BACKGROUND: Intraventricular metastases (IVMs) are uncommon, and their optimal management remains debatable. OBJECTIVE: To define the safety and efficacy of stereotactic radiosurgery (SRS) in the treatment of IVMs. METHODS: This retrospective, multicenter study included patients managed with SRS for IVMs. SRS-induced adverse events, local tumor or intracranial progression, and the frequency of new-onset hydrocephalus or leptomeningeal spread were documented. Analyses of variables related to patient neuroimaging or clinical outcomes were also performed. RESULTS: The cohort included 160 patients from 11 centers who underwent SRS for treatment of 1045 intracranial metastases, of which 196 were IVMs. The median survival from SRS was 10 months. Of the 154 patients and 190 IVMs with imaging follow-up, 94 patients (61%) experienced distant intracranial disease progression and 16 IVMs (8.4%) progressed locally. The 12- and 24-month local IVM control rates were 91.4% and 86.1%, respectively. Sixteen (10%) and 27 (17.5%) patients developed hydrocephalus and leptomeningeal dissemination post-SRS, respectively. Adverse radiation effects were documented in 24 patients (15%). Eleven patients (6.9%) died because of intracranial disease progression. CONCLUSION: SRS is an effective treatment option for IVMs, with a local IVM control rate comparable with SRS for parenchymal brain metastases. Leptomeningeal spread and hydrocephalus in patients with IVM occur in a minority of patients, but these patients warrant careful follow-up to detect these changes.
BACKGROUND: Neurosurgical 3D visualizers and simulators are innovative devices capable of defining a surgical strategy in advance and possibly making neurosurgery safer by rehearsing the phases of the operation beforehand. The aim of this study is to evaluate Surgical TheaterTM (Surgical Theater LLC, Mayfield, OH, USA), a new 3D neurosurgical planning, simulation, and navigation system, and qualitatively assess its use in the operating room. METHODS: Clinical data were collected from 30 patients harboring various types of brain tumors; Surgical TheaterTM was used for the preoperative planning and intraoperative 3D navigation. Preoperative and postoperative questionnaires were completed by first and second operators to get qualitative feedback on the system's functionality. Furthermore, we measured and compared the impact of this technology on surgery duration. RESULTS: Neurosurgeons were overall satisfied when using this rehearsal and navigation tool and found it efficient and easy to use; interestingly, residents considered this device more useful as compared to their more senior colleagues (with significantly higher scores, P<0.05), possibly because of their limited anatomical experience and spatial/surgical rehearsal ability. The length of the surgical procedure was not affected by this technology (P>0.05). CONCLUSIONS: Surgical TheaterTM system was found to be clinically useful in improving anatomical understanding, surgical planning, and intraoperative navigation, especially for younger and less experienced neurosurgeons.

Preoperative clinical MRI protocols for gliomas, brain tumors with dismal outcomes due to their infiltrative properties, still rely on conventional structural MRI, which does not deliver information on tumor genotype and is limited in the delineation of diffuse gliomas. The GliMR COST action wants to raise awareness about the state of the art of advanced MRI techniques in gliomas and their possible clinical translation. This review describes current methods, limits, and applications of advanced MRI for the preoperative assessment of glioma, summarizing the level of clinical validation of different techniques. In this second part, we review magnetic resonance spectroscopy (MRS), chemical exchange saturation transfer (CEST), susceptibility-weighted imaging (SWI), MRI-PET, MR elastography (MRE), and MR-based radiomics applications. The first part of this review addresses dynamic susceptibility contrast (DSC) and dynamic contrast-enhanced (DCE) MRI, arterial spin labeling (ASL), diffusion-weighted MRI, vessel imaging, and magnetic resonance fingerprinting (MRF). Pacienti s metastatickým NSCLC, u kterých nebyly prokázány řídící mutace, jsou v první linii léčeni imunoterapií v monoterapii nebo v kombinaci s chemoterapií. Studie CheckMate 9LA prokázala účinnost a bezpečnost duální imunoterapie (nivolumab, ipilimumab) s limitovanou chemoterapií. Ze zmiňované kombinace mají výrazný přínos i nemocní s negativní PD-L1 expresí a vstupně detekovanými mozkovými metastázami. Naše kazuistika popisuje případ 72letého pacienta s NSCLC a mozečkovou metastázou, který je léčen režimem CheckMate 9LA.
