PURPOSE OF THE STUDY The literature is conflicting as to whether the high hip center (HHC) reconstruction in total hip arthroplasty (THA) is an ideal option for patients with severe developmental dysplasia of the hip (DDH). This study aimed to compare the mid-term functional and radiographic outcomes of THA using HHC versus anatomical hip center (AHC) technique in Crowe types II and III DDH. Our hypothesis was that there may be no differences in terms of functional and radiographic outcomes between patients who underwent THA using HHC or AHC. MATERIAL AND METHODS Fifty-seven patients who underwent a primary THA due to Crowe type-II or type-III DDH were retrospectively reviewed and included. Patients were divided into two groups as per the hip center reconstruction: Group A (AHC technique) and group H (HHC technique). A cementless cup was inserted in 25 hips (19 female, 6 male; mean age = 51 years, age range = 28-67)) at near-AHC in group A and 32 hips (22 female, 10 male; mean age = 53 years, age range = 29-68) at HHC position in group H. To assess clinical status, the Harris Hip Score (HHS) was used at the final follow-up. In radiographical assessment, component loosening and osseointegration of the acetabular cup were examined on follow-up radiographs. Complications were also recorded. RESULTS The mean follow-up was 41 months (range, 25-84) in group A and 40 months (range, 24-86) in group H. The mean HHS was 83 (range, 74-91) in group A and 83.6 (range, 73-94) in group H (p = 0.741). Osteolysis was determined in three patients from each group. The other 51 cups demonstrated a minimum of one radiographic sign of osseointegration. Although the overall complication rate was higher in group A (64%) than in group H (46%), this difference reached no statistical significance (p = 0.11). CONCLUSIONS The HHC technique using cementless acetabular fixation seems to be a valuable alternative option to AHC technique in cases of Crowe types II and III DDH. Several studies have documented that formation of oxidant mediators may induce apoptosis in nucleated and anucleated cells by modulating intracellular signalling pathways. Reactive oxygen species (ROS) play a very important role in the platelet function. γ-Glutamyltransferase (GGT), a novel source of cellular production of oxidants in the presence of iron and reduced glutathione (GSH), is also found on platelets. The role of platelet-bound GGT in platelet apoptosis and oxidative stress is unknown. The aim of our study was to determine the effects of platelet GGT activity on oxidative stress and apoptotic events in vitro via determination of lipid peroxidation (LPO), protein oxidation, GSH, catalase, caspase-3 activation and phosphatidylserine (PS) exposure in the presence of holo-transferrin (Tf). Stimulation of platelet GGT activity with GSH and glycylglycine (GlyGly) increased caspase-3 activation and PS exposure. A significant increase in lipid and protein oxidation and decrease in GSH and catalase levels was also observed in platelets with stimulation of GGT activity in the presence of Tf. Inhibition of GGT activity effectively reduced all the markers. These results suggest that generation of ROS by the GGT/GSH/Tf system can modify the platelets' redox environment and induce apoptosis in in vitro conditions.

