Notwithstanding the fact that streptomycetes are overlooked in clinical laboratories, studies describing their occurrence in disease and potential pathogenicity are emerging. Information on their species diversity in clinical specimens, aetiology and appropriate therapeutic treatment is scarce. We identified and evaluated the antibiotic susceptibility profile of 84 Streptomyces clinical isolates from the Czech Republic. In the absence of appropriate disk diffusion (DD) breakpoints for antibiotic susceptibility testing (AST) of Streptomyces spp., we determined DD breakpoints by correlation with the broth microdilution method and by the distribution of zone diameters among isolates. Correlation accuracy was high for 9 antibiotics, leading to the establishment of the most valid DD breakpoints for Streptomyces antibiotic susceptibility evaluation so far. Clinical strains belonged to 17 different phylotypes dominated by a cluster of strains sharing the same percentage of 16S rRNA gene sequence identity with more than one species (S. albidoflavus group, S. hydrogenans, S. resistomycificus, S. griseochromogenes; 70% of isolates). AST results showed that Streptomyces exhibited intrinsic resistance to penicillin, general susceptibility to amikacin, gentamycin, vancomycin and linezolid, and high percentage of susceptibility to tetracyclines and clarithromycin. For the remaining antibiotics, AST showed inter- and intra-species variations when compared to available literature (erythromycin, trimethoprim-sulfamethoxazole), indicating a region-dependent rather than species-specific patterns.
The Czech National Collection of Type Cultures (CNCTC), based on the results of MALDI-TOF and sequencing analysis, completed the identification of the 16S rRNA genes of nine isolates from field laboratories where standard biochemical methods are used and, therefore, further identification could not be carried out. These referral strains were either only rarely or uncommonly isolated in the context of the reported diagnosis.

Cílem sdělení je prezentace případu 93leté pacientky s postižením v oblasti nosu vyvolaným Nocardia abscessus. Po poranění nosu při pádu na zahradě došlo k rozvoji zánětu měkkých tkání nosu. Kultivace z kožních píštělí prokázala bakterie rodu Nocardia, fenotypovými i genotypovými metodami byla identifikována Nocardia abscessus. Pacientka byla léčena trojkombinací antibiotik a chirurgickou revizí rány. CT vyšetřením nebylo zjištěno postižení kostěných struktur. Autoři poukazují na nezbytnost mikrobiologického vyšetření, fenotypových i genotypových metod identifikace pro cílenou léčbu.
We report a case of unusual microbiological findings. Injury of the nose in the garden was followed by inflammation of the soft tissues. Gram smear showed bacteria of Nocardia sp., subsequent phenotype and genotype analyses identified Nocardia abscessus. CT examination did not reveal any damage to the hard tissues. The patient was treated with combination of antibiotics and surgical debridement. The authors point out the necessity of microbiological phenotypic and genotypic identification of Nocardia species for the purpose of targeted therapy.

Rapidly growing mycobacteria (RGM) inhabit soil and water but certain strains represent a health risk for human and animals. Both clinical and soil RGM may be under selection pressure for resistance to tetracycline (TET) antibiotics, since tetracyclines are administrated to humans and farm animals, and TET residues enter soil through manuring; however, resistance to TET and the presence of TET-resistance genes have been assessed only in clinical isolates. We were therefore interested in comparing soil and clinical RGM in terms of TET resistance and the presence of TET-resistance genes. We used 44 RGM from grasslands with different exposure to animal manure, and 38 clinical RGM from Czech hospitals. There was no difference between the clinical and soil isolates in TET resistance, with >50% resistant isolates in both groups. otr(A), otr(B), tet(K), tet(L) or tet(M) were not detected in any soil or clinical isolate. In contrast, most isolates harbored tet(V) and tap, both encoding mycobacterial efflux pumps, including species where these genes have never been evidenced before. The phylogeny of tet(V) correlated with isolates' BOX-PCR profiles, suggesting that this gene evolved along with mycobacterial genomes as a part of the intrinsic resistome. In certain cases, tet(V) and/or tap were found in TET-sensitive isolates, or inversely, were not found in resistant strains. Concluding, intrinsic efflux pumps may be more important for TET resistance than horizontally transferred genes in both soil and clinical RGM. Their simple presence, however, does not attest to resistance, and therefore their diversity, function and expression merit further research.

