Cieľ: Prezentácia výsledkov reoperácie 3 prípadov neuzavretej makulárnej diery (MD) pomocou 23 gauge pars plana vitrektómie (PPV), s rozšírením peelingu vnútornej limitujúcej membrány (MLI) a aplikáciou zátky zo sušenej ľudskej amniovej membrány (hAM) subretinálne cez dieru makuly s následnou tamponádou kavity sklovca expanzným plynom. Materiál a metodika: Diagnóza perzistujúcej diery makuly u troch pacientiek unilaterálne sa potvrdila pomocou SD-OCT. U prvej pacientky sa jednalo o primárnu dieru, u druhej pacientky o sekundárnu dieru po cystoidnom edéme makuly ako následok oklúzie centrálnej vény sietnice a u tretej pacientky bola diagnostikovaná atrofia pigmentového epitelu (RPE) vo foveole niekoľko rokov pred rozvojom MD. Prvé dve pacientky podstúpili reoperáciu o štyri mesiace po primárnej operácii, tretia pacientka podstúpila až dve predchádzajúce pars plana vitrektómie (PPV), poslednú pred jedenástimi rokmi. V úvode operácie sa najskôr vykonala revízia periférie a odstránenie zvyškov sklovca, rozšírila sa zóna peelingu MLI. Pripravila sa zátka zo sušenej hAM. Následne sa plug hAM mikropinzetou inzeroval cez MD subretinálne. Realizovala sa štandardná kryopexia za sklérotómie, výmena tekutiny za vzduch a kavita sklovca sa tamponovala expanzným plynom. Výsledky: U dvoch pacientiek sa dosiahlo uzavretie MD, u tretej pacientky sa značne a dlhodobo zredukovalo cystoidné presiaknutie okrajov a zmešenie priemeru defektu, avšak MD ostala otvorená. Všetky pacientky zaznamenali mierne zlepšenie zrakovej ostrosti a stratu rušivých zrakových fenoménov. Záver: Potvrdili sme, že inzercia hAM plugu je plne indikovaná u perzistujúcich MD aj veľkých rozmerov. Je vhodné bazálnu membránu zátky orientovať smerom k neurosenzorickej časti sietnice a chorionálnu stranu k RPE kvôli rastovým faktorom, ale aj pre konkavitu zátky smerom k RPE. Je možné, že uprednostnenie tamponády perfluoropropánom (C3F8) je výhodnejšie ako tamponáda sulfurhexafluoridom (SF6). Termín reoperácie približne 3-4 mesiace po prvej zlyhanej vitrektómii možno považovať za optimálny.
Purpose: To evaluate the results of 3 cases with persistent macular holes (MH) treated by 23-gauge vitrectomy, extension of internal limiting membrane peeling, a human amniotic membrane (hAM) plug insertion into the subretinal space through MH and expanding gas endotamponade. Material and methodology: The diagnosis of persistent MH in three patients was unilaterally confirmed using SD-OCT. In the first patient a primary MH was present. In the second patient a secondary MH occurred after cystoid macular edema because of central retinal vein occlusion. The third patient suffered with sustained atrophy of the retinal pigment epithelium (RPE) in the foveola several years before the development of MH. All patients were females. The first two patients underwent reoperation four months after the primary surgery, the third patient underwent two previous pars plana vitrectomies (PPVs), the last one 11 years ago. First a revision of the periphery and removal the vitreous was performed, the ILM peeling zone was extended. The plug from the dehydrated hAM was prepared. Subsequently, the hAM plug was inserted via MH subretinally. Standard cryopexy behind the sclerotomies, fluid-for-air exchange, and vitreous cavity tamponade with expansile gas were performed. Results: Two patients achieved MH closure, in the third patient surgery significantly reduced cystoid edema of the MH edges and the MH diameter, but the MH remained open. All patients experienced a mild improvement in visual acuity and loss of disturbing visual phenomena. Conclusion: We have confirmed that hAM plug insertion is feasible for persistent MH even of large sizes. It is essential to orient the basal membrane of the plug towards the neurosensory part of the retina and the chorionic side to the RPE due to growth factors but also for the concavity of the plug towards the RPE. It is possible that the use of tamponade with perfluoropropane (C3F8) is preferable to tamponade with sulfur hexafluoride (SF6). The time of reoperation approximately 3–4 months after the first failed vitrectomy can be considered optimal.

