The uterine tube (UT) pathologies account for 25-35% of female factor infertility. Although these peculiar organs were first studied several hundred years ago, they have become overlooked and neglected mainly due to the successes of reproductive medicine. Nevertheless, the reproductive medicine still faces many challenges regarding the fertility outcomes of in vitro fertilization (IVF). Many obstacles and problems can be resolved by a more detailed understanding of the UT morphology and function during normal reproduction. Over the course of the 21st century, many new insights have been obtained: the presence of a population of telocytes in the tubal wall responsible for normal motility and hormone sensory function, the demonstration of lymphatic lacunae of the mucosal folds necessary for oocyte capture and tubal fluid recirculation, or a thorough profiling of the immune makeup of the UT epithelial lining with the discovery of regulatory T cells presumably important for maternal tolerance towards the semi-allogenic embryo. New discoveries also include the notion that the UT epithelium is male sex hormone-sensitive, and that the UT is not sterile, but harbors a complex microbiome. The UT epithelial cells were also shown to be the cells-of-origin of high-grade serous ovarian carcinomas. Finally, yet importantly, several modern morphological directions have been emerging recently, including cell culture, development of tubal organoids, in silico modelling, tissue engineering and regenerative medicine. All these novel insights and new approaches can contribute to better clinical practice and successful pregnancy outcomes.

This article summarizes the importance of the exact morphology of human uterine/fallopian tube epithelium at the scanning electron microscopy (SEM) level for the clinical outcome even nowadays. Visual referential micrographs from SEM reflect two ways to view human epithelial cell lining surfaces: the surface epithelial uterine tube from surgical tissue biopsy and human fallopian tube epithelial cells (HFTEC) culture monolayer surface. One colorized image visualizes ciliated cells, distinguishes them from non-ciliated cells, and provides an educational benefit. A detailed description of the ultrastructure in referential and pathologic human uterine tube epithelium is important in defining the morphological basis of high-grade carcinomas, in the mechanism of pathophysiology, and in discussing options for its prevention. Cell cultures of human fallopian tube epithelial cells offer new approaches in simulating the mechanisms of cancer genesis or may help to elucidate the genetic basis of several diagnoses. New technical approaches in SEM provide higher resolution and detailed surface images. The SEM modality is still one of the current options in diagnostics and may be useful for advancing human reproductive organ cancer research.

Cíl práce: Popis případu ektopické tubární gravidity řešeného salpingektomií s následným negativním histopatologickým nálezem. Metodika: Vlastní pozorování a literární rešerše. Závěr: I v případě správně diagnostikované tubární ektopické gravidity může být výsledek histologického vyšetření negativní.
Objective: Case description of negative histopathology result after salpingectomy due to tubal ectopic pregnancy. Methods: Own observation and review of the literature. Conclusion: A negative histopathology result could occur even in the case of correctly diagnosed and surgically treated tubal ectopic pregnancy.

OBJECTIVES: Poor survival of high-grade serous pelvic cancer is caused by a lack of effective screening measures. The detection of exfoliated cells from high-grade serous pelvic cancer, or precursor lesions, is a promising concept for earlier diagnosis. However, collecting those cells in the most efficient way while fulfilling all requirements for a screening approach is a challenge. We introduce a new catheter for uterine and tubal lavage (UtL) and the clinical evaluation of its performance. METHODS/MATERIALS: In study I, the clinical feasibility of the UtL using the new catheter was examined in 93 patients admitted for gynecologic surgery under general anesthesia. In study II, the safety of the UtL procedure was assessed. The pain during and after the UtL performed under local anesthesia was rated on a visual analog scale by 22 healthy women. RESULTS: In study I, the UtL was carried out successfully in 92 (98.9%) of 93 cases by 16 different gynecologists. It was rated as easy to perform in 84.8% of patients but as rather difficult in cancer patients (odds ratio, 5.559; 95% confidence interval, 1.434-21.546; P = 0.007). For benign conditions, dilatation before UtL was associated with menopause status (odds ratio, 4.929; 95% confidence interval, 1.439-16.884; P = 0.016). In study II, the pain during UtL was rated with a median visual analog scale score of 1.6. During a period of 4 weeks after UtL, none of the participants had to use medication or developed symptoms requiring medical attention. The UtL took 6.5 minutes on average. The amount of extracted DNA was above the lower limit for a sensitive, deep-sequencing mutation analysis in all cases. CONCLUSIONS: Our studies demonstrate that the UtL, using the new catheter, is a safe, reliable, and well-tolerated procedure, which does not require elaborate training. Therefore, UtL fulfils all prerequisites to be used in a potential screening setting.