V posledních 20 letech vzrůstá počet humánních nokardiových infekcí v důsledku zlepšené diagnostiky a zvyšujícího se počtu imunokompromitovaných pacientů. Klinické manifestace mohou mít formu kožní, plicní, při diseminaci může být postižen kterýkoli orgán včetně mozku. Při diagnostice a léčbě pacientů s nokardiovými infekcemi je nezbytná mezioborová spolupráce. Popisujeme dva případy nemocných s mozkovými abscesy nokardiové etiologie s neurochirurgickým zákrokem.
The recorded incidence of human infections caused by Nocardia spp. has increased over the past 20 years, both in response to improved diagnostic techniques and to rising numbers of immunocompromised patients. Nocardial infections primarily affect the skin and lungs, but the bacteria may disseminate to any organ, including the brain. The diagnosis and treatment of patients suffering from a Nocardia infection require an interdisciplinary approach. Here we describe two case reports of patients suffering from brain abscesses caused by Nocardia that required neurosurgical treatment.

Častými „kontaminantami" zachycenými při mykobakteriologické kultivaci dekontaminovaných vzorků jsou bakterie řádu Actinomycetales. Jsou to obvykle bakterie klasifikované v čeledi Corynehacterineae rodů Corynebacteríum, Dietzia, Gordonia, Nocardia, Rhodococcus a Tsukamurella. Tyto bakterie často kolonizují dýchací cesty a za určitých podmínek mohou vyvolat život ohrožující onemocnění. U pacientů s těžkou imunodeficiencí jsou pravidelným vyvolavatelem život ohrožujících infekcí bakterie rodu Nocardia. Jsou to vláknité bakterie, vytvářející v kultuře vzdušné mycelium, jsou částečně acidorezistentní a jsou rezistentní na lysozym. Vyvolávají nokardiózu, tj. vzácné ale závažné onemocnění pacientů s různými typy imunodeficience. Diferenciálně diagnosticky je třeba odlišit rody Streptomyces, Actinomadura, Nocardiopsis a další půdní saprofyty, které nejsou acidorezistentní, jsou citlivé na lysozym a rychleji rostou. Často kolonizují dýchací cesty u pacientů s postižením plic, onemocnění vyvolávají velmi vzácně. Diagnostika aerobních aktinomycet a vyšetření citlivosti na antibiotika mají svá úskalí vyplývající z delší růstové doby, obtížné barvitelnosti a necharakteristických biochemických reakcí. K přesné identifikaci do rodu a species je nutná polyfázová identifikace izolatu s využitím metod molekulární mikrobiologie. Při včasné diagnóze bývají infekce vyvolané aerobními aktinomycetami dobře léčitelné cílenou terapií antibiotiky.
Frequent "contaminants" detected during mycobacterial culture of decontaminated samples are bacteria of the order Actinomycetales. These are usually bacteria classified as the family Corynebacterineae, genera Corynebacterium, Dietzia, Gordonia, Nocardia, Rhodococcus and Tsukamurella. These bacteria frequently colonize the airways and, under certain circumstances, they may cause life-threatening diseases. In severely immunocompromised patients, they regularly cause life-threatening infections with bacteria of the genus Nocardia. These filamentous bacteria, developing aerial mycelium in the culture, are partly acid-resistant and resistant to lysozyme. They cause nocardiosis, a rare but serious disease in patients with various types of immune deficiency. Differential diagnosis must distinguish between the genera Streptomyces, Actinomadura and Nocardiopsis and other soil saprophytes that are not acid-resistant, sensitive to lysozyme and faster growing. They frequently colonize the airways of patients with lung disease but very rarely cause diseases. The diagnosis of aerobic actinomycetes and determination of their sensitivity to antibiotics are problematic since they grow longer, are difficult to stain and are involved in atypical biochemical reactions. Precise identification of the genera and species requires polyphasic identification of isolates using molecular microbiology methods. If diagnosed early, infections caused by aerobic actinomycetes are easy to treat with targeted antibiotic therapy.
