Maintenance of Na+ and K+ gradients across the cell plasma membrane is an essential process for mammalian cell survival. An enzyme responsible for this process, sodium-potassium ATPase (NKA), has been currently extensively studied as a potential anticancer target, especially in lung cancer and glioblastoma. To date, many NKA inhibitors, mainly of natural origin from the family of cardiac steroids (CSs), have been reported and extensively studied. Interestingly, upon CS binding to NKA at nontoxic doses, the role of NKA as a receptor is activated and intracellular signaling is triggered, upon which cancer cell death occurs, which lies in the expression of different NKA isoforms than in healthy cells. Two major CSs, digoxin and digitoxin, originally used for the treatment of cardiac arrhythmias, are also being tested for another indication-cancer. Such drug repositioning has a big advantage in smoother approval processes. Besides this, novel CS derivatives with improved performance are being developed and evaluated in combination therapy. This article deals with the NKA structure, mechanism of action, activity modulation, and its most important inhibitors, some of which could serve not only as a powerful tool to combat cancer, but also help to decipher the so-far poorly understood NKA regulation.

Cardiotonic steroids (CTS) are steroidal drugs, processed from the seeds and dried leaves of the genus Digitalis as well as from the skin and parotid gland of amphibians. The most commonly known CTS are ouabain, digoxin, digoxigenin and bufalin. CTS can be used for safer medication of congestive heart failure and other related conditions due to promising pharmacological and medicinal properties. Ouabain isolated from plants is widely utilized in in vitro studies to specifically block the sodium potassium (Na+/K+-ATPase) pump. For checking, whether ouabain derivatives are robust inhibitors of Na+/K+-ATPase pump, molecular docking simulation was performed between ouabain and its derivatives using YASARA software. The docking energy falls within the range of 8.470 kcal/mol to 7.234 kcal/mol, in which digoxigenin was found to be the potential ligand with the best docking energy of 8.470 kcal/mol. Furthermore, pharmacophore modeling was applied to decipher the electronic features of CTS. Molecular dynamics simulation was also employed to determine the conformational properties of Na+/K+-ATPase-ouabain and Na+/K+-ATPase-digoxigenin complexes with the plausible structural integrity through conformational ensembles for 100 ns which promoted digoxigenin as the most promising CTS for treating conditions of congestive heart failure patients.

The aim of the study was to compare the adsorption ability of two adsorbent materials, namely diosmectite and activated charcoal towards selected model compounds that are most commonly involved in acute intoxication. Eleven model compounds were selected: acetylsalicylic acid, α-amanitin, amlodipine, digoxin, phenobarbital, ibuprofen, imipramine, carbamazepine, oxazepam, promethazine, and theophylline. Of the tested compounds, promethazine and imipramine were the most effectively adsorbed to diosmectite. Their adsorption to diosmectite (0.356±0.029mg promethazine/mg diosmectite and 0.354±0.019mg imipramine/mg diosmectite, respectively) was significantly higher than their adsorption to activated charcoal. The effect of temperature and pH on the adsorption efficiencies was also evaluated. In the case of experiments with mixture of both adsorbents, they mostly behaved in a solution independently or in a slightly antagonistic way. Using various methods such as N2 adsorption and thermogravimetric analysis, the structure and texture of diosmectite and activated charcoal were attained.

