Súhrn: Cieľ práce: Predstaviť prípad akútnej hemoragickej cievnej mozgovej príhody počas III. trimestra gravidity a popísať úspešný manažment s porodením zdravého dieťaťa. Kazuistika: Hemoragická cievna mozgová príhoda (HCMP) v gravidite je ojedinelý, ale vážny stav, ktorý sa podieľa na vysokej materskej aj fetálnej mortalite a morbidite. Včasnou a rýchlou diagnostikou môžeme zlepšiť prognózu rodičky a priaznivo ovplyvniť rozsah neurologickej dysfunkcie. Naša kazuistika popisuje prípad tehotnej pacientky v 37. týždni gravidity s bolesťami hlavy a celkovou alteráciou stavu, ktorej bola pomocou zobrazovacích metód diagnostikovaná HCMP neznámej etiológie. Popisujeme diagnostický postup a manažment s následnou neurochirurgickou intervenciou. Gravidita bola ukončená akútnym cisárskym rezom v 37. týždni gravidity. Záver: Výskyt hemoragickej cievnej mozgovej príhody v gravidite je ojedinelý. Vzhľadom na to, že nie sú dostupné jednoznačné doporučené postupy na ukončenie tehotnosti, každý prípad posudzujeme individuálne.
Objective: To present a case of acute haemorrhagic stroke during 3rd trimester of pregnancy and to describe management and successful delivery of healthy baby. Case report: Haemorrhagic stroke is responsible for significant morbidity and mortality. Prognosis can be improved only by urgent diagnosis and care. We report a case of pregnant woman at 37th week of pregnancy with acute haemorrhagic stroke of unknown etiology with clinical appearance of thunderclap headaches and overall disorientation. We describe diagnostic approach and a successful management followed by further differential diagnosis and treatment. The foetus was delivered by acute caesarean section at 37th week of pregnancy. Conclusion: Occurrence of haemorrhagic stroke in pregnancy is rare. There are no specific guidelines that recommend the time and mode of delivery; therefore, each case is assessed individually.
Idiopatická granulomatózna mastitída postihuje predovšetkým ženy vo fertilnom veku, najčastejšie niekoľko mesiacov až rokov po ukončení dojčenia. Etiopatogenéza je do dnešného dňa neznáma. Pretože klinické prejavy môžu napodobňovať infekčné mastitídy alebo inflamatórny karcinóm, diagnostický proces býva často zdĺhavý. Správna diagnóza sa určuje až na základe histopatologického nálezu. Doteraz neexistuje jednotný konsenzus na liečbu idiopatickej granulomatóznej mastitídy. Chronická povaha ochorenia ako aj tendencia ku rekurenciám napriek benígnej povahe ochorenia spôsobuje, že zasahuje do kvality života pacientok. Na nepriaznivý vplyv na život ženy poukazujú aj kazuistiky z Gynekologicko-pôrodníckej kliniky FN v Trenčíne.
Idiopathic granulomatous mastitis mainly affects women of childbearing age, mostly several months to years after breastfeeding. The etiopathogenesis is unknown to this day. Because clinical manifestations can mimic infectious mastitis or inflammatory carcinoma, the diagnostic process is often prolonged. The correct diagnosis is determined only on the basis of histopathological findings. To this day, there is no uniform consensus on the treatment of idiopathic granulomatous mastitis. The chronic nature of the disease as well as the tendency to recurrences despite the benign nature of the disease affects the quality of life of patients. Case reports from the Gynecoly and Obstetrics Clinic in University Hospital in Trenčín also point to an adverse effect on women’s lives.