Krvácanie z pažerákových varixov je u pacientov s portálnou hypertenziou spájané s vysokou mortalitou. Títo pacienti sa k zdravotnej starostlivosti často dostávajú prostredníctvom ambulancií záchrannej služby a príjmových ambulancií nemocníc. K endoskopickému riešeniu sa však často dostanú až po niekoľkých hodinách. V situáciách prednemocničnej a neodkladnej nemocničnej starostlivosti, kedy kriticky chorý pacient nereaguje na farmakologickú liečbu je potrebné zvážiť prechodné riešenie. Na tento účel je dnes k dispozícii Sengstaken-Blakemorová sonda (a jej rôzne varianty) alebo pažerákový stent (Danišov stent). Účelom tohto prehľadu bolo nájsť a popísať informácie o použiteľnosti pažerákového stentu v kritických situáciách urgentnej starostlivosti (pri nereagujúcej farmakologickej liečbe a časovej nedostupnosti akútneho endoskopického riešenia). Do prehľadu boli zahrnuté publikácie od roku 2010. Súčasné štúdie, ktoré popisujú klinické údaje o Danišovom stente, sú však zamerané na situácie po zlyhaní predchádzajúcej liečby. Chýbajú tak priame klinické údaje o využití tejto metódy pred endoskopickou intervenciou. Dostupné klinické štúdie a doporučené postupy v danej oblasti však uvádzajú, že Danišov stent vo všeobecnosti dosahuje lepší pomer prínosov a rizík a takisto aj lepšiu nákladovú efektivitu v porovnaní so Sengstaken-Blakemorovou sondou. V budúcnosti by boli vhodné ďalšie štúdie, ktoré by poskytli klinické údaje o použití pažerákového stentu ako primárneho riešenia život ohrozujúcich stavov bez predchádzajúcich intervencií.
Bleeding from esophageal varices is associated with high mortality in patients with portal hypertension. These patients often get access to health care through ambulances of the emergency service and emergency departments of hospitals. However, it often takes several hours to get the endoscopic treatment. In situations of pre-hospital and urgent hospital care, when a critically ill patient does not respond to pharmacological treatment, it is necessary to consider a temporary solution. For this purpose, a Sengstaken- Blakemore tube (and its variants) or an esophageal stent (Danis stent) are available today. The purpose of this review was to find and describe the information on the esophageal stent applicability in critical situations of urgent care (when pharmacological treatment failed, and acute endoscopic solution is temporarily unavailable). Publications since 2010 were included in the review. However, the current studies describing clinical data on Danis stent are focused on situations after failure of previous treatment. Thus, there is a lack of direct clinical data on the use of this method before endoscopic intervention. However, available clinical studies and guidelines in the field indicate that the Danis stent generally achieves a better benefit-risk ratio and also better cost-effectiveness compared to the Sengstaken–Blakemore tube. Future studies would be useful to provide clinical data on esophageal stenting as a primary treatment for life-threatening conditions without prior intervention.
AIMS: To evaluate the estimated fetal radiation dose during prophylactic internal iliac arterial occlusion in patients with abnormal placenta and to estimate the risk of radiation induced cancer in child age. METHODS: Prophylactic occlusion of the internal iliac arteries during Caesarean section was performed in 42 patients with placenta praevia and/or placenta accreta spectrum. Fogarty embolectomy catheters were used for prophylactic occlusion of the internal iliac arteries. All procedures were performed in the hybrid operating room using Philips Allura Xper FD 20 X-ray system. Low dose X-ray fluoroscopy (7.5 frames per second) was used. The CODE (Conceptus dose estimation) Software was used to estimate the fetal dose and the risk of radiation induced carcinoma. RESULTS: Fluoroscopy times required for insertion of Fogarty catheters were 0.5-4.2 min (mean: 1.7 min, median: 1.5 min). The estimated radiation dose to the fetus was 0.26-3.36 mGy (mean: 1.49 mGy, median: 1.25 mGy). The risk of radiation induced cancer in child age was 0.01-0.04% (mean 0.02%, median 0.01%). One patient developed thrombosis of a common femoral artery. CONCLUSION: Prophylactic occlusion of the internal iliac arteries is a simple and safe procedure with minimal risk of complications and with a very low estimated radiation dose to the fetus.