Výtažky z rostlin, obsahujících srdeční glykosidy, byly po staletí podávány nemocným s „vodnatelností“ – s otoky a ascitem. Když William Withering roku 1785 vydal knížku „Pojednání o náprstníku a o některých způsobech jeho užití v medicíně“, věděl, že digitalis je účinný jen u některých případů vodnatelnosti, ale nepředpokládal, že by tento účinek byl následkem působení léku na srdce. Nicméně věděl, že „činnost srdeční je (lékem) ovlivněna”. I když klinická pozorování, výsledky pokusů na zvířatech i data z hemodynamických studií prokazovaly účinnost digitalisu v léčbě srdeční slabosti, vznikaly opakovaně spory o účinnosti digitalisu a jeho správné indikaci. Na konci minulého století, kdy nebylo o účinnosti digitalisu pochybností, byla předmětem sporu otázka, zda se má digoxin podávat i pacientům se sinusovým rytmem, když jsou léčení ACE-inhibitory, a zda jím mají být léčeni i pacienti – sice symptomatičtí – ale s normální systolickou funkcí. Roku 1993 byly publikovány dvě studie, které ukázaly, že stav pacientů se zhoršuje, je-li digoxin vysazen. Studie RADIANCE ukázala, že po přerušení podávání digoxinu progreduje symptomaticky srdeční slabost, sníží se ejekční frakce a klesne výkon srdeční při zátěži. Studie PROVE přinesla průkaz účinku digoxinu u pacientů se systolickou dysfunkcí a sinusovým rytmem. V roce 1997 byly uveřejněny výsledky studie DIG. Výsledky ukázaly, že digoxin snižuje častost hospitalizací, ale nemá signifikantně příznivý vliv na dobu přežití. Nové vyhodnocení této studie publikované na začátku roku 2006 (Ahmed et al.) nicméně ukázalo, že digoxin redukuje jak mortalitu, tak častost hospitalizací u všech pacientů včetně těch, kteří mají normální systolickou funkci, je-li podáván v dávkách 0,125 až 0,250 mg, jimiž se většinou dosahuje sérové koncentrace 0,5–0,9 ng/ml. Autoři na základě těchto dat doporučují, aby byl digoxin podáván již před přidáním ACE-inhibitorů, blokátorů AT1–receptorů, velkých dávek diuretik a před rozhodnutím o eventuální resynchronizační léčbě. Tím se přiklánějí k původnímu fyziologickému konceptu léčby srdeční insuficience, který byl s úspěchem praktikován před zavedením diuretik k perorální léčbě a je též základem Braunwaldova algoritmu (1980) – začíná se omezením tělesné námahy a podáváním digoxinu. Tento postup je logický a je spojen s minimálními náklady na léčení.
Several plant extracts containing cardiac glycosides have been used in the treatment of dropsy (oedemas) for many centuries. When William Withering 1785 published his book „An account of the foxglove and some of its medical uses“, he was aware that digitalis is effective only in certain medical forms of dropsy but did not associate this effect with the cardiac action of the drug. However, he recognised that „the motion of the heart was affected“. Although clinical experience, experimental data and haemodynamic studies supported the use of digitalis for the treatment of heart failure, repeated controversies surrounded its efficacy and appropriated indication. At the end of the last century, the issue was not whether digoxin is effective, but whether it has a role in patients with sinus rhythm treated with ACE-inhibitors and in heart failure with preserved systolic function. In 1993, two studies that assessed the effect of digoxin withdrawal noted adverse effects after discontinuation: In the RADIANCE Study, digoxin withdrawal was associated with significant worsening of heart failure and decrease in exercise capacity and LVEF, and the PROVE Trial provided strong evidence of digoxin efficacy in patients with systolic dysfunction and sinus rhythm. In the DIG Study (1997) digoxin reduced the rate of hospitalisations but did not significantly improve the survival. The reevaluation of this study by Ahmed et al. (2006), however, demonstrated that digoxin reduces mortality and hospitalisation rate in all patients including those with preserved systolic function if used in doses of 0,125–0,250 mg reaching the SDC of 0,5–0,9 ng/ml. The authors conclude that digoxin should be prescribed before adding ACE-inhibitors, angiotensin receptor blockers, aggressive diuretic therapy and before considering cardiac resynchronisation therapy, thus rehabilitating the physiological approach to the treatment of chronic heart failure, starting the treatment by restriction of physical activity and digitalis, as practised before introduction of oral diuretics, and newly suggested by Braunwald 1980. It is a logical inexpensive approach.
Digoxin je jedním z nejstarších užívaných léčiv v kardiologii a své místo v léčbě srdečních onemocnění si zachoval dodnes. Jeho použití je vhodné u nemocných se srdečním selháním s fibrilací síní, ale i u nemocných se sinusovým rytmem. Dle závěrů velké, kontrolované, randomizované studie DIG nesnižuje, ale ani nezvyšuje mortalitu, významně zlepšuje kvalitu života a snižuje počet nutných hospitalizací. Použití digoxinu jako antiarytmika je omezeno na kontrolu srdeční frekvence u některých nemocných s fibrilací síní. V jiných indikacích se náprstníkové glykosidy v současnosti nepoužívají.
Digoxin is one of the oldest used medicaments in cardiology, and it has still retained its place in the treatment of heart disease. It can be used for patients with heart failure and atrial fibrillation as well as for patients with sinus rythm. According to the conclusions of a large randomised, placebo-controlled study digoxin neither decreases nor increases mortality, but greatly improves the quality of life and lowers the number of necessary hospitalizations. The use of digoxin as an antiarrythmic is for the strict control of the heart rate of patients with atrial fibrillation. At present no other indications for the use of the foxglove glycosides are indicated.