Cíl studie: Prezentujeme případ peritoneální tuberkulózy u mladé pacientky a poukazujeme na obtíže diagnostiky tohoto onemocnění v prostředí jeho nízkého výskytu ve vyspělých zemích. Vlastní pozorování: Peritoneální tuberkulóza je neobvyklým projevem extrapulmonální infekce způsobené Mycobacterium tuberculosis. Diagnóza bývá často obtížná pro její nespecifické klinické, laboratorní i radiologické nálezy a onemocnění může být zaměněno za ovariální malignitu. Prezentujeme případ 28leté ženy s ascitem, hraniční velikostí lymfatických uzlin, elevací Ca 125, minimálním nálezem na plicích a nejasným nálezem na PET-CT v oblasti ovarií. Diagnóza tuberkulózy byla v našem případě stanovena na základě laparoskopického nálezu a kultivace Mycobacterium tuberculosis. Závěr: V diferenciální diagnóze ascitu s elevací Ca 125 je třeba zvažovat možnost TBC infekce, navzdory její nízké incidenci ve vyspělých zemích, a podle toho upravit management a léčbu.
Objective: We present a case of peritoneal tuberculosis of young woman with focus on difficult diagnosis of this low incidence disease in developed countries. Design: Case report. Department of Micobiology, Faculty Hospital, Olomouc. Case report: Peritoneal tuberculosis is an uncommon site of extrapulmonary infection caused by Mycobacterium tuberculosis. Diagnosis is often difficult and because of its nonspecific clinical, laboratory and radiological findings, the disease may be mistaken as ovarian malignancy. We present a case of 28 years old woman with ascites, enlarged lymphonodes, elevated Ca 125 and unusuall adnexal masses on PET/CT in ovarian locality. The diagnosis of tuberculosis in our case was made by laparoscopy and cultivation of Mycobacterium tuberculosis. Conclusion: In women with ascites and Ca 125 elevation, the posibility of TB infection should be, despite the low incidence of this disease in developed countries, still considered.

Těhotenství neznámé lokalizace (PUL – pregnancy of unknown location) je termín používaný pro rozdělení těhotenství u žen s pozitivním těhotenským testem, u kterých se těhotenství nepodařilo prokázat vaginálním ultrazvukem. Až 5–42 % žen s pozitivním těhotenským testem, které jsou vyšetřeny ultrazvukem, mají PUL. K určení viability těhotenství se stanovují hladiny sérového lidského choriového gonadotropinu (hCG) a progesteronu. Tyto hormony pomáhají vyčlenit skupinu žen s PUL s vysokým rizikem mimoděložního těhotenství, u kterých je ale jen v 6–20 % následně diagnostikováno mimoděložní těhotenství. Cílem je identifikace žen s PUL s nízkým rizikem, u kterých je možná expektace bez intervence, na rozdíl od žen s vysokým rizikem, které je potřeba sledovat častěji a bezpečně identifikovat ty, které vyžadují terapeutický zásah.
Pregnancy of unknown location (PUL) is the term used to categorise a pregnancy in women with a positive pregnancy test when no pregnancy has been visualised by transvaginal ultrasound. Between 5–20 % of women seen for ultrasound examination with a positive pregnancy test have a PUL. To determine the viability of pregnancy, serum human chorionic gonadotropin (hCG) and progesterone are used. These hormones help clinicians to indicate PUL with high risk of an ectopic pregnancy, but only 6–20 % of the patients are subsequently diagnosed with an ectopic pregnancy. The aim is identifying women with PUL, which are at low risk, where expectancy is possible unlike women at high risk, which need to be observed more frequently and identify those requiring therapeutic intervention.

Diagnosis of Borderline papillary serous tumor of the fallopian tube was comprehensively established by Zheng in 1996 supported mostly by a histological similarity to its ovarian counterpart. It is a very rare entity with eight cases published so far and the ninth case described here as a 41-year-old woman presented with non-specific lower abdominal pain, dyspareunia and dysuria. Left adnexal mass was identified and she was operated on. It turned out the tumor was attached exclusively to the left tube, with no connection to any of the surrounding structures and with histology of borderline serous tumor with non-invasive implants in the left and right ovary and visceral peritoneum. Reviewing available data on genetics of these tumors there was diploid status in one examined tumor, and in our case no mutations of KRAS, BRAF and p53 genes were found. Histomorphology remains the mainstay of diagnosis and staging operation is the mainstay of patient management. Prognosis is uncertain with a 6-year survival documented in one case.
