A recently published meta-analysis of 21 studies confirmed a protective effect of tubal ligation on the risk of invasive ovarian cancer. This protective measure has received little attention, particularly due to the elusive underlying mechanism. In this commentary we discuss available data concerning the contribution of different potential mechanisms by which tubal ligation might prevent ovarian cancer. Included were studies published in English, identified through a literature search using PubMed and EMBASE. Four main mechanisms are discussed: (a) a screening effect, (b) alteration of ovarian function, (c) a mechanical barrier against ascending carcinogenic agents and (d) prevention of endometrial and proximal Fallopian tube cell ascent. There are arguments supporting the major role of a mechanical barrier to the ascent of endometrial cells into the peritoneal cavity for decreasing risk of ovarian cancer after tubal ligation. Prevention of retrograde transport of carcinogenic substances from the vagina may be an additional mechanism.

Cíl: Demonstrovat případ levostranné torze adnex způsobené současným výskytem dvou benigních teratomů - vaječníku a vejcovodu. Typ studie: Kazuistika. Název a sídlo pracoviště: Gynekologicko-porodnická klinika 1. LF UK a VFN, Praha. Kazuistika: 24letá nuligravida s několikadenní anamnézou bolestí v levém podbřišku a na ultrazvuku s nálezem semicystického útvaru v oblasti levých adnex byla indikována k laparoskopii s podezřením na teratom ovaria. Při ní byl zjištěn koetánní teratom levého ovaria a současně torze levého vejcovodu dilatovaného rovněž dermoidem. Byla provedena levostranná salpingektomie a enukleace teratomu z ovaria. Tento překvapivý nález byl potvrzen histopatologem. Závěr: Náhlá torzní gynekologická příhoda může být způsobena nejen torzí děložního myomu či tumorem zvětšeného ovaria, ale vzácně i torzí dermoidu lokalizovaném ve vejcovodu.
Objective: The authors demonstrate the case of left adnexal torsion caused by 2 benign teratomas in its ovarian and tubal location. Design: Case report. Setting: Department of Obstetrics and Gynecology, 1st Faculty of Medicine and General Teaching Hospital, Charles University, Prague. Case: 24-years old woman underwent laparoscopy for several days lasting pain in lower abdomen and ultrasound finding of semicystic mass in the left adnexal region with suspicion of teratoma. The benign teratoma of the left ovary together with a torsion of the left Fallopian tube distended by teratoma was revealed during surgery. Left side salpingectomy and enucleation of the ovarian teratoma has been performed. This surprising finding was confirmed by histopathological examination. Conclusion: The acute gynaecological torsion event may be caused not only by torsion of uterine fibroid or enlarged tumorous ovary but rarely also by a teratoma of the Fallopian tube.

OBJECTIVE: To study the localization of blood vessels within a tumor and the shape of the flow curve as a method of assessing ovarian neoplasms. METHODS: We studied 39 patients with malignant tumors and 63 patients with benign ovarian tumors by means of vaginal color Doppler ultrasound, noting the localization of blood vessels in the tumors, the shape of the flow curve, and peripheral resistance. RESULTS: Blood vessels could be visualized in 95% of the malignant tumors and in 70% of the benign tumors. Blood vessels tended to be localized centrally (65 versus 5%) in malignant tumors and peripherally in benign tumors (65 versus 0%). A diastolic notch was seen in 89% of the benign tumors, but in none of the malignant tumors. The mean resistance index (RI) +/- standard deviation was 0.48 +/- 0.19 in malignant and 0.69 +/- 0.09 in benign tumors (P < .05). The corresponding values for the pulsatility index (PI) were 0.56 +/- 0.13 and 1.06 +/- 0.07, respectively (P < .01). CONCLUSIONS: Low RI and PI values are general indicators of tumor growth. The localization of blood vessels within an ovarian tumor and the presence or absence of a diastolic notch are the most useful variables in the evaluation of ovarian tumors.