Patients with non-oncogene-addicted metastatic NSCLC are treated with first- line immunotherapy with or without chemotherapy. CheckMate 9LA trial demonstrated survival benefits and safety of dual immunotherapy (nivolumab, ipilimumab) with limited chemotherapy. Survival outcomes were significantly improved in patients with tumor PD-L1 expression < 1% and baseline brain metastases. We present a clinical case of a 72 years old patient with NSCLC and cerebellar metastasis treated according to CheckMate 9LA trial.

Cíl: Cílem této práce bylo vyhodnocení bezpečnosti supracerebellárního transtentoriálního přístupu na našem souboru pacientů. Jedná se o technicky náročný přístup, který jsme využili při operacích zadní a střední mediotemporální oblasti, laterálního mezencefala a posteromediálního talamu. Soubor a metodika: Jedná se vyhodnocení souboru našich 8 pacientů operovaných supracerebellárním transtentoriálním přístupem v letech 2013–2021. V jednom případě se jednalo o anaplastický astrocytom, 3× o primooperaci glioblastoma multiforme, 1× difuzního středočárového H3K27M gliomu, 1× o operaci recidivy glioblastoma multiforme, 1× o operaci nízkostupňového gliomu a jednou jsme tímto přístupem operovali akutně prokrvácený kavernom lemniskálního trigona mesencefali. Všichni naši pacienti byli operováni v poloze v polosedě. Výsledky: 30denní mortalita v našem souboru je nulová. U pacientky s difuzním středočárovým H3K27M gliomem primárně operované v neurologicky těžkém stavu jsme museli provést časnou reoperaci stejným přístupem z důvodu prokrvácení rezidua tumoru. U našeho prvního pacienta s rozsáhlým mediotemporálním anaplastickým astrocytomem jsme museli po první době časně doplnit subokcipitální přístup ke zvýšení radikality resekce, po operaci měl horní kvadrantanopsii. U jednoho pacienta došlo k ischemii okcipitálního laloku z důvodu peroperačně pozorovatelné léze arteria cerebri posterior obklopené glioblastomem. Klinicky byla po operaci přítomna hemianopsie. Závěr: Jedná se o technicky náročný, přitom ale o bezpečný přístup. U lézí postihujících mediotemporální oblast je výhodný pro její střední a zadní část, ale při resekci lze dosáhnout anteriorně až k uncu a amygdale. Předpokládanou podmínkou je precizní znalost anatomie, na našem pracovišti pro tento přístup preferujeme polohu v polosedě. Důležitá je dostatečná likvorová dekomprese vypuštěním moku z cisterna magna ze separátní durální incize a šetrná manipulace s cévními strukturami tentoriální incisury.