Cieľ štúdie: Analýza frekvencie cisárskeho rezu, vaginálneho inštrumentálneho pôrodu a závažnej perineálnej morbidity v Slovenskej republike v rokoch 2007–2018. Metodika: Analýza prospektívne zbieraných dát o cisárskych rezoch, forcepse, vákuumextrakcii v období rokov 2007–2018 a o ruptúrach hrádze 3. a 4. stupňa a epiziotómiách v období rokov 2008–2018 z pôrodníc v Slovenskej republike. Výsledky: Frekvencia cisárskeho rezu stúpla z 24,1 % v roku 2007 na 30,8 % v roku 2013 a do roku 2018 klesla na 29,6 %. Frekvencia vákuumextrakcie stúpla od roku 2007 do roku 2018 z 1,3 % na 2,0 %. Frekvencia forcepsu klesla od roku 2007 do roku 2018 z 0,56 % na 0,43 %. V rokoch 2008–2018 frekvencia ruptúr hrádze 3. a 4. stupňa stúpla z 0,4 % na 0,8 % a frekvencia epiziotómií klesla zo 74,7 % na 47,7 %. Záver: Frekvencia cisárskeho rezu v Slovenskej republike bola najvyššia v roku 2013 (30,8 %), ale v nasledujúcich rokoch pozvoľna klesala. Frekvencia vákuumextrakcie stúpla a forcepsu klesla. Frekvencia epiziotómií mala klesajúci a ruptúr hrádze 3. a 4. stupňa stúpajúci trend.
Objective: Analysis of caesarean section, vaginal instrumental deliveries and severe perineal morbidity in the Slovak Republic in the years 2007–2018. Methods: The analysis of prospectively collected caesarean section and vaginal instrumental delivery data in the years 2007–2018, and episiotomies and severe perineal morbidity data in the years 2008–2018 from obstetrics hospitals in the Slovak Republic. Results: Caesarean section rate progressively increased from 24.1% in 2007 up to 30.8% in 2013 and decreased to 29.6% in 2018. Vacuum-extraction frequency was 1.3% in 2007 and increased up to 2.0% till 2018. Forceps frequency decreased since 2008–2018 from 0.56% to 0.43%. In the years 2008–2018, frequency of perineal tears of the 3rd and 4th degree increased from 0.4% to 0.8%. Frequency of episiotomies decreased in the years 2008–2018 from 74.7% to 47.7%. Conclusion: The highest caesarean section rate in the Slovak Republic – 30.8% occurred in 2013, but slowly decreased in the following years. The frequency of vacuum extraction increased and forceps decreased. Frequency of episiotomies had decreased and severe perineal tears held an increasing trend.

Cieľ štúdie: Analýza prípadov materských úmrtí v Slovenskej republike v rokoch 2007–2018. Metodika: Analýza vybraných ukazovateľov materskej mortality v období rokov 2007–2018. Výsledky: V rokoch 2007–2018 umrelo v tehotnosti a do 42 dní po jej skončení 84 žien – index materskej úmrtnosti bol 12,57 na 100 000 živonarodených novorodencov. Priama materská úmrtnosť bola 4,64 (umrelo 31 matiek), nepriama materská úmrtnosť 6,74 (umrelo 45 matiek) a očistená materská úmrtnosť 11,38. Náhodná materská úmrtnosť bola 1,19 na 100 000 živonarodených novorodencov (umrelo osem matiek). Porovnanie dát Slovenskej gynekologicko-pôrodníckej spoločnosti odhalilo 64 % podhodnotenie dát o materskej mortalite z rutinných štatistík. Záver: Materská úmrtnosť v Slovenskej republike bola naďalej jedna z najvyšších v Európskej únii a nekoreluje s dobrou úrovňou perinatálnej úmrtnosti. V Slovenskej republike treba hľadať cesty k znižovaniu materskej mortality a zlepšeniu zberu dát.
