturální instabilita). Výzkum i praxe shodně ukazují, že pestrost klinického obrazu RS vyžaduje komplexní terapeutickou intervenci. Článek shrnuje mechanizmy účinku a nejnovější doporučení u hlavních forem pohybových aktivit užívaných u pacientů s RS, tj. u aerobního, rezistentního a kombinovaného tréninku. Autoři je doplňují o své klinické zkušenosti včetně náčrtu vlastní podoby skupinového kruhového aerobně-rezistentního tréninku. Ostatní složky komplexní léčby článek záměrně nezohledňuje.
The effect of controlled movement activity in the treatment of multiple sclerosis (MS) has repeatedly been demonstrated by numerous studies that have also shown that exercise does not affect severity or frequency of relapses. This paper summarizes current state of knowledge on movement activities mechanisms and their application in the treatment of major MS-related neurological symptoms (fatigue, spastic paresis, ataxia, postural instability). Both research and clinical practice consistently show that the complexity of the clinical picture of MS requires a comprehensive therapeutic intervention. The paper reviews the mechanisms of action and the latest recommendations on the major forms of physical activities used in patients with MS, i.e. aerobic, resistance and combined training. The authors add their clinical experience, including their own design of a group circuit aerobic-resistance training. The paper does not discuss any other elements of the comprehensive therapeutic approach as this is out of the scope of the paper. Key words: multiple sclerosis – motor activities – training The authors declare they have no potential conflicts of interest concerning drugs, products, or services used in the study. The Editorial Board declares that the manuscript met the ICMJE “uniform requirements” for biomedical papers.
