Neadekvátnost mozkové perfuze při řízené hypotenzi se může podílet na vzniku mozkové ischemie. Jedenácti nemocným ASA I-II podstupujících plánovanou neurochirurgickou operaci mozkového aneurysmatu byl předoperačně zaveden katétr do jugulámího bulbu ke sledování hodnot saturace krve jugulámího bulhu (Sj02), mozkové extrakce kyslíku (CEO2) a lakíátu v průběhu řízené hypotenze (ŘH). K řízené hypotenzi byl použit nitroprusid sodný, cílová hodnota středního arteriálního tlaku byla 50-55 mmHg. V hodnotách SjO2 a CEO2 nebyly rozdíly v průběhu ŘH v porovnání s hodnotami před jejím zahájením. Signifikantně vyšší hladiny laktátu z krve jugulámího bulbu byly zaznamenány po ukončení ŘH v porovnám s hodnotami před zahájením RH a vjejímprůběhu. Nebyla zjištěna korelace mezi hodnotami MAP a Sj02 v průběhu ŘH. Jugulárníoxymetrii nelze považovat za přínosnou v hodnocení adekvátnosti mozkové perfuze v průběhu řízené hypotenze u nemocných s předpokladem zachované autoregulace mozkového řečiště.
Non-adequacy of brain tissue perfusion during induced hypotension may contribute to cerebral ischemia. Jugular bulb catheter was inserted in eleven patients scheduled for neurosurgical repair of aneurysm, to allow monitoring of blood oxygen saturation in jugular bulb (SJO2), cerebral oxygen lactate levels during induced hypotension. Sodium nitroprusside was used to induced hypotension with target MAP 50 - 55 mmHg. During induced hypotension there were no differences in SJO2 and CEO2 values compared to .similar values prior to induction of hypotension. Significantly higher lactate levels from jugular bulb were obtained after the re.storationfrom induced hypotension, compared to values prior to and during induced hypotension. There was not found a correlation between MAP levels and SJO2 values during induced hypotension. Jugular bulb oximetry can not be consider beneficial in assessing the adequacy of cerebral perfusion during induced hypotension in patients with preserved autoregulation of cerebral circulation.
Autoři podávají retrospektivní analýzu léčby osmi intrakraniálních aneuryzmat a tří aneuryzmat extrakraniáhiího úseku arteria carotis interna endovaskulární cestou. U šesti intrakraniálních aneuryzmat došlo v minulosti k subarachnoidálnímu krvácení. Léčebné výkony se sestávaly z uzávěru niateřské tepny odpoutatelným balónkem u čtyř aneuryzmat (z toho 2krát peroperačně) a z endovaskulami výplně vaku aneuryzmat mechanicky odpoutatelnými spirálami. U jednoho aneuryzmatu původně indikovaného k endovaskulární výplni spbálami došlo k rupture během angiografického vstřiku se vznikem hematocefalu. Ve snaze snížit krvácení byla mateřská tepna uzavřena balonkem, který byl během okamžité operace odpoután. Nemocná je bez paréz, s lehkými fatickými poruchami. Nemocní po endosakulámí léčbě jsou pravidelně sledováni.
The authors present a retrospective analysis of the treatment of eight intracranial aneurysms and three aneurysms of the extracranial portion of the internal carotid artery by the endovascular route. In six intracranial aneurysms subarachnoid haemorrhage developed in the past. The therapeutic procedures included occlusion of the parent artery in four aneurysms with a detachable balloon (inc. twice during operation) and filling the endovascular of the sac of the aneurysms by mechanically detachable spirals. In one aneurysm originally indicated for endovascular filling with spirals a rupture occurred during the angiographic injection and haematocephalus developed. In order to reduce the haemorrhage the parent artery was occluded by means of a balloon which was detached during the urgent operation. The patient has no pareses and has slight phatic disorders. Patients are regularly checked after endosaccular treatment.

Autoři analyzují komplikace 85 endovaskulárních výkonů provedených u 52 nemocných. V souboru bylo zahrnuto 23 nemocných s piální arteriovenózní malformací (AVM), 7 nemocných s karotickými aneuryzmaty, 11 nemocných s přímými karotidokavernózními píštělemi a 11 nemocných s durálními AVM. Byly zaznamenány 4 vážné komplikace (7,7% komplikací vztažených k počtu nemocných). Jedna nemocná zemřela a 1 nemocná měla závažný neurologický deficit. Dva další nemocní, jedna s rupturou aneuryzmatu a další s krvácením z AVM, byli úspěšně doléčeni. Dále bylo zaznamenáno 8 (15%) nezávažných komplikací a u 3 nemocných (6%) embolizace nebyla provedena pro technické komplikace. Funkční testy (Brietalový test, balonkový uzávěr karotidy) a EEG sledovaní mohou předejít ischemickým komplikacím.
Complications of endovascular treatment of 52 patients in 85 procedures were analysed. Twenty three patient s with pial arteriovenous malformations (AVM), 7 patients with carotid aneurysms, 11 patients with direct carotid cavernous fistulas and 11 patient s with dural AVM were involved in the study. There were 4 major complications (7,7% complications rate per patient). One patient died and one patient had severe neurolgic deficit. Two other patients, one with aneurysm rupture and one with AVM hemorrhage were successfuly treated with good result. There were 8 (15%) minor complications and in 3 patients (6%) the therapeutic embolizations were not performed because of technical complications. Functional testing (Brietal test, balloon occlusion test) and EEG monitoring can prevent ischemic complications.

Autoři shrnují své první zkušenosti s endovaskulární embolizací mozkových arteriovenózních malforniaci u 32 nemocných léčených v období 2 let a 8 měsíců. Všechny arteriovenózní malformace až na jednu byly uloženy supratentoriálně, 62,5 % malformací se projevilo krvácením, 37,5 % bylo ve stupni III podle Spetzlera a Martina, 37,5 % ve stupni TV, 15,6 % ve stupni V, 6,25 % ve stupni II a 3,1 % ve stupni I. Průměrný objem malformace byl naměřen 17,58 cm3 a embolizací byl v průměru redukován na 5,33 cm3. U 9,4 % nemocných bylo dosaženo úplného uzávěru malformace. U 6,25 % nemocných byla embolizace komplikována krvácením. Neurologicky dočasný i trvalý deficit byl zaznamenán u 9,4 % nemocných. 68,8 % nemocných bylo po embolizací indikováno nebo již léčeno radiochirurgicky. Embolizační léčba se tedy nejčastěji uplatňuje při zmenšení nidu malformace, což zlepší vyhlídky na úspěšnou radiochirurgickou nebo neurochirurgickou léčbu rozsáhlých lézí.
The authors summarize their initial experience with endovascular embolization of arteriovenous malformations in 32 patients treated during a period of two years and eight months. All arteriovenous malformations except one were supratentorial, 62.5 % of the malformations were manifested by haemorrhage, 37.5 % were grade III according to Spetzler and Martin, 37.5 % grade IV, 15.6 % grade V, 6.25 % grade II and 3.1 % grade I. The mean volume of the malformations was 17.58 ml and by embolization it was reduced on average to 5.33 % ml. In 9.4 % of the patients complete occlusion of the malformation was achieved. In 6.25 % of the patients emboUzation was compUcatedby haemorrhage. A temporary or permanent neurological deficit was recorded in 9.4 % of the patients. 68.8 % of the patients were indicated after embolization for radiosurgery or were already treated by radiosurgery. Embolization treatment is thus used most frequently to reduce the nidus of the malformation, which improves the perspectives of successful radiosurgical or neurosurgical treatment of extensive lesions.