Objective: Analysis of maternal mortality in the Slovak Republic in the years 2007–2018. Methods: The analysis of selected maternal mortality data in the years 2007–2018. Results: In the years 2007–2018, 84 women died during pregnancy or within 42 days of the end of pregnancy – maternal mortality ratio was 12.57 per 100,000 live births. Direct maternal mortality ratio in this period was 4.64 (31 maternal deaths), indirect maternal mortality 6.74 (45 maternal deaths) and direct obstetrics death ratio was 11.38 per 100,000 live births. Pregnancy-associated but unrelated maternal death ratio was 1.19 (eight maternal deaths). The Slovak Gynecological and Obstetric Society revealed a 64% underestimation of maternal mortality data in comparison with routine statistical data. Conclusion: Maternal mortality ratio in the Slovak Republic was one of the highest in the European Union and was discordant with a good level of perinatal mortality. In the Slovak Republic, it is necessary to look for ways to reduce maternal mortality and improve data collection.

Cieľ štúdie: Analýza materskej morbidity v Slovenskej republike v rokoch 2012–2018. Metodika: Analýza vybraných ukazovateľov materskej morbidity v období rokov 2012–2018 prospektívne zbieraných zo všetkých pôrodníc v Slovenskej republike. Výsledky: V rokoch 2012–2018 bola incidencia závažného peripartálneho krvácania 2,17; incidencia peripartálnych hysterektómií 0,89; transportu matky na jednotky intenzívnej starostlivosti 1,59; eklampsie 0,21; HELLP syndrómu 0,73; abnormálne invazívnej placentácie 0,37; ruptúry uteru 0,68; závažnej sepsy v gravidite a puerpériu 0,18 a nonfatálnej embólie plodovou vodou 0,027 na 1 000 pôrodov. Záver: Incidencia celkovej závažnej akútnej materskej morbidity v Slovenskej republike bola 6,84 na 1 000 pôrodov. V slovenských podmienkach je ešte stále priestor na znižovanie závažnej akútnej materskej morbidity.
Objective: Analysis of maternal morbidity in the Slovak Republic in the years 2012–2018. Methods: The analysis of selected maternal morbidity data prospectively collected in the years 2012–2018 from all obstetric units in the Slovak Republic. Results: In the years 2012–2018, incidence of severe peripartum bleeding was 2.17, peripartum hysterectomy was 0.89, maternal admission to intensive care units was 1.59, eclampsia was 0.21, HELLP syndrome was 0.73, abnormally invasive placentation was 0.37, uterine rupture was 0.68, severe sepsis in pregnancy and puerperium was 0.18 and nonfatal amniotic fluid embolism was 0.027 per 1,000 births. Conclusion: Incidence of total severe acute maternal morbidity in the Slovak Republic was 6.84 per 1,000 births. In Slovak local conditions, there is still room for reduction of severe acute maternal morbidity.

Výsledky: V roku 2007 bolo v Slovenskej republike 63 pôrodníc, 51 146 pôrodov a narodilo sa 51 650 živých novorodencov. Do roku 2018 klesol počet pôrodníc na 51, počet pôrodov sa zvýšil na 57 085 a počet živonarodených novorodencov na 57 773. Úhrnná plodnosť v roku 2007–2018 stúpla z 1,27 na 1,54. Frekvencia predčasných pôrodov v roku 2007 bola 7,3 %, v roku 2010 sa zvýšila na 8,5 % a do roku 2018 klesla na 7 %. Perinatálna mortalita v roku 2007 bola 6,2 ‰, v roku 2017 klesla na 4,4 ‰, ale v roku 2018 stúpla na 5 ‰ a podľa kritérií WHO (World Health Organization) na 6,6 ‰. Na perinatálnej mortalite v rokoch 2007–2018 sa podieľala mŕtvorodenosť 65 %, nízka pôrodná hmotnosť 63 % a závažné vrodené vývojové chyby 19 %. V rokoch 2007–2018 klesol transport in utero novorodencov s hmotnosťou 1 000–1 499 g do perinatologických centier z 57 % na 56 % a novorodencov s hmotnosťou do 1 000 g zo 75 % na 73 %. Záver: Slovenská republika dosiahla historicky najlepší výsledok perinatálnej mortality v roku 2017 – 4,4 ‰, ale v ďalšom roku sa zvýšila nad 5 ‰. Na ďalší pokles perinatálnej mortality v SR treba zlepšiť prenatálnu diagnostiku závažných vrodených vývojových chýb, transport plodov s veľmi nízkou pôrodnou hmotnosťou in utero, centralizáciu vysokorizikových gravidít, personálne a materiálno-technické vybavenie pôrodníc a novorodeneckých jednotiek intenzívnej starostlivosti.
