The objective of this quality evaluation was to evaluate the changes in public health nursing (PHN) interventions after the implementation of an evidence-based family home visiting (EB-FHV) guideline encoded using the Omaha System. Design and sample: This quality improvement evaluation was conducted using a secondary dataset of 27,910 PHN family home visiting interventions from visits to 129 adult clients enrolled in EB-FHV programs in a Midwestern PHN agency. The interventions were documented 12 months before and 14 months after EB-FHV Guideline implementation. The EB-FHV consisted of 94 PHN interventions for 10 Omaha System problems, with electronic health record (EHR) data generated by PHNs during routine clinical documentation. Standard descriptive and inferential statistics were employed in the analysis. Measures: The Omaha System was used to compare PHN practice before and after the guideline implementation. Results: Documentation patterns revealed that PHNs tailored interventions while also shifting toward the use of the EB-FHV guideline interventions. Ten EB-FHV problems accounted for 96.3% of interventions documented before and 98.5% of interventions documented after implementation. The proportion of interventions before and after EB-FHV by problem differed significantly for all problems except Substance use. Fewer interventions were provided after EB-FHV for the primary problems of Pregnancy and Postpartum, with a shift to more interventions for Caretaking/parenting. Conclusion: The PHN documentation demonstrated an adherence to the EB-FHV guideline, while tailoring the evidence-based interventions differentially by problem. Further research is needed to extend this quality improvement approach to other guidelines and populations.

Ve Velké Británii pracuji (M. R.) čtyři roky a nedávno jsem zde úspěšně složila MRCPsych zkoušku (obdoba atestační zkoušky). Během své přípravy jsem měla možnost pracovat jeden a půl roku v komunitních týmech v oblasti Liverpoolu a Birminghamu. Také jsem spolupracovala s krizovými komunitními týmy během svých nočních služeb. S ohledem na to, že připravovaná reforma české psychiatrie do značné míry zrcadlí západní model, článek si klade za cíl osvětlit, jak v praxi reformovaná komunitní psychiatrie funguje, co jsou její výhody a jaké jsou potenciální problémy, na které je třeba myslet při jejím realizování. Jedná se o osobní zkušenost první autorky článku s prací ve Velké Británii, nikoliv o výsledky vlastního výzkumu nebo systematický literární přehled poznatků v dané oblasti.
I (M.R.) have been working four years in the field of psychiatry in the United Kingdom, and have passed my MRCPsych exam recently. During my training, I had the opportunity to work for 1.5 years in community teams in Mersey and West Midlands. I have wide experience in working with crisis teams during the out of hours. As the newly prepared reform of the Czech psychiatry mirrors the British model to some extent, I felt it could be useful to share my experience in working in the UK with my colleagues from the Czech Republic. This article is focused on benefits and drawbacks of the current UK psychiatry system, based on the personal experience of the first author. The content of the article is neither original research, nor a comprehensive literary review.

Mezi hlavní výhody návštěvní služby sestrou v domácím prostředí patří především soukromí pacienta. Pacienti se zotavují lépe doma než v nemocnicích či různých sociálních zařízeních. Pacientům je poskytována péče podle individuálních potřeb dle holistického přístupu. Dalším přínosem domácího ošetřování je ekonomičnost. Sestry pracující u praktického lékaře pro dospělé vykonávající návštěvní službu znají celou zdravotní dokumentaci jejich registrovaných pacientů a též znají jejich rodinnou i sociální situaci a bydlení. Díky těmto aspektům je pak péče o pacienta efektivnější. Dalším pozitivem je také rychlejší návaznost poskytované ošetřovatelské i lékařské péče. Pacienti sestru lépe znají, důvěřují jí a jsou k ní otevřenější. Důležitou markantou je demografický rozdíl v obvodu praktického lékaře pro dospělé, počet registrovaných pacientů a počet zaměstnaných sester.
A significant benefit of the nurse's visits in patient's home is mainly the regard for patient's private space. The patients recover better in home conditions than in hospitals or other social facilities. They are provided with individual care according to their specific needs. The economic factor is also important. The nurses employed in the GP's surgery for adults who regularly visit their patients are familiar with the entire health documentation of the patients and their families, as well as their social conditions. This is why the care is more effective. Another important factor is a faster sequence of nursing and the following medical care. The patients know the nurse well, they trust her and thus they are usually more open towards her.

BACKGROUND: Heart failure (HF) is a chronic disease of great clinical and economic significance for both the healthcare system and patients themselves. AIM: To determine the consumption of medical resources for treatment and care of HF patients and to estimate the related costs. METHODS: The study involved 400 primary care practices and 396 specialist outpatient clinics, as well as 259 hospitals at all reference levels. The sample was representative and supplemented with patient interview data. Based on the consumption of particular resources and the unit costs of services in 2011, costs of care for HF patients in Poland were estimated. Separate analyses were conducted depending on the stage of the disease (according to NYHA classification I-IV). The public payer's perspective and a one year time horizon were adopted. RESULTS: Direct annual costs of an HF patient's treatment in Poland may range between PLN 3,373.23 and 7,739.49 (2011), the main cost item being hospitalisation. The total costs for the healthcare system could be as high as PLN 1,703 million, which is 3.16% of the National Health Fund's budget (Ex. rate from 05.03.2012: 1 EUR = 4.14 PLN). CONCLUSIONS: The costs of treating heart failure in Poland are high; proper allocation of resources to diagnostic procedures and treatment may contribute to rationalisation of the relevant expenditure.