Aim: The aim of this work was a retrospective evaluation of the safety of the supracerebellar transtentorial approach in our patient series. It represents a technically challenging approach, which we used during surgeries of the posterior and medial mediotemporal area, lateral mesencephalon and posteromedial thalamus. Materials and methods: We evaluate the series of our 8 patients, which we operated on using the supracerebellar transtentorial approach during 2013–2021. In one case, we dealt with anaplastic astrocytoma, 3× with glioblastoma multiforme primary surgery, 1× with diffuse midline H3K27M glioma, 1× with glioblastoma multiforme recurrence surgery, 1× with low-grade glioma surgery and once we operated using this approach an acutely bleeding cavernoma of the lemniscal trigone of the mesencephalon. All our patients were operated on in a semisitting position. Results: The thirty-day mortality rate of our series is zero. In case of a patient with diffuse midline H3K27M glioma which was primarily operated on in a bad neurological condition, we had to perform early revision surgery using the same approach due to residual tumor hemorrhage. In case of our first patient with extensive mediotemporal anaplastic astrocytoma, we had to add the suboccipital approach for resection radicality increase early after the first phase of surgery; after the surgery, he had superior quadrantanopsia. In one patient’s case, ischemia of the occipital lobe occurred due to an intraoperatively visible lesion of the posterior cerebral artery inside the glioblastoma. After surgery, hemianopsia was present. Conclusion: The approach poses a technically challenging, but concurrently safe surgical trajectory. In case of the lesions affecting the mediotemporal area, it is advantageous for its medial and posterior part, but during the resection it is possible to reach as far as the uncus and amygdala. The prerequisite condition is accurate anatomical knowledge; in our department, we prefer the semisitting position for this approach. It is important to have a sufficient cerebrospinal fluid decompression by releasing the fluid from the cisterna magna from a separate dural incision and a gentle manipulation of the vascular structures of the tentorial incisura.

Úvod a cíle: Mezi nově zaváděné postupy u operací nádorů mozku patří tzv. awake kraniotomie. Jedná se o operaci mozku při vědomí, kdy jsou monitorovány řečové a jazykové funkce logopedem za aktivní účasti pacienta. Cílem takto vedené operace je zachování řečových a jazykových funkcí pacienta a zároveň co největší možný rozsah resekce nádoru mozku. V příspěvku jsou shrnuty dosavadní zkušenosti a poznatky ve FN Ostrava. Metodika: Retrospektivní hodnocení úspěšnosti provedených awake kraniotomií s exstirpacemi nádorů mozku v elokventních oblastech mozku. Kritériem úspěšnosti awake kraniotomie bylo zachování řečových a jazykových schopností pacientů po operaci ve srovnání s jejich stavem před operací. Popis postupů, předoperačních vyšetření a intraoperačního testování klinickým logopedem. Výsledky: Porovnáním výsledků testování řečových a jazykových funkcí u 54 pacientů, operovaných v letech 2013–2021, před awake kraniotomií a po ní bylo zjištěno, že u 40 z 54 pacientů nedošlo po operaci ke zhoršení řečových a jazykových funkcí. Z logopedického hlediska se tak jedná o 74% úspěšnost. Závěr: Awake kraniotomie je novým postupem zaváděným při operacích gliomů mozku v elokventních řečových zónách. Metoda přispívá značnou měrou k zachování řečových a jazykových kompetencí pacienta a tím i k zachování kvality jeho života.
Introduction and aims: An awake craniotomy is one of the newly introduced procedures for brain tumor surgery. It is a brain operation where speech and language functions are monitored by a clinical speech therapist with the active participation of the patient who is awake during the procedure. The aim of such an operation is to preserve the speech and language functions of the patient as well as resection of the brain tumor to the greatest possible extent. This paper summarizes the current experience and knowledge of awake craniotomy at the University Hospital in Ostrava. Methodology: Retrospective evaluation of the success of performed awake craniotomies with extirpations of tumors located in eloquent regions of the brain. The criterion for the success of awake craniotomy was the preservation of patient’s speech and language skills after surgery compared to their condition before surgery. Description of procedures, preoperative examinations, and intraoperative testing by a clinical speech therapist. Results: By comparing the results of testing speech and language functions in a group of 54 patients operated between 2013 and 2021 before and after awake craniotomy, it was found that in 40 of 54 patients there was no deterioration of speech and language functions after surgery. From a speech therapy perspective, this is a 74% success rate. Conclusion: Awake craniotomy is a new procedure introduced during brain glioma operations in eloquent regions of the speech zones. The method contributes significantly to the preservation of the patient’s speech and language competences and thus to the preservation of his quality of life.