Objective: Analysis of perinatal mortality in the Slovak Republic during the years 2007–2018. Methods: Analysis of prospectively collected selected perinatal data in the years 2007–2018. Results: In the year 2007, there were 63 obstetrics units, 51,146 deliveries and that of live births 51,650 in the Slovak Republic. The number of obstetrics units decreased to 51 in the years 2018, the total number of deliveries increased to 57,085 and that of live births increased to 57,773. The total fertility rate in the years 2007–2018 increased from 1.27 to 1.54. The preterm deliveries rate increased from 7.3% in the year 2007 to 8.5% in the year 2010 and decreased to 7% in the year 2018. The perinatal mortality rate decreased from 6.2 in the year 2007 to 4.4 in the year 2017, increased again in the years 2018 to 5.0 and according to the criteria of WHO (World Health Organization) to 6.6 per 1,000 still- and live-births. During the years 2007–2018 at perinatal mortality stillbirth participate with 65%, low birth weight with 63% and severe congenital anomalies with 19%. Transport in utero to perinatological centers in the years 2007–2018 has decreased from 57 to 56% for infants 1,000–1,499 g and from 75 to 73% for infants below 1,000 g. Conclusion: In the year 2017, perinatology in the Slovak Republic reached the best result in the perinatal mortality rate – 4.4‰ (0.44%), but has increased to over 5‰ next year. To further reduce perinatal mortality in the Slovak Republic, it is necessary to improve the prenatal diagnosis of severe congenital abnormalities, transport in utero of very low birth weight fetuses, centralization of high-risk pregnancies, obstetric personnel and material-technical equipment of obstetricians and neonatal intensive care units.

BACKGROUND: To prospectively assess LLL incidence among cervical cancer patients treated by uterine surgery complemented by SLN biopsy, without PLND. METHODS: A prospective study in 150 patients with stage IA1-IB2 cervical cancer treated by uterine surgery with bilateral SLN biopsy. Objective LLL assessments, based on limb volume increase (LVI) between pre- and postoperative measurements, and subjective patient-perceived swelling were conducted in six-month periods over 24-months post-surgery. RESULTS: The cumulative incidence of LLL at 24 months was 17.3% for mild LLL (LVI 10-19%), 9.2% for moderate LLL (LVI 20-39%), while only one patient (0.7%) developed severe LLL (LVI > 40%). The median interval to LLL onset was nine months. Transient edema resolving without intervention within six months was reported in an additional 22% of patients. Subjective LLL was reported by 10.7% of patients, though only a weak and partial correlation between subjective-report and objective-LVI was found. No risk factor directly related to LLL development was identified. CONCLUSIONS: The replacement of standard PLND by bilateral SLN biopsy in the surgical treatment of cervical cancer does not eliminate the risk of mild to moderate LLL, which develops irrespective of the number of SLN removed.

