V naší laboratoři bylo vyšetřeno 376 vzorků trusu koček (01/2009 až 06/2009) pomocí PCR na Tritrichomas foetus (T.f). Jednalo se o případy zaslané veterinárními lékaři s podezřením na T.f. Kočky pocházely převážně z Německa, několik vzorků ze zahraničí. Třicet šest zvířat mělo pozitivní nález na T.f (9,6 %), u čtyř dalších koček (1 %) bylo prokázáno Tritrichomonas spp., Pokud to bylo možné, byly shromážděny podmínky chovu a anamnéza pozitivních zvířat. Převážná část těchto zvířat byla jeden rok stará nebo mladší. Jednalo se téměř výlučně o čistokrevné kočky. Mnoho jich žilo v chovech, v útulcích nebo alespoň v domácnostech s více kočkami, avšak také se vyskytovaly kočky chované samostatně a evropské krátkosrsté kočky. Dodatečně bylo vyšetřeno dalších 297 vzorků trusu koček (12/2008 až 03/2009) pomocí PCR na T. specifickém – PCR. U dvou koček bylo nejdříve odhaleno molekulárně biologicky Pentatrichomonas hominis (P. hominis) bez přítomnosti T.f. Data ukázala, že T.f. se vyskytuje jako možný původce průjmů koček také v Německu a musíme ji zahrnout do diferenciální diagnostiky. Musí být proveden výzkum patogenity P. hominis, stejně tak i další studie o prevalenci k T.f a P. hominis. f. Tyto vzorky pocházely ze zásilek k diagnostice průjmů a byly vybrány jen na základě hlenově – krvavého charakteru trusu. Další data k těmto kočkám nebyla dostupná. Z těchto 297 zvířat reagovalo 6,7 % pozitivně na trichomonády –PCR a polovina těchto zvířat v T.f. specifickém – PCR. U dvou koček bylo nejdříve odhaleno molekulárně biologicky Pentatrichomonas hominis (P. hominis) bez přítomnosti T.f. Data ukázala, že T.f. se vyskytuje jako možný původce průjmů koček také v Německu a musíme ji zahrnout do diferenciální diagnostiky. Musí být proveden výzkum patogenity P. hominis, stejně tak i další studie o prevalenci k T.f a P. hominis.

There were examined 376 samples of cat feces (from 01/2009 to 06/2009) using the PCR for Tritrichomonas foetus (T.f.) in our laboratory. The veterinarians sent us these samples with a suspicion of T.f. The cats came mostly from Germany, a few came from foreign countries. 36 animals (9.6%) were positive for T.f., in 4 other cats (1%) Trichomonas spp. was proved. In cases where it was possible, the anamnesis and the housing conditions of positive animals were collected. Most of these animals were one year old or younger. Almost all of them were purebred cats. Many cats lived in breeding, in shelters or at least in households with more cats. But there were also separately kept cats and European Shorthairs. Other 297 samples of cat feces were examined additionally (from 12/2008 to 03/2009) using the PCR for T.f. These samples came from shipments sent for the diagnosis of diarrhea and they were selected only on the basis of bloody mucus character of the feces. Other data about these cats were not accessible. 6.7% from these 297 animals responded positively to Trichomonas-PCR and a half of these animals to T.f. specific-PCR. Pentatrichomonas (P.) hominis without a presence of T.f. was detected in 2 cats primarily based on the molecular biology. The data showed that T.f. occurs as a potential etiological agent of diarrhea in cats also in Germany and it is necessary to include it in a differential diagnosis. It is necessary to realize a research on the pathogenicity of P. hominis and other studies on the prevalence of T.f. and P. hominis.

