OBJECTIVES: To evaluate risk factors, diagnostic procedures, and treatment outcomes of invasive aspergillosis (IA) in patients with hematological malignancies. METHODS: A retrospective analysis of data from proven/probable IA cases that occurred from 2005 to 2009 at 10 hematology centers was performed. RESULTS: We identified 176 IA cases that mainly occurred in patients with acute leukemias (58.5%), mostly those on induction/re-induction treatments (39.8%). Prolonged neutropenia was the most frequent risk factor for IA (61.4%). The lungs were the most frequently affected site (93.8%) and computed tomography detected abnormalities in all episodes; however, only 53.7% of patients had findings suggestive of IA. Galactomannan (GM) detection in serum or bronchoalveolar lavage fluid (positive in 79.1% and 78.8% of episodes, respectively) played a crucial role in IA diagnosis. Neutrophil count and antifungal prophylaxis did not influence the GM positivity rate, but empirical therapy decreased this rate (in serum). Of the IA cases, 53.2% responded to initial antifungal therapy. The combination of voriconazole and echinocandin, even as initial or salvage therapy, did not perform better than voriconazole monotherapy (p=0.924 for initial therapy and p=0.205 for salvage therapy). Neutrophil recovery had a significant role in the response to initial (but not salvage) antifungal therapy. CONCLUSIONS: Our retrospective analysis identified key diagnostic and treatment characteristics, and this understanding could improve the management of hematological malignancy patients with IA.

Invazivní mykotická onemocnění patří k závažným život ohrožujícím stavům imunokompromitovaných pacientů. Zavedení antimykotika Amphotericinu B (AmB), respektive jeho různých lékových forem, do klinické praxe vedlo k výraznému zlepšení léčebných výsledků a ke snížení mortality imunokompromitovaných pacientů s prokázanou nebo předpokládanou invazivní mykózou (1). Limitací zůstává jeho toxicita – akutní a opožděná nefrotoxicita a akutní s infuzí spojená nežádoucí reakce (Infusion – related reaction – IRR). Nefrotoxicita je nyní ovlivnitelná volbou vhodné lékové formy AmB a adekvátní podpůrnou péčí, především hyperhydratací (2). IRR v podobě pyrexie, třesavky, zimnice však zatím zcela odstranitelná není a ani zavedení premedikace s výjimkou kortikoidů příliš neovlivnilo incidenci IRR (3). Vzhledem k nejasné patogenezi IRR a jen ojedinělým systematickým prospektivním studiím, vedly snahy o minimalizaci IRR k prospektivní studii u pediatrických onkologických pacientů s indikovaným podáním AmB. Flowcytometrické stanovení subpopulací lymfocytů a stanovení hladin interleukinů v korelaci s plazmatickými hladinami AmB, s klinickou incidencí IRR, lékovou formou AmB a s typem premedikace svědčí pro intimní vztah změny v množství paměťových efektorových Th lymfocytů a v interleukinovém spektru a incidencí IRR.
The invasive mycotic infections belong to serious life- threatening diseases. The therapy using different forms of Amphotericin B (AmB) improved treatment outcomes for immunocompromised patients with invasive mycosis (1). Recently the mortality for invasive mycosis is lower then it was before AmB introduction (1), however the acute and even the late nephrotoxicity of AmB as well as the Infusion- related reaction (IRR) limit its use in clinical practice. The choice of AmB´s form and the adequate supportive care (hyperhydratation) can minimize the AmB´s nephrotoxicity (2). There was an endeavor to pre-medicate patients to minimize fevers, chills and rigor as IRR´s most common symptoms. But there were almost no successful results except steroids. Only corticosteroids are known to be able to decrease IRR´s incidence (3). Because of just limited knowledge of IRR´s pathogenesis and almost no prospective studies there is a need of more clinical data. The prospective study in pediatric oncology patients treated with AmB was initiated in order to improve our understanding of IRRs. Flowcytometric measurement of lymphocytes´ subpopulations and plasma cytokines´ levels in comparison with plasmatic AmB levels, clinical incidence of IRRs, type of AmB and type of premedication point out the causal nexus among the changes of a count of Th-memory effectors, cytokine levels and the incidence of IRR.
The primary aim of the research was to find the delay between the first symptoms of an autistic disorder being recognized by parents and diagnosis in our centre. A secondary objective was to evaluate the number of contacts with professionals (physicians, teachers, and speech therapists) in which parents pointed out special manifestations seen in children and, in spite of that, the children were not referred to a specialist. A retrospective study assessed 204 children (59 girls, 145 boys) in total; 126 children (39 girls, 87 boys) with childhood autism (CHA), 57 (17 girls, 40 boys) with atypical autism (AA), and 21 (3 girls, 18 boys) with Asperger's syndrome (AS). The mean age at appearance of the first signs was 29.7 months (range 0-70, median 30+/-17.0) in N=201, and the average age at diagnosis was 81.5 months (range 13-276, median 69.5+/-45.2) in N=204. The mean delay in making a diagnosis was 51.3 months (range 0-246, median 39+/-40.9) in N=201. The delay in diagnosis is shortest in patients with AA (a mean period of 44.4 months = 3 years and 8 months), longer in CHA patients (49.5 months = 4 years and 2 months), and longest in patients with AS (80.8 months = 6 years and 9 months). A statistically significant difference in the period to diagnosis was found between CHA and AS patients (p=0.023) and between AA and AS patients (p=0.019). The mean number of visits to physicians and other specialists before referring to a specialized centre for diagnosis in N=133 was 2.4 (range 1-5, median 2+/-0.9). The diagnosis of autism is made late and early educational and behavioural interventions cannot be initiated.
