Despite the adenoids are regularly removed in patients with mucopolysaccharidoses (MPS), the underlying tissue and cellular pathologies remain understudied. We characterized an (immuno)histopathologic and ultrastructural phenotype dominated by lysosomal storage changes in a specific subset of adenotonsillar paracortical cells in 8 MPS patients (3 MPS I, 3 MPS II, and 2 MPS IIIA). These abnormal cells were effectively detected by an antibody targeting the lysosomal membrane tetraspanin CD63. Important, CD63+ storage vacuoles in these cells lacked the monocytes/macrophages lysosomal marker CD68. Such a distinct patterning of CD63 and CD68 was not present in a patient with infantile neurovisceral variant of acid sphingomyelinase deficiency. The CD63+ storage pathology was absent in two MPS I patients who either received enzyme-replacement therapy or underwent hematopoietic stem cells transplantation prior the adenoidectomy. Our study demonstrates novel features of lysosomal storage patterning and suggests diagnostic utility of CD63 detection in adenotonsillar lymphoid tissue of MPS patients.

BACKGROUND: Mucopolysaccharidosis IVA (MPS IVA), or Morquio A syndrome, is a rare inherited metabolic disorder caused by deficiency of the lysosomal enzyme N-acetylgalactosamine-6-sulfatase. A progressive systemic skeletal chondrodysplasia, leading to significant morbidity and reduced life expectancy is the main clinical feature of this multisystemic disease. Although enzyme replacement therapy with elosulfase alfa is established in Europe, the rarity of disease and other factors still set hurdles in having patients treated in some countries. Aim of this statement is to provide evidence-based guidance for the enzyme replacement treatment of Morquio A patients, harmonizing recommendations from published guidelines with the real-life clinical practice in the Central and South-Eastern European region. PARTICIPANTS: The Consensus Group, convened by 8 Steering Committee (SC) members from 7 Central and South-Eastern European countries, consisted of a multidisciplinary group of 17 experts in the management of MPS in Central and South-Eastern Europe. CONSENSUS PROCESS: The SC met in a first virtual meeting with an external scientific coordinator, to discuss on clinical issues to be analyzed in guidance statements. Statements were developed by the scientific coordinator, evaluated by the SC members in a first modified-Delphi voting and adapted accordingly, to be submitted to the widest audience in the Consensus Conference. Following discussion and further modifications, all participants contributed to a second round of modified-Delphi voting. RESULTS: Nine of ten statements, concerning general guidelines for management of MPS IVA patients and specific recommendations for treatment, received final consensus. CONCLUSIONS: European guidelines and evidence-based recommendations for Morquio A patients should be considered in the real life of Central and South-Eastern European countries and adapted to unique clinical practice approaches and criteria for patients' access to treatment and reimbursement in the region.

BACKGROUND: Mucopolysaccharidoses (MPS) are a group of lysosomal storage disorders caused by defects in genes coding for different lysosomal enzymes which degrade glycosaminoglycans. Impaired lysosomal degradation causes cell dysfunction leading to progressive multiorgan involvement, disabling consequences and poor life expectancy. Enzyme replacement therapy (ERT) is now available for most MPS types, offering beneficial effects on disease progression and improving quality of life of patients. The landscape of MPS in Europe is not completely described and studies on availability of treatment show that ERT is not adequately implemented, particularly in Southern and Eastern Europe. In this study we performed a survey analysis in main specialist centers in Southern and Eastern European countries, to outline the picture of disease management in the region and understand ERT implementation. Since the considerable number of MPS IVA patients in the region, particularly adults, the study mainly focused on MPS IVA management and treatment. RESULTS: 19 experts from 14 Southern and Eastern European countries in total responded to the survey. Results outlined a picture of MPS management in the region, with a high number of MPS patients managed in the centers and a high level of care. MPS II was the most prevalent followed by MPS IVA, with a particular high number of adult patients. The study particularly focused on management and treatment of MPS IVA patients. Adherence to current European Guidelines for follow-up of MPS IVA patients is generally adequate, although some important assessments are reported as difficult due to the lack of MPS skilled specialists. Availability of ERT in Southern and Eastern European countries is generally in line with other European regions, even though regulatory, organizational and reimbursement constrains are demanding. CONCLUSIONS: The landscape of MPS in Southern and Eastern European countries is generally comparable to that of other European regions, regarding epidemiology, treatment accessibility and follow up difficulties. However, issues limiting ERT availability and reimbursement should be simplified, to start treatment as early as possible and make it available for more patients. Besides, educational programs dedicated to specialists should be implemented, particularly for pediatricians, clinical geneticists, surgeons, anesthesiologists and neurologists.