Cíl studie: Popsat případ profylaktické balonkové bilaterální okluze ilických aretrií během císařského řezu u Svědkyně Jehovovy. Typ studie: Kazuistika. Pracoviště: Porodnicko-gynekologická klinika LF a FN Olomouc; Radiologická klinika LF a FN Olomouc; Ústav zdravotnických věd FHS UTB; Gynekologickoporodnické oddělení, Nemocnice Přerov, SMN, a.s. Vlastní pozorování: Popisujeme případ oboustranné profylaktické balonkové okluze ilických arterií během císařského řezu u Svědkyně Jehovovy s cílem snížení rizika těžkého peroperačního krvácení. Dvaceti osmiletá primipara podstoupila profylaktickou balonkovou katetrizaci vnitřních ilických arterií pomocí intervenční radiologie. Dva 6-Fr katétry byly zavedeny transfemorálně bilaterálně do vnitřních ilických arterií v pozici „cross over“ podle Seldingerovy standardní techniky. Výkon byl bez komplikací, odhadovaná krevní ztráta byla 500 ml. Závěr: Profylaktické umístění intravaskulárních balonkových katétrů je proveditelnou metodou u Svědkyň Jehovových ve snaze o snížení rizika těžkého krvácení během císařského řezu.
Objective: To describe a case report with prophylactic bilateral iliac artery balloon occlusion during cesarean section in Jehova´s Witnesses patient. Design: Case report. Setting: Department of Obstetrics and Gynecology, University Hospital, Olomouc; Department of Radiology, University Hospital, Olomouc; Department of Health Care Sciencies, Bata University, Zlín. Case report: We describe case report with prophylactic bilateral iliac artery balloon occlusion during cesarean section in Jehova´s Witnesses patient in attempt to decrease the risk of heavy peroperative bleeding. Twenty eight years old primigravida underwent prophylactic internal iliac artery balloon catheterization with interventional radiology preoperatively. Two 6-Fr balloon catheters transfemorally bilaterally up to internal iliac artery with position a “cross over“ were introduced, according to Seldinger’s standard technique. The procedure was without complications, estimated blood loss was 500 ml. Conclusion: Prophylactic placement of intravascular balloon catheters is a feasible treatment for Jehova´s Witnesses patients in efforts to decrease the risk of heavy bleeding during cesarean section.

The author presents a case study of the use of resuscitative endovascular balloon occlusion (REBOA) as a suitable alternative to thoracotomy and clamping of the descending aorta to control retroperitoneal bleeding in a patient with a pelvic injury. The patient who suffered multiple trauma after car accident, type C pelvic injury and retroperitoneal bleeding among other things, was following the pre-hospital ambulance care transported to the department of emergency medicine, with catecholamine infusion to support the blood flow. After the primary survey following the ATLS principles, the patient was taken for a CT scan. The CT examination revealed also multiple sources of retroperitoneal bleeding. Subsequently, the patient was brought to the operating room, where endovascular balloon occlusion of the descending aorta was performed to temporarily control retroperitoneal bleeding, which provided more time to treat the patient in line with the damage control surgery principles. In bleeding patients who suffered blunt torso traumas and serious haemorrhagic shock, or patients "in extremis", the survival after emergency thoracotomy ranges only around 1%. The to date results of REBOA technique applied in same indications are very promising globally. The survival rate increases multiple times especially in hypotensive patients, without the necessity of their immediate cardiopulmonary resuscitation. Paediatric airway endoscopy is accepted as a diagnostic and therapeutic procedure, with an expanding number of indications and applications in children. The aim of this European Respiratory Society task force was to produce a statement on interventional bronchoscopy in children, describing the evidence available at present and current clinical practice, and identifying areas deserving further investigation. The multidisciplinary task force panel performed a systematic review of the literature, focusing on whole lung lavage, transbronchial and endobronchial biopsy, transbronchial needle aspiration with endobronchial ultrasound, foreign body extraction, balloon dilation and occlusion, laser-assisted procedures, usage of airway stents, microdebriders, cryotherapy, endoscopic intubation, application of drugs and other liquids, and caregiver perspectives. There is a scarcity of published evidence in this field, and in many cases the task force had to resort to the collective clinical experience of the committee to develop this statement. The highlighted gaps in knowledge underline the need for further research and serve as a call to paediatric bronchoscopists to work together in multicentre collaborations, for the benefit of children with airway disorders.

