Kontext: V posledních několika desetiletích prodělala léčba srdečního selhání skok dopředu, zvláště v oblasti přístrojové léčby; výsledkem bylo snížení zátěže srdečním selháním a úmrtnosti na toto onemocnění. Nicméně u srdečního selhání se sníženou ejekční frakcí lze o zahájení přístrojové léčby uvažovat až po selhání maximální farmakoterapie podle doporučených postupů (guidelines). Z rozvíjejících se zemí je k dispozici omezené množství údajů o adherenci k doporučeným postupům pro diagnostiku a léčbu srdečního selhání. Cílem této studie bylo proto zjistit adherenci kardiologů k optimalizaci farmakoterapie podle doporučených postupů ještě před implantací přístrojů pacientům se srdečním selháním a sníženou ejekční frakcí. Pacienti a metody: Do studie byli zařazeni pacienti se srdečním selháním a sníženou ejekční frakcí, přijatí pro implantaci příslušného přístroje (implantabilní kardioverter-defibrilátor) nebo zahájení srdeční resynchronizační léčby; byly shromažďovány údaje pacientů včetně demografických charakteristik, léků a jejich dávek užívaných při vstupu do studie stejně jako typ implantovaného přístroje a indikace k implantaci. Výsledky: Z 31 pacientů zařazených do studie bylo 25,8 % žen. Průměrný věk byl 57,1 ± 11,6 roku. Beta-blokátory užívalo 83,9 % pacientů, inhibitory angiotenzin konvertujícího enzymu / blokátory receptoru AT1 pro angiotenzin II 38,7 % a antagonisty mineralokortikoidních receptorů 67,74 % pacientů. Léčba byla zahájena nebo dávky titrovány směrem vzhůru méně než dva měsíce před implantací příslušného přístroje v 73,68 % primárně indikovaných případů. Třem pacientům (9,67 %) byl přes splnění indikací pro srdeční resynchronizační terapii (blokáda levého Tawarova raménka nebo kompletní blokáda) implantován kardioverter-defibrilátor. U pacientů s primárními indikacemi se rozhodnutí o implantaci opíralo o doporučení guidelines v 9,67 % případů, kdy byla podávána medikamentózní léčba podle doporučených postupů s titrací dávek směrem vzhůru až do dosažení maximálních, na důkazech založených dávek ještě před implantací příslušného přístroje. Závěry: Naše studie prokázala špatnou adherenci k farmakoterapii podle doporučených postupů u pacientů se srdečním selháním a sníženou ejekční frakcí před implantací přístroje. Je třeba vyvinout maximální úsilí pro překonání tohoto nedostatku v praxi a dosáhnout u pacientů lepších výsledků nemluvě o nákladové účinnosti jako dosud přehlíženém ekonomickém faktoru zdravotní politiky. © 2021, ČKS.
Background: Over last few decades, treatment of heart failure witnessed a leap especially regarding device therapy which decreases heart failure burden and mortality, however; in heart failure with reduced ejection fraction introduction of device therapy should be considered after maximizing guideline directed medical therapy. There is limited data from emerging countries regarding adherence to guidelines in heart failure management. Accordingly, this study sought to investigate adherence of cardiologists to optimization of guideline directed medical therapy prior to device implantation in patients with heart failure and reduced ejection fraction. Patients and methods: This study enrolled patients with heart failure and reduced ejection fraction who were admitted for cardiac device implantation (implantable cardioverter defibrillator or cardiac resynchronization therapy), patients' data were collected; including demographic characteristics, baseline medications and doses were recorded as well as type of implanted device and its indication.

Úvod: Čekací listina na transplantaci srdce obsahuje soubor nemocných s pokročilým srdečním selháním, u nichž byly vyčerpány ostatní možnosti léčby a kteří splňují kritéria pro indikaci k srdeční transplantaci. Rozbor čekací listiny: K 22. 6. 2009 bylo na čekací listině na transplantaci srdce zařazeno 51 nemocných. V 90 % šlo o nemocné s ICHS nebo dilatační kardiomyopatií. Medián čekací doby byl 307 dnů pro pacienty v urgentním a 398 dnů pro pacienty v normálním pořadí. Závažná plicní hypertenze se vyskytla u 22 % nemocných, medián koncentrace natriuretického peptidu typu B (BNP) byl 517,9 ng/l s vysokou interindividuální variabilitou. U 80 % nemocných byly využity nefarmakologické postupy léčby srdečního selhání, nejčastěji revaskularizace myokardu, implantace kardioverteru-defi brilátoru (ICD) a resynchronizační léčba. U 20 % nemocných byly nalezeny preformované protilátky proti HLA. Závěr: Domníváme se, že tento rozbor a diskuse o způsobu péče o nemocné s pokročilým srdečním selháním může přinést zajímavé informace pro klinické kardiology a může napomáhat v péči o tyto nemocné.
Introduction: The waiting list for heart transplantation comprises patients with advanced heart failure in whom all other therapeutic options have failed and who meet the criteria for heart transplantation. Waiting list analysis: As of 22 June 2009, the waiting list for heart transplantation included 51 patients. Ninety percent of these were individuals with coronary heart disease or dilated cardiomyopathy. The median waiting time was 307 days for urgent patients, and 398 days for those listed as normal status candidates. Severe pulmonary hypertension was present in 22% of patients, median B-natriuretic peptide concentration was 517.9 n ith high inter-individual variability. Eighty percent of patients were treated using non-pharmacological management of heart failure, most often by myocardial revascularization, placement of an implantable cardioverter- -defi brillator, and cardiac resynchronization therapy. Preformed anti-HLA antibodies were detected in 20% of patients. Conclusion: We hope this analysis of and discussion about the techniques of management of patients with advanced heart failure may furnish information of interest for the clinical cardiologist and be helpful in improving care of these patients.

Public-access defibrillation with automated external defibrillators (AEDs) is being implemented in many countries worldwide with considerable financial implications. The potential benefit and economic consequences of focused or unfocused AED deployment are unknown. METHODS AND RESULTS: All cardiac arrests in public in Copenhagen, Denmark, from 1994 through 2005 were geographically located, as were 104 public AEDs placed by local initiatives. In accordance with European Resuscitation Council and American Heart Association (AHA) guidelines, areas with a high incidence of cardiac arrests were defined as those with 1 cardiac arrest every 2 or 5 years, respectively. There were 1274 cardiac arrests in public locations. According to the European Resuscitation Council or AHA guidelines, AEDs needed to be deployed in 1.2% and 10.6% of the city area, providing coverage for 19.5% (n=249) and 66.8% (n=851) of all cardiac arrests, respectively. The excessive cost of such AED deployments was estimated to be $33 100 or $41 000 per additional quality-adjusted life year, whereas unguided AED placement covering the entire city had an estimated cost of $108 700 per quality-adjusted life year. Areas with major train stations (1.8 arrests every 5 years per area), large public squares, and pedestrianized areas (0.6 arrests every 5 years per area) were main predictors of frequent cardiac arrests. CONCLUSIONS: To achieve wide AED coverage, AEDs need to be more widely distributed than recommended by the European Resuscitation Council guidelines but consistent with the American Heart Association guidelines. Strategic placement of AEDs is pivotal for public-access defibrillation, whereas with unguided initiatives, AEDs are likely to be placed inappropriately.