Mukopolysacharidózy (MPS) jsou lyzosomální střádavá onemocnění způsobená nedostatečnou aktivitou některého z lyzosomálních hydrolytických enzymů podílejících se na postupné degradaci glykosaminoglykanů (původně mukopolysacharidů). Mají řadu společných rysů, jednotlivé typy se však vzájemně odlišují. Klinické projevy onemocnění se vyvíjejí postupně a jsou závislé na typu MPS i na její tíži. Patří mezi ně hepatomegalie/splenomegalie, pupeční a/nebo tříselná kýla, kraniofaciální dysmorfie ve smyslu hrubých rysů obličeje, opakované infekty horních cest dýchacích, záněty středouší, poruchy sluchu, chlopenní vady či kombinované plicní postižení. Konstantní je kostní postižení s poruchou růstu, s deformitami páteře a s charakteristickým rentgenovým nálezem dysostosis multiplex. Variabilní je kognitivní postižení. Výraznou změnu v prognóze pacientů s MPS přineslo zavedení enzymové substituční terapie ((enzyme replacement therapy, ERT). Tato léčba zmírňuje viscerální postižení a zlepšuje kvalitu života pacientů, neovlivňuje však postižení CNS. Pro maximální účinek enzymové substituční terapie je důležitá časná diagnostika a časné zahájení léčby.
Mucopolysaccharidoses (MPS) are lysosomal storage disorders caused by low activity of some of lysosomal hydrolytic enzymes degrading the glycosaminoglycans (originally mucopolysaccharides). All forms of the disease share a lot of common features, although particular MPS types differ one from each other. Clinical signs evolve gradually and depend on the MPS type and its severity. They include hepato-/splenomegaly, umbilical and/or inguinal hernia, craniofacial dysmorphy with coarse facial features, recurrent upper respiratory tract infections, middle ear infections, hearing disorder, cardiac valve involvement or combined pulmonary involvement. Bone involvement with short stature, backbone deformities and characteristic X-ray changes outlined as dysostosis multiplex are constant features. Cognitive impairment is variable. The enzyme replacement therapy (ERT) has brought a significant change to the prognosis of patients with MPS. It improves the visceral involvement together with the patients‘ quality of life. It has no effect upon CNS impairment. The early diagnosis and therapy introduction is essential for its maximal benefit.

Lysosomální střádavá onemocnění (LSDs, Lysosomal Storage Diseases) tvoří skupinu více než 70 vzácných dědičných metabolických onemocnění, jejichž příčinou je nejčastěji porucha funkce některého z lysosomálních enzymů. Cílem přehledového sdělení je představení enzymové substituční terapie (ERT, Enzyme Replacement Therapy), která si našla v terapii LSDs zásadní místo. ERT je založena na náhradě defektního enzymu pomocí rekombinantního proteinu podávaného v 2–4hodinové intravenózní infuzi jednou za 1 či 2 týdny. Tato léčba je v současné době k dispozici pro sedm LSDs – Gaucherovu chorobu, Fabryho chorobu, Pompeho chorobu a mukopolysacharidózu typu I, II, IVA a VI. ERT významně ovlivňuje viscerální manifestaci onemocnění (hepatomegalie, splenomegalie, vitální kapacita plic, svalová slabost, anémie, gastrointestinální obtíže aj.), není ovšem účinná v terapii postižení centrálního nervového systému (CNS) a pouze malý efekt je pozorován u postižení kosti, chrupavky a srdečních chlopní. Časně zahájená ERT je schopna zamezit manifestaci téměř všech symptomů onemocnění, vyjma postižení CNS. U části pacientů se na podávaný enzym může rozvinout imunitní reakce s klinickými projevy alergie a sníženého efektu terapie. Stručně jsou zmíněny i další modality léčby LSDs: transplantace hematopoetických kmenových buněk, substrát redukující terapie, použití chaperonů a genová terapie. Závěr: ERT zpomaluje progresi onemocnění a výrazně mění jeho přirozený průběh, zlepšuje kvalitu života a prodlužuje přežití pacientů. Omezení jsou daná špatným průnikem do některých tkání, možnými alergickými reakcemi a vysokou finanční náročností. Zásadním předpokladem pro úspěch ERT je její časné zahájení při rychlé diagnostice onemocnění.
Lysosomal storage diseases (LSDs) form a group of more than 70 rare inherited metabolic diseases, usually caused by a malfunction of some of the lysosomal enzymes. The aim of this review is to introduce enzyme replacement therapy (ERT) which gained an indispensable role in the therapy of LSDs. ERT is based upon replacement of the defective enzyme by a recombinant protein administred in a 2–4 hour infusion once a week or once a fortnight. This therapy is currently available for seven LSDs: Gaucher disease, Fabry disease, Pompe disease and mucopolysaccharidoses I, II, IVA and VI. ERT significantly affects visceral manifestations of the diseases (hepatomegaly, splenomegaly, vital lung capacity, muscle weakness, anaemia, gastrointestinal disorders etc.), yet it is not effective in treating the central nervous system (CNS) involvement and only a small effect is observed in treating disease manifestations in bones, cartilage and heart valves. An immune reaction with clinical presentation of an allergic reaction and therapy effectiveness decrease can occur with a portion of patients. Other modalities for LSDs therapy are concisely mentioned: haematopoietic stem cell transplantation, substrate reduction therapy, chaperons and gene therapy. Conclusion: ERT slows the progression of the disease and markedly alters its natural course, improves quality of life and prolongs patients’ lifespan. The limitations include insufficient effect in some tissues, possible allergic reactions and great financial demands. An essential presumption for effectiveness and success of ERT is its early initiation following quick diagnosis of the disease.
