V této kazuistice popisujeme pediatrického pacienta s výskytem vzácného spinálního tumoru, který byl akutně resekován s inoperabilním reziduem. Histologie prokázala ektopický adenom adrenokortikálního typu manifestující se jak neurologickými příznaky z útlaku míchy, tak i endokrinologickými, z důvodu nadprodukce pohlavních hormonů spojených s předčasnou pseudopubertou a urychleným růstem. Pomocí CT a celotělového PET/MRI vyšetřování nebylo nalezeno žádné další ložisko tumoru. Nejednalo se o sporadický výskyt, pacient zdědil patologickou alelu genu pro protein TP53 - Li-Fraumeni syndrom po otcově linii, kde byl zvýšený výskyt malignit v brzkém věku. Pacient je nyní již rok sledován dle standardů pro Li-Fraumeni syndrom. V případě progrese a při pochybnostech o hranici adenom/karcinom bude adjuvantní léčba stavěna na imunoterapii na základě zvýšené exprese PD-L1 prokázané pomocí imunohistochemie.
The aim of this article is to describe a case of the infrequent spinal adenoma in paediatric patient. This adenoma was resected with inoperable residuum and histology confirmed an extremely rare spinal adrenocortical adenoma. Tumor was was presented by symptoms of spinal compression and endocrinological signs of pseudopubertas praecox. There was not any other lesion found via PET/MRI examination. Tumor was not accidental, patient inherited pathological allele of the gene coding TP53 protein from the father line, where was frequent occurrence of malignancies in early age. Understanding principles of Li-Fraumeni syndrome is crucial to assess the effective follow-up. In case of future progression with malignant transformation, we plan to use the immunotherapy to target high expression of PD-L1 proved by immunohistochemistry.

Adrenal incidentalomas (AI) are very common and mostly they are non-functioning adenomas (NFA). NFAs are often associated with insulin resistance and metabolic syndrome. Several biomarkers, including certain growth factors, may participatein the pathogenesis ofmetabolic changes in patients with adrenal adenomas.Patients with NFA and age-matched control subjects were enrolled in the study. Data on age, gender, presence of metabolic syndrome or its components were obtained for each subject. Blood samples were obtained and glycemia, insulinemia, lipid profile, and selected growth factor levels were measured. Forty-three patients with NFA and 40 controls were included in the study. Differences were not found in the metabolic syndrome and its components prevalence or in the biochemical profile between patients and the control group. Significant differences were noticed in the levels of IGF1, IGF2, and IGFBP3 (p=0.016, p=0.005, p=0.004, respectively), but there were no differences in VEGF or EGF concentrations. In NFA patients, an association between glycemia and EGF levels was present (p=0.026). No significant correlations between tumor size and insulin or growth factor concentrations were present in AI patients. Significantly higher serum IGF1, IGF2, and IGFBP3 concentrations in NFA patients may support the role of the IGF axis in the pathogenesis of adrenocortical lesions.No correlation between IGFs or IGFBP3 and parameters of glucose or lipid metabolism was found. Present results may support the role of the growth hormone axis rather than hyperinsulinemia and insulin resistance in the pathogenesis of adrenocortical adenomas.

We report a case of an aldosterone producing adrenocortical adenoma with rhabdoid features in a 16-year-old girl. Grossly, the tumor measured 30 mm in diameter and weighed 24 g. Histologically, the tumor was composed of approximately equal parts of tumor cells with rhabdoid features arranged in a solid and trabecular pattern and cells characterized by compact eosinophilic cytoplasm, solid growth with focal necroses, and increased mitotic activity. The lipid-rich tumor cells with ample clear vacuolized cytoplasm represent a minor component. Immunohistochemically, all the tumor cells showed the same results and were positive for vimentin, synaptophysin, Melan A, and alpha-inhibin. Cytokeratin CAM 5.2 was positive only focally. Chromogranin A, actin, alpha-actin, S100 protein, EMA, and cytokeratin AE1/AE3 were negative. Rhabdoid features have been described in many tumors of variable histogenesis; however, to the best of our knowledge, the presence of rhabdoid phenotype has never been described in either adrenocortical adenoma or carcinoma.
