Lumbální spinální stenóza je klinicko‑radiologický syndrom, nejnovější definice zohledňují klinickou manifestaci i anatomické změny (zúžení páteřního kanálu). Za nejvhodnější neinvazivní vyšetření k potvrzení anatomického zúžení páteřního kanálu nebo přítomnosti komprese nervových kořenů u pacientů s klinickým podezřením na lumbální spinální stenózu je považována magnetická rezonance bederní páteře. K přesnějšímu hodnocení radiologického nálezu byla definována řada kvantitativních parametrů popisujících rozměry páteřního kanálu, nicméně korelace těchto parametrů s klinickým obrazem není uspokojivá. Perspektivním postupem ke zvýšení výtěžnosti radiologického vyšetření se jeví zohlednění dynamického aspektu lumbální spinální stenózy a důraz na tíži komprese nervových a cévních struktur v páteřním kanálu s využitím morfologického popisu durálního vaku a jeho obsahu.
Lumbar spinal stenosis is a clinical-radiological syndrome; a recent definition covers both the clinical manifestation and the anatomic abnormality (narrowing of the spinal canal). Magnetic resonance imaging is suggested as the most appropriate non-invasive test to confirm the presence of anatomic narrowing of the spinal canal or the presence of nerve root impingement in patients with clinical suspicion of lumbar spinal stenosis. Many numerical parameters describing the dimensions of spinal canal have been defined for more precise assessment of radiological images; correlation between these radiological parameters and clinical symptoms, however, is unsatisfactory. To improve the usefulness of radiological imaging in patients with lumbar spinal stenosis, it seems to be meaningful to take into account the dynamic aspect of lumbar spinal stenosis and to assess neural and vascular tissue impingement based on morphology of the dural sac and its content. Prezentujeme vzácnou kazuistiku schwannomu v lumbo-sakrální oblasti páteřního kanálu u adolescenta. Nemoc se u šestnáctiletého chlapce projevila noční segmentovou bolestí zad a snížením citlivosti v dermatomu L4 s převahou na pravé straně. Postupná svalová slabost dolních končetin byla pozorována dva měsíce před operací. Vyšetření pomocí magnetické rezonance páteře prokázalo dobře ohraničenou masu uvnitř páteřního kanálu. Peroperačně byla nádorová tkáň křehká, vaskularizovaná, v kontaktu s filum terminale a byla kompletně odstraněna. Spinální schwannom byl histologicky potvrzen.
We present a rare case of a spinal schwannoma of a lumbo-sacral tract in an adolescent. A 16-year-old boy presented with segmental back pain and decreased sensation in the L4 dermatome predominately at the right side. Progressive weakness of lower extremities. It was noted for two months before surgery. Spine magnetic resonance imaging revealed a well – demarcated isointense intraspinal mass. Intraoperatively, the tumorous tissue was brittle, vascularized, attached to filum terminale and completely removed. Spinal schwannoma was histologically confirmed.

Pneumorachis (PNR) je stav, kdy se v páteřním kanále nachází vzduch. Jde o situaci neobvyklou, většinou diagnostikovanou náhodně při zobrazovacích vyšetřeních zaměřených na jiné orgánové systémy. PNR je diagnóza radiologická, ne klinická. Vzduch se do páteřního kanálu dostává šířením měkkými tkáněmi a podél vertebrálních nervověcévních svazků nejčastěji u pacientů, u nichž došlo k rozvoji pneumomediastina, pneumotoraxu nebo pneumoperitonea. Ve většině případů je PNR asymptomatické a není provázeno žádnými komplikacemi. Uveden je případ adolescenta, kde ke vzniku PNR došlo na podkladě spontánního pneumomediastina.
Pneumorrhachis (PNR), the phenomenon of intraspinal air, is an exceptional imaging finding. PNR is primarily a radiographic and not clinical diagnosis. Air within the spinal canal can be divided into primary and secondary PNR, descriptively classified into extra- or intradural. Usually is PNR seen in association with other manifestation of air leak (for example pneumomediastinum, pneumothorax, pneumoperitoneum). PNR usually represents an asymptomatic epiphenomenon and the intraspinal air reabsorbs spontaneously. We report the case of an adolescent who developed PNR after spontaneous pneumomediastinum. The patient recovered uneventfully and no underlying cause was found.

BACKGROUND: Marfan's syndrome is an inherited connective tissue disorder that affects many organ systems and has widespread phenotype expression. The diagnosis is therefore made by phenotype assessment. Dural ectasia has been classified as a major diagnostic criterium with a prevalence of over 90% in patients with Marfan's syndrome. The objective of this study determine the feasibility of performing CT angiographic examination of aorta and large vessels for dural ectasia grading in a single CT exam of patients with Marfan's syndrome. PATIENTS AND METHODS: 7 examinations in Marfan patients were performed of which 6 were CTA. In all exams the aorta was evaluated and simultaneously the lumbosacral region of the spine was well delineated. We performed calculation of spinal canal (SCI) and dural sac (DSI) indices and also qualitative grading of dural sac involvement. RESULTS: In 4 patients both SCI (> 4,50) and DSI (> 3,75) fulfil the diagnostic criteria of Marfan's syndrome. In 1 patient complete spondyloptosis was detected. CONCLUSIONS: Whole body CT examination is the perfect tool for both aorta and spinal canal evaluation. Despite differences in spinal canal grading methods in all of our patients significant changes of the spinal canal in the lumbosacral region were detected. Dural ectasia is easy to detect and quantify and may therefore be used as a sensitive and specific sign of Marfan's syndrome.
