Fabry disease (FD) is a lysosomal storage disorder caused by pathogenic mutations in the alpha-galactosidase A (AGALA) encoding gene region. This rare disease affects several organs including the cochlea-vestibular system. Tinnitus and sensorineural hearing loss (SNHL) are reported among otoneurological symptoms. Early and correct diagnosis of FD is important with a view to available therapy. The aim of the study was to screen for alpha-galactosidase deficiency in men with tinnitus/SNHL. A prospective multicentric study including consecutive patients with SNHL confirmed by tone audiometry or tinnitus evaluated (10/2016-8/2019). The diagnosis of AGALA deficiency was done by dry blood spot method using a threshold of 1.2 µmol/l/h. Only men aged 18-60 were included. 181 patients were subject to evaluation. SNHL was reported in 126 (70%) patients, 50 (28%) patients had unilateral, 76 (42%) patients had bilateral SNHL. Tinnitus was found in 161 (89%) patients, unilateral in 96 (53%) and bilateral in 65 (36%) patients. Suspected FD was not detected in any patient; alpha-galactosidase The AGALA values ranged 1.5-8.8 µmol/l/h, an average of 3.4 µmol/l/h. None of the 181 patients participating in the study had AGALA levels below the threshold 1.2 µmol/l/h. The occurrence of tinnitus and sensorineural hearing loss in men appears to be an irrelevant clinical sign for FD systematic screening.
PURPOSE OF THE STUDY The incidence of isolated orbital floor fractures has an increasing tendency. Their optimal management is not uniform and is still being discussed in the literature. The therapeutic decision as to whether surgical intervention is necessary or conservative approach is adequate vitally depends on clinical and CT findings. Incorrect treatment can lead to serious consequences, especially to persistent diplopia and enophthalmos. The objective of our study was to evaluate the radiological indication criteria for surgery and the clinical outcomes thereafter. MATERIAL AND METHODS The retrospective monocentric study of the group of 53 patients who underwent the isolated orbital floor fracture reconstruction during the period from 1/1/2006 to 31/12/2016 at the Clinic of Otolaryngology and Head and Neck Surgery of the St. Anne's University Hospital, Brno. The ealuated parameters wee the following: trauma cause, clinical symptoms, evaluation of CT parameters (MH index, RF index, MRI index), time interval from injury to surgery, complications. RESULTS Trauma cause: an assault 30/53 (57%), a fall 14/53 (26%), sports 4/53 (7%), a road traffic accident 4/53 (7%), an accident at work 1/53 (2%). Clinical symptoms: eyelid haematoma and/or swelling 53/53 (100%), diplopia 29/53 (55%), emphysema 29/53 (55%), infraorbital nerve hypoesthesia 4/53 (7%). Radiological report of the CT: RF index > 50% (defect length more than a half of the orbital floor length) 49/53 (92%), RF index <50% (defect length less than a half of the orbital floor length) 4/53 (7%). MH index (maximum height of periorbital herniation): mean value 9.0 mm (2.8-14.2 mm), MRI index (rectus inferior muscle index): <1.5 15/53 (28%), ≥ 1.5 38/53 (72%). Time interval from injury to surgery: mean value 11 days (3-21 days). Complications 6 weeks postoperatively: diplopia 4/53 (7%), ectropion 2/53 (4%), enophtalmos 0/53 (0%), visual damage 0/53 (0%). CONCLUSIONS The choice between the surgical and conservative management of the isolated orbital fracture is the key factor to ensure a good therapeutic result. The evaluation of CT findings is crucial for the decision-making process. The key radiological parameters are the standardized assessment of the orbital floor defect size (RF index), orbital tissue herniation (MH index) and the assessment of damage to the intraorbital muscles (MRI index). As demonstrated by the results of our analysis, surgical reconstruction of the orbital floor by nasoseptal cartilage represents a highly effective and safe method. Bolest ucha (otalgia) patří k častým symptomům, které vedou k nutnosti návštěvy lékaře. Nejčastější příčinou jsou zánětlivé procesy v oblasti zevního a středního ucha, na které je článek zaměřen, je ale nutno v rámci diferenciální diagnostiky pomýšlet i na možnost bolesti propagované do ucha při onemocnění v oblasti dutiny ústní, hltanu, čelistního kloubu i krční páteře. Etiopatogeneze, rozdělení a klinická manifestace zánětu zevního zvukovodu (otitis externa) je diskutována v první části článku. Popsány jsou jednotlivé formy zánětu, typické subjektivní potíže nemocných i možné rizikové faktory. V rámci terapeutické rozvahy jsou uvedeny možnosti léčby zahrnující lokálně působící i celkově podávané preparáty. Akutní zánět středního ucha (otitis media acuta) je onemocněním typickým pro dětský věk, nicméně se setkáváme s touto problematikou i u dospělých pacientů. Klinické symptomy kromě bolesti zahrnují i různě závažnou poruchu sluchu, příznaky celkové a pominout nelze i varovné příznaky možných komplikací při šíření zánětlivého procesu do oblasti vnitřního ucha nebo intrakraniálně. Diskutována je otázka úlohy praktického lékaře v diagnostice a léčbě zánětu středouší a situace, při nichž je nezbytné vyšetření a léčba ORL specialistou.