BACKGROUND: SENTIX (ENGOT-CX2/CEEGOG-CX1) is an international, multicentre, prospective observational trial evaluating sentinel lymph node (SLN) biopsy without pelvic lymph node dissection in patients with early-stage cervical cancer. We report the final preplanned analysis of the secondary end-points: SLN mapping and outcomes of intraoperative SLN pathology. METHODS: Forty-seven sites (18 countries) with experience of SLN biopsy participated in SENTIX. We preregistered patients with stage IA1/lymphovascular space invasion-positive to IB2 (4 cm or smaller or 2 cm or smaller for fertility-sparing treatment) cervical cancer without suspicious lymph nodes on imaging before surgery. SLN frozen section assessment and pathological ultrastaging were mandatory. Patients were registered postoperatively if SLN were bilaterally detected in the pelvis, and frozen sections were negative. TRIAL REGISTRATION: ClinicalTrials.gov (NCT02494063). RESULTS: We analysed data for 395 preregistered patients. Bilateral detection was achieved in 91% (355/395), and it was unaffected by tumour size, tumour stage or body mass index, but it was lower in older patients, in patients who underwent open surgery, and in sites with fewer cases. No SLN were found outside the seven anatomical pelvic regions. Most SLN and positive SLN were localised below the common iliac artery bifurcation. Single positive SLN above the iliac bifurcation were found in 2% of cases. Frozen sections failed to detect 54% of positive lymph nodes (pN1), including 28% of cases with macrometastases and 90% with micrometastases. INTERPRETATION: SLN biopsy can achieve high bilateral SLN detection in patients with tumours of 4 cm or smaller. At experienced centres, all SLN were found in the pelvis, and most were located below the iliac vessel bifurcation. SLN frozen section assessment is an unreliable tool for intraoperative triage because it only detects about half of N1 cases.

Cíl práce: Popsat případ echinokokového onemocnění v gynekologické praxi a poukázat na úskalí jeho diagnostiky.Typ studie: Kazuistika. Název a sídlo pracoviště: Gynekologicko pôrodnická klinika, Fakultná nemocnica Trenčín, Slovensko. Vlastní pozorování: Prezentován je případ imunosuprimované pacientky s karcinomatózou peritonea, mírnou elevací Ca 125 a zvýšeným CRP suponující nález gynekologické etiologie s histopatologickým výsledkem a závěrem parazitárního onemocnění - echinokokózy (hydatidózy). Závěr: V diferenciální diagnostice peritoneální karcinomatózy a ascitu zvláště u imunosuprimovaných pacientek s pozitivní sociální anamnézou je třeba zvažovat i možnost raritních parazitárních onemocnění, jako je echinokokóza, která se svými projevy podobá maligním nádorům.
Objective: Describe the case of echinococcal disease in gynecological practice and point out the complications of its diagnosis. Design: Case report. Setting: Department of Obstetrics and Gynaecology, Faculty Hospital Trenčín, Slovakia. Case report: The case is presented in an immunosuppressed patient with peritoneal carcinomatosis, mild Ca 125 elevation and increased CRP presenting as a finding of gynecological etiology with histopathological outcome and conclusion of parasitic disease - echinococcosis (hydatidosis). Conclusion: In the differential diagnosis of peritoneal carcinomatosis and ascites, especially in immunosuppressed patients with a positive social (or epidemiological) history, the possibility of rare parasitic diseases such as echinococcosis, which resembles malignant tumors, should be considered.

The quality of pathological assessment is crucial for the safety of patients with cervical cancer if pelvic lymph node dissection is to be replaced by sentinel lymph node (SLN) biopsy. Central pathology review of SLN pathological ultrastaging was conducted in the prospective SENTIX/European Network of Gynaecological Oncological Trial (ENGOT)-CX2 study. All specimens from at least two patients per site were submitted for the central review. For cases with major or critical deviations, the sites were requested to submit all samples from all additional patients for second-round assessment. From the group of 300 patients, samples from 83 cases from 37 sites were reviewed in the first round. Minor, major, critical, and no deviations were identified in 28%, 19%, 14%, and 39% of cases, respectively. Samples from 26 patients were submitted for the second-round review, with only two major deviations found. In conclusion, a high rate of major or critical deviations was identified in the first round of the central pathology review (28% of samples). This reflects a substantial heterogeneity in current practice, despite trial protocol requirements. The importance of the central review conducted prospectively at the early phase of the trial is demonstrated by a substantial improvement of SLN ultrastaging quality in the second-round review.