Cíl: Cílem naší studie bylo odlišit glioblastom (GBM) od solitární metastázy mozku za pomoci strojových modelů vyvinutých na základě radiomických dat získaných automatickou segmentací nádoru z konvenčích MR skenů pacientů pomocí umělé inteligence. Metody: Naše studie byla prováděna na jednom pracovišti a byla retrospektivní. Do studie bylo zařazeno 35 pacientů s GBM a 25 pacientů se solitární metastázou na mozku, u nichž byla před operací provedena MR mozku s kontrastní látkou. Do programu BraTumIA byly nahrány T1 vážené obrazy, T1 vážené obrazy po podání kontrastní látky, T2 vážené obrazy a T2 vážené obrazy s využitím sekvence fluid attenuated inversion recovery (FLAIR). V programu byly léze pacienta pomocí umělé inteligence rozděleny do čtyř různých segmentů: nekróza, nesytící se solidní oblast, sytící se solidní oblast a peritumorózní edém. Z T1 obrazů po podání kontrastní látky a T2 FLAIR obrazů bylo extrahováno 856 znaků. Pro výběr znaků, optimalizaci modelu a validaci byl použit vnořený (nested) přístup. Byly modelovány umělé neuronové sítě, podpůrný vektorový stroj, náhodný les a naivní bayesovský klasifikátor. Funkce modelu byla hodnocena pomocí přesnosti, senzitivity, specificity a plochy pod křivkou (area under the curve; AUC). Výsledky: Mezi skupinami s GBM a s metastázou nebyly rozdíly ve věku a pohlaví. Nejúspěšnější výsledky byly získány pomocí algoritmu neuronové sítě – byla získána hodnota AUC 0,970. U algoritmů za použití podpůrného vektorové stroje, naivního bayesovského klasifikátoru, logistické regrese či náhodného lesu byly získány hodnoty AUC 0,959, 0,955, 0,955, respektive 0,917. Závěr: V diferenciální diagnostice GBM a solitárních metastáz mozku mohou modely umělé inteligence založené na radiomických datech pomocí automatické segmentace objektivně a s vysokou přesností odlišovat tak, že závislost na prostředku a osobě udržují na nejnižší úrovni za použití prostých konvenčních sekvencí.
Aim: Our study aimed to distinguish glioblastoma (GBM) from solitary brain metastasis with machine models developed with radiomics data obtained by artificial intelligence-based automatic tumour segmentation over conventional MRI of the patients. Methods: Our study was conducted as single-centre and retrospective. Thirty-five GBM and 25 solitary brain metastasis patients who had pre-operative contrast-enhanced brain MRI were included in the study. T1-weighted, postcontrast T1-weighted, T2-weighted and fluid attenuated inversion recovery (FLAIR) T2-weighted images of the patients were uploaded to the program named BraTumIA. With the program, the patient‘s lesions were divided into four different segments by artificial intelligence as necrosis, non-enhancing solid area, enhancing solid area and peritumorous oedema. 856 features were extracted from T1 post-contrast and T2 FLAIR images. A nested approach was used for feature selection, model optimization and validation. Artificial neural networks, support vector machine, random forest and naive bayes were modelled. Accuracy, sensitivity, specificity and area under the curve (AUC) parameters were used to evaluate the model performance. Results: There was no difference between GBM and metastasis groups in terms of age and gender. The most successful results were obtained in the neural network algorithm; 0.970 AUC was found. Other support vector machine, naive bayes, logistic regression and random forest algorithms also found 0.959, 0.955, 0.955, 0.917 AUC values, respectively. Conclusion: In the differential diagnosis of GBM and solitary brain metastasis, radiomics-based artificial intelligence models obtained by automatic segmentation can distinguish objectively and with high accuracy by keeping device and person dependency at the lowest level with only conventional sequences.

Low-density lipoprotein (LDL) modifications and platelet activation are major risk factors for cardiovascular diseases. When platelets are exposed to oxidative stress, they become activated. Oxidized LDL (ox-LDL) and metal-catalysed oxidation systems such as Fe3+/ascorbic acid increase free radical production. We wanted to verify whether melatonin has a protective effect against oxidative modifications and phosphatidylserine externalization in platelets induced by ox-LDL and Fe3+/ascorbic acid. For in vitro effects of melatonin on platelets, ADPactivated platelets were incubated with ox-LDL or Fe3+/ascorbic acid for 1 h at 37 oC with or without melatonin. Then platelet malondialdehyde, protein carbonyl and glutathione levels were measured. Platelet phosphatidylserine exposure was measured with annexin-V using flow cytometry. Malondialdehyde, protein carbonyl and phosphatidylserine levels of platelets treated with Fe3+/ascorbic acid significantly increased compared to the control group. Glutathione contents of Fe3+/ascorbic acid-treated platelets significantly decreased. Melatonin pre-treatment of Fe3+/ascorbic acid-treated platelets caused a mar ked reduction in malondialdehyde and phosphatidylserine levels and a marked increase in glutathione levels. Melatonin also caused non-significant reduction in protein carbonyl contents of Fe3+/ascorbic acid-treated platelets. Malondialdehyde, protein carbonyl and phosphatidylserine levels of platelets treated with ox-LDL also significantly increased compared to the control group. Platelet glutathione levels non-significantly decreased with ox-LDL. With addition of melatonin, malondialdehyde, protein carbonyl and phosphatidylserine levels of platelets treated with ox-LDL significantly decreased. These data suggest that melatonin may protect platelets from iron overload-induced and ox-LDL-induced oxidative modifications and also from the triggering signals of apoptosis activation, possibly due to its scavenger effect on toxic free radicals.