Cíl: Porovnat účinnost a bezpečnost distálního protekčního zařízení (Filtr) s proximální protekcí (Mo.Ma systém) během karotického stentingu (KAS) a potvrdit či vyvrátit nižší výskyt mikroembolizací při použití Mo.Ma systému. Určit efekt mikroembolizací na kognitivní funkce. Metodika: Padesát šest pacientů bylo randomizováno do dvou skupin dle použité protekce (skupina Filtr vs. Mo.Ma). Všichni pacienti podstoupili magnetickou rezonanci (MR) mozku před výkonem a po něm. Třicet dva pacientů podstoupilo revidovaný Adenbrookský kognitivní test (ACE-R) před a měsíc po výkonu. Dále byl sledován výskyt 30denní mortality a morbidity. Výsledky: Nové ischemické léze po KAS byly nalezeny celkem u 32,14 % pacientů (n = 18). Ve skupině Filtr (n = 37) u 32,43 % pacientů (n = 12), ve skupině Mo.Ma (n = 19) u 31,58 % (n = 6; p = 0,4741). Pouze 38,89 % lézí bylo lokalizováno čistě v povodí intervenované tepny, ve skupině Filtr 16,67 % (p = 0,006), ve skupině Mo.Ma 83,33 % (p = 0,037). Signifikantní pokles v ACE-R testu byl zaznamenán jednou. Závěr: Nové ischemické léze po KAS jsme našli v obou sledovaných skupinách, předpokládaný nižší výskyt ve skupině Mo.Ma se nepotvrdil. Ve skupině Filtr bylo signifikantně více lézí lokalizováno mimo povodí intervenované tepny nebo ve více povodích současně. Ve skupině Mo.Ma bylo signifikantně více nových lézí v povodí intervenované tepny. Negativní vliv mikroembolizací na výsledek v ACE-R testech se nám nepodařilo prokázat.
Aim: To compare safety and efficacy of distal protection devices (filters) and the proximal protection device (Mo.Ma system) during carotid artery stenting (CAS) and to prove or reject lower incidence of new microembolic lesions with Mo.Ma protection. To determine the impact of microembolic lesions after CAS on cognitive functions. Methods: Fifty-six patients were randomized into two groups according to the cerebral protection used (Filter vs. Mo.Ma group). All patients underwent brain magnetic resonance imaging (MRI) before and after stenting. Thirty-two patients were tested before and 30 days after stenting with Adenbrook Cognitive Examination revised (ACE-R) tests. Results: 32.14% (n = 18) of all patients had new ischemic lesions on MRI after CAS, 32.43% (n = 12) of the Filter group patients (n = 37) and 31.58% (n = 6) of the Mo.Ma group patients (n = 19). Only 38.89% of all new ischemic lesions were located solely in the territory of the treated artery, 16.67% in the Filter group (p = 0.006) and 83.33% in the Mo.Ma group (p = 0.037). Significant decline in ACE-R test was found in one patient only. Conclusion: New ischemic lesions after CAS were present on MRI in both groups with no significant difference. Significantly more lesions were located outside the territory of the treated artery in the Filter group and inside the territory in the Mo.Ma group. We did not prove negative impact of new lesions on the ACE-R tests results.
