BACKGROUND: Heart failure (HF) is a chronic disease of great clinical and economic significance for both the healthcare system and patients themselves. AIM: To determine the consumption of medical resources for treatment and care of HF patients and to estimate the related costs. METHODS: The study involved 400 primary care practices and 396 specialist outpatient clinics, as well as 259 hospitals at all reference levels. The sample was representative and supplemented with patient interview data. Based on the consumption of particular resources and the unit costs of services in 2011, costs of care for HF patients in Poland were estimated. Separate analyses were conducted depending on the stage of the disease (according to NYHA classification I-IV). The public payer's perspective and a one year time horizon were adopted. RESULTS: Direct annual costs of an HF patient's treatment in Poland may range between PLN 3,373.23 and 7,739.49 (2011), the main cost item being hospitalisation. The total costs for the healthcare system could be as high as PLN 1,703 million, which is 3.16% of the National Health Fund's budget (Ex. rate from 05.03.2012: 1 EUR = 4.14 PLN). CONCLUSIONS: The costs of treating heart failure in Poland are high; proper allocation of resources to diagnostic procedures and treatment may contribute to rationalisation of the relevant expenditure.

Cíl: Cílem práce byla analýza ročních nákladů na konvenční léčbu jednoho pacienta s fibrilací síní (FS). Metody: Zdravotní údaje o pacientech byly získány pomocí dotazníku rozeslaného kardiologům v České republice. Do studie bylo zahrnuto 306 pacientů (94 žen), průměrného věku 63,7 ± 11,4 let s potvrzenou diagnózou FS, kteří měli standardní léčbu řízenou kardiology a u nichž nebyla provedena katetrizační ablace. Náklady byly hodnoceny z pohledu plátce zdravotní péče. Celkové náklady na léčbu pacientů s FS byly vypočítány ohodnocením částek utracených za diagnostické a léčebné výkony, antiarytmickou a antikoagulační/antiagregační léčbu a hospitalizace a některé další úkony. Výsledky: Celkové náklady na léčbu jednoho pacienta za rok činily 42 406,60 Kč (1 462 eur). Nejdražší byly diagnostické a léčebné výkony (55 %), mezi nimi intervenční výkony, jako je selektivní koronarografie, katetrizační ablace flutteru síní I. typu a implantace kardiostimulátoru pro dysfunkci sinusového uzlu. Hospitalizace činily 27 % celkových nákladů, náklady na antiarytmickou a antikoagulační léčbu představovaly 9 % celkových nákladů. Náklady na hospitalizace a antiarytmickou a antikoagulační léčbu byly nižší než u obdobných studií zabývajících se touto problematikou. Závěr: Náklady 1 462 eur na zdravotní péči u jednoho pacienta s FS za rok odpovídají přibližně polovině jednoročních nákladů ve Francii (okolo 3 000 eur).
The Aim: The aim of the study was to analyze the annual costs of conventional therapy per patient with atrial fibrillation (AF). Methods: The healthcare data of patients were obtained using a questionnaire distributed to cardiologists across the Czech Republic. The study included 306 patients (94 women) with a mean age of 63.7 ± 11.4 years, with a confirmed diagnosis of AF, receiving standard cardiologist-guided therapy, who had not have catheter-based ablation. Costs were analyzed in terms of the healthcare cost payer. Total costs of treatment of AF patients were calculated as the sums spent on diagnostic and therapeutic procedures, antiarrhythmic and anticoagulation/antiplatelet therapy, hospitalization, and some other procedures. Results: Total treatment-related costs per patient per year amounted to 42,406.60 Czech crowns (1,462 euros). The most costly were diagnostic and therapeutic procedures (55%) including interventional procedures such as selective coronary angiography, catheter-based ablation of Type I atrial flutter, and pacemaker implantation for sinus node dysfunction. Hospitalization was accounting for 27% of total costs, the cost of antiarrhythmic and anticoagulation therapy represented 9% of total costs. The costs of hospitalization and antiarrhythmic and anticoagulation therapy were lower compared with those reported by other, similar studies addressing this issue. Conclusion: The annual costs of 1,462 euros for healthcare per AF patient are about a half of those reported from France (approx. 3,000 euros).