BACKGROUND: Electrographic flow (EGF) mapping enables full spatiotemporal reconstruction of organized wavefront propagation to identify extrapulmonary vein sources of atrial fibrillation (AF). OBJECTIVES: FLOW-AF (A Randomized Controlled Study to Evaluate the Reliability of the Ablacon Electrographic FLOW [EGF] Algorithm Technology [Ablamap Software] to Identify AF Sources and Guide Ablation Therapy in Patients With Persistent Atrial Fibrillation) was multicenter, randomized controlled study of EGF mapping to: 1) stratify a nonparoxysmal AF population undergoing redo ablation; 2) guide ablation of these extrapulmonary vein AF sources; and 3) improve AF recurrence outcomes. METHODS: FLOW-AF enrolled persistent atrial fibrillation (PerAF)/long-standing PerAF patients undergoing redo ablation at 4 centers. One-minute EGF maps were recorded from standardized biatrial basket positions. Patients with source activity ≥26.5% were randomized 1:1 to PVI + EGF-guided ablation vs PVI only; patients without sources ≥26.5% threshold were not randomized. Follow-up and electrocardiographic monitoring occurred at 3, 6, and 12 months. RESULTS: We enrolled 85 patients (age 65.6 ± 9.3 years, 37% female, 24% long-standing PerAF). Thirty-four (40%) patients had no sources greater than threshold; at least 1 source greater than threshold was present in 46 (60%) (EGF-guided ablation, n = 22; control group, n = 26). Patients with sources were older (68.2 vs 62.6 years; P = 0.005) with higher CHA2DS2-VASc scores (2.8 vs 1.9; P = 0.001). The freedom from safety events was 97.2%, and 95% of EGF-identified sources were successfully ablated. In randomized patients, AF-free survival at 12 months was 68% for EGF-guided ablation vs 17% for the control group (P = 0.042); freedom from AF/atrial tachycardia/atrial flutter at 12 months was 51% vs 14% (P = 0.103), respectively. CONCLUSIONS: In nonparoxysmal AF patients undergoing redo ablation, EGF mapping identified AF sources in 60% of patients, and could be successfully ablated in 95%. Compared with PVI alone, PVI + source ablation improved AF-free survival by 51% on an absolute basis. BACKGROUND: Electrographic flow (EGF) mapping enables the dynamic detection of functional or active atrial fibrillation (AF) sources outside the pulmonary veins (PVs), and the presence or absence of these sources offers a novel framework for classifying and treating persistent AF patients based on the underlying pathophysiology of their AF disease. OBJECTIVE: The primary objective of the FLOW-AF trial is to evaluate the reliability of the EGF algorithm technology (Ablamap software) to identify AF sources and guide ablation therapy in patients with persistent AF. METHODS: The FLOW-AF trial (NCT04473963) is a prospective, multicenter, randomized clinical study in which patients with persistent or long-standing persistent AF who have failed prior PV isolation (PVI) undergo EGF mapping after confirmation of intact PVI. In total, 85 patients will be enrolled and stratified based on the presence or absence of EGF-identified sources. Patients with an EGF-identified source above the predetermined activity threshold of ≥26.5% will be randomized in a 1:1 fashion to PVI only vs PVI + ablation of EGF-identified extra-PV sources of AF. RESULTS: The primary safety endpoint is freedom from serious adverse events related to the procedure through 7 days following the randomization procedure; and the primary effectiveness endpoint is the successful elimination of significant sources of excitation with the target parameter the activity of the leading source. CONCLUSIONS: The FLOW-AF trial is a randomized study designed to evaluate the ability of the EGF mapping algorithm to identify patients with active extra-PV AF sources.

Machine learning classifications of first-episode psychosis (FEP) using neuroimaging have predominantly analyzed brain volumes. Some studies examined cortical thickness, but most of them have used parcellation approaches with data from single sites, which limits claims of generalizability. To address these limitations, we conducted a large-scale, multi-site analysis of cortical thickness comparing parcellations and vertex-wise approaches. By leveraging the multi-site nature of the study, we further investigated how different demographical and site-dependent variables affected predictions. Finally, we assessed relationships between predictions and clinical variables. 428 subjects (147 females, mean age 27.14) with FEP and 448 (230 females, mean age 27.06) healthy controls were enrolled in 8 centers by the ClassiFEP group. All subjects underwent a structural MRI and were clinically assessed. Cortical thickness parcellation (68 areas) and full cortical maps (20,484 vertices) were extracted. Linear Support Vector Machine was used for classification within a repeated nested cross-validation framework. Vertex-wise thickness maps outperformed parcellation-based methods with a balanced accuracy of 66.2% and an Area Under the Curve of 72%. By stratifying our sample for MRI scanner, we increased generalizability across sites. Temporal brain areas resulted as the most influential in the classification. The predictive decision scores significantly correlated with age at onset, duration of treatment, and positive symptoms. In conclusion, although far from the threshold of clinical relevance, temporal cortical thickness proved to classify between FEP subjects and healthy individuals. The assessment of site-dependent variables permitted an increase in the across-site generalizability, thus attempting to address an important machine learning limitation.