Ear pain (earache, otalgia) in one of the common symptoms reffering patients to the ENT specialist. This article deals with earache,as a frequent cause of a GP visit. Although inflammations of the external auditory canal and middle ear are the most commoncauses of otalgia, other etiology including dental, oral and pharyngeal problems must be excluded. Also cervical spine and temporomandibularjoint affections presenting as ear pain (otalgy) can lead to otalgia. The first part of the article focuses on etiology,subjective symptoms and possible forms of acute inflammation of the external auditory canal (otitis externa). Possible risk factorsare also discussed (diabetes, immunodeficiency). Therapeutic approach includes especially local treatment (local antibiotics andcorticosteroids) and systemic medication in the severe course of the disease. Midle ear inflammations (otitis media) is typical forpaediatric population but it can be diagnosed also in adults. It can be accompanied with hearing loss, fever and other systemicsymptoms. Warning signs including peripheral vestibular syndrome (dizziness, nystagmus), haedache and other neurologicalsymptomatology can indicate severe complications when inflammation spreads to the inner ear and intracranial structures. Therole of the GPs and ENT specialist in diagnosis and therapy of the ottic inflammations are discussed.

Úvod: Syndrom Freyové (aurikulotemporální syndrom) je častá komplikace chirurgie příušní žlázy, který je způsoben aberantním prorůstáním parasympatických vláken nervus auriculotemporalis do kožních potních žlázek. Typicky je syndrom charakterizován pocením, zarudnutím a pálením kůže parotické oblasti, zejména v návaznosti na chuťové podněty. Vytvoření interpoziční bariéry mezi kůži a lůžko po parotidektomii může vést ke snížení rizika této komplikace. Cíl: Cílem studie bylo stanovení četnosti výskytu syndromu Freyové u operovaných pacientů a zhodnocení účinnosti použití svalového laloku z musculus sternocleidomastoideus (m. SCM) v jeho prevenci. Soubor a metodika: Jde o retrospektivní studii. Soubor 167 pacientů, u kterých byla provedena parciální či totální parotidektomie v letech 2007–2011, byl rozdělen do dvou skupin. U první skupiny pacientů byla provedena rekonstrukce svalovým lalokem z m. SCM (n = 42), u druhé skupiny rekonstrukce provedena nebyla (n = 125). Diagnostika syndromu Freyové v pooperačním období byla stanovena na základě subjektivního hodnocení symptomů, údajů z dotazníku a objektivním průkazem Minorovým testem. Výsledky: Celkový výskyt syndromu Freyové činil 15 % (25/167 pacientů), ve skupině bez rekonstrukce 16 % (22/125), ve skupině s rekonstrukcí 7 % (3/42). Rozdíl mezi skupinami ve výskytu aurikulotemporálního syndromu byl dle Fisherova exaktního testu statisticky významný (p < 0,05). Závěr: Svalový lalok z m. SCM použitý k vytvoření interpoziční bariéry mezi kožní lalok a resekční lůžko po parotidektomii představuje jednoduchou, rychlou a efektivní metodu v prevenci syndromu Freyové po parotidektomii.
Background: Frey´s syndrome (auricotemporal syndrome) is frequent sequelae of parotid gland surgery caused by inappropriate regrowth of parasympathetic fibres of the auriculotemporal nerve into sweat glands of the skin. Typically, the syndrome is characterized by sweating, erythema and flushing of the skin overlying the parotid region, especially in response to taste stimuli. Placement of an interpositional barrier between the skin and the parotid gland can prevent these complications. Aim: The purpose of this study was to evaluate the incidence of Frey´s syndrome and the impact of using sternocleidomastoid muscle (SCM) flap on this syndrome. Material and methods: In a retrospective study, a series of 167 patients who underwent partial or total parotidectomy between January 2007 and December 2011 were divided into two groups. One group had an SCM flap reconstruction (n = 42), and the other group did not (n = 125). A subjective clinical evaluation, a questionnaire and the objective Minor´s test were used to diagnose the syndrome post-surgery. Results: The overall incidence of Frey´s syndrome was 15% (25/167 patients), 16% (22/125) in the non-SCM flap group and 7% (3/42) in the SCM flap group, respectively. There was a statistically significant difference between the two groups according to the Fisher´s frequency exact test (p < 0.05). Conclusion: The SCM flap, used as an interposing barrier between the overlying skin and the parotid bed following parotidectomy, is a simple, fast and efficient method for preventing Frey´s syndrome following parotidectomy.