The aim of the present study was to evaluate the possible protective effect of polydatin (PD) on cisplatin (Cis) induced oxidative stress in rats. Totally, thirty male Wistar albino rats were fed standard rodent diet and divided into 5 equal groups: the control group (vehicle treated) was treated with physiological saline for ten days both orally and intraperitoneally (i.p.), the second group was orally treated with physiological saline and 7 mg/kg single i.p. injection of Cis on the seventh day, and third, fourth, and fifth groups were treated orally PD at 25, 50, and 100 mg/kg/day, respectively for 10 days starting seven days before Cis injection and 7 mg/kg single i.p. Cis was injected on the seventh day. Cis resulted in significant increase malondialdehyde levels and decreased glutathione levels. In addition, Cis treatment decreased superoxide dismutase and catalase activities in erythrocyte and tissues. Also, Cis treatment caused to increase DNA damage and affected serum biochemical parameters whereas slightly decreased AchE activity. However, treatment of PD resulted in reversal of Cis-induced oxidative stress, lipid peroxidation, and activities of antioxidant enzymes. In conclusion, PD has protective effect in rats against Cis-induced oxidative stress, enhances antioxidant defence mechanism, and regenerates their tissues.

Activated platelets and glycated lipoproteins are responsible for atherothrombosis in diabetics. Melatonin and native high-density lipoproteins are crucial in the preservation of pro/oxidant-antioxidant balance. The aim of the present study was to investigate the in vitro effects of native high-density lipoproteins and melatonin on altering the platelet response induced by glycated lipoproteins. Low-density lipoproteins and high-density lipoproteins were purified from plasma by ultracentrifugation and were glycated with glucose for three weeks. After incubation with or without melatonin/or native highdensity lipoproteins, low-density lipoproteins, glycated low-density lipoproteins/glycated high-density lipoproteins were added to ADP-induced platelets. Oxidative parameters, caspase-3/9 and nitric oxide levels were measured spectrophotometrically; CD62-P/ annexin-V expression was determined by flow cytometry. In glycated low-density lipoprotein/glycated high-density lipoprotein-treated groups, platelet malondialdehyde/ protein carbonyl, P-selectin, annexin-V, caspase-3/9 levels were increased (ranging from P < 0.001 to P < 0.01); glutathione and nitric oxide levels were reduced (ranging from P < 0.001 to P < 0.01). In glycated low-density lipoprotein/glycated high-density lipoprotein-treated groups, melatonin treatment reduced malondialdehyde, protein carbonyl, CD62-P, annexin-V and caspase-3/9 (P < 0.001, P < 0.01) levels and elevated nitric oxide (only glycated low-density lipoproteins). In glycated low-density lipoprotein/glycated high-density lipoprotein-treated groups, native high-density lipoprotein treatment reduced malondialdehyde, protein carbonyl, annexin-V, caspase-3/9 levels (P < 0.001, P < 0.01) and increased glutathione; nitric oxide levels (only with gly-HDL). Both melatonin and high-density lipoproteins should be regarded as novel promising mechanism-based potential therapeutic targets to prevent atherothrombosis in diabetics.

AIM/BACKGROUND: Pistacia terebinthus is used as a coffee substitute in the East and Southern Anatolia regions of Turkey. It contains unsaturated fatty acids, tocopherols, polyphenols and carotenoids. P. terebinthus has anti-inflammatory and potential antioxidant activity. In this study we evaluated the protective effects of P. terebinthus coffee (PTC) on thioacetamide (TAA)-induced liver injury in rats. MATERIALS AND METHODS: Twenty-eight male Sprague-Dawley rats were equally randomized into four groups. Chronic liver injury was induced with TAA (100 mg/kg i.p. three times weekly). The first group of rats served as control and received only tap water (G1), and the remaining groups of rats received PTC, p.o (G2); TAA (G3); TAA plus PTC, p.o (G4), respectively. RESULTS: After 8 weeks, PTC intake significantly reduced fibrosis/inflammation scores (p PTC intake reduced transforming growth factor beta (TGF-β) concentrations in the liver (p PTC intake. DISCUSSION AND CONCLUSION: PTC intake provided beneficial effects against TAA-induced liver injury in rats. PTC probably suppresses the proinflammatory cytokines through NF-κB signaling pathway.

Abnormal drainage of the superior vena cava into the left atrium is a rare congenital cardiac malformation. It leads to cyanosis due to right-to-left shunt. In this report we present the management strategy of a 43-day-old baby who was investigated for mild cyanosis since birth and was diagnosed with superior vena cava draining into the left atrium.
